RESUMO

Acute purulent meningitis of the newborn and infant still raise difficult diagnosis problems due to the often misleading onset, and require an energetic and early intervention, adapted, as much as possible, to the etiologic aspects. The study reports on 90 cases of acute purulent meningitis in the newborn and infant, admitted to the pediatric ward of the Bistrita County Hospital, for 6 years (1983-1988). The clinical and biological aspects were various, sometimes blurred by the antibiotic administration. The study of CSF was the only valuable and compulsory parameter for supporting the diagnosis of meningeal infection and its etiology. In 63.3% cases, the meningeal infection was secondary to a primary infectious focus. Living gram-negative bacilli had the most frequent etiology, both in the newborn and in the infant. Meningitis with nonspecific etiology still hold 45.5%, and the most useful therapy in this situation seemed to be ampicillin + gentamicin + Biseptol and ampicillin + chloramphenicol. The age of the newborn and infant, the male sex, the biologic defects, the type and virulence of the etiologic agent were favouring factors in the appearance and severe evolution of the meningeal infection. Mortality was of 18.8% and the early found sequelae 8.8%. The evolution was favourable, when the diagnosis was precocious and the therapeutic intervention was energetic.

RESUMO

A total of 59 clinical observations were analyzed, dealing with perforated post-bulbar ulceration. The patients were hospitalized and underwent surgery in the Clinic for Surgery of the Emergency Hospital from Bucharest between 1968 and 1979. The frequency of the perforated post-bulbar ulcer, compared to the total number of patients operated for perforated bulbar ulcer over the same period of time was of 1.41%. The important factors were analyzed, which have influenced the clinical evolution and the surgical treatment. These are : age, the interval between the onset of the symptoms and the time of hospitalization, the symptoms presenting on hospitalization, the anatomo-clinical forms which were encountered during surgery, as well as the type of the surgical interventions. The surgical interventions were practiced in accordance with the general and the local conditions of each patient. A death-rate of 5.08% was recorded in the group.

RESUMO

The authors present 83 cases of associated ulcer and cirrhosis, evaluating the frequency of this complication in relation with the total number of ulcers hospitalized over a determined time. The percentage of this association was of 1,6 of the total number of patients with ulcers, and of 10,2 of the total number of patients with cirrhosis. The major complications of this association were hemorrhage and perforation, both in an identical percentage (38). In the case of hemorrhage the authors recommend extirpation of the ulcer. In situ hemostasis should be avoided, when possible because the risk of continued hemorrhage is greater than that of gastric resection. When hemorrhage is due to ruptured oesophageal varicosities endocardial transgastric ligature is recommended by the abdominal route, according to the Skinner-Suteu procedure. In perforated ulcers excision is recommended, associated with pyloroplastia and vagotomy.

Sera from 544 hospital personnel members with different degrees of exposure to the agent of hepatitis B were tested for HB8Ag and anti-HB8 by counterelectrophoresis, radioimmunoassay and passive hemagglutination. The laboratory staff and persons having direct contact with patients exhibited a significantly higher prevalence of anti-HB8 (16.2%) as compared with the other groups: administrative staff (4.4%), healthy persons (4.6%) and blood donors (7.9%). These data prove the relationship between exposure to HB8Ag and anti-HB8 prevalence, more obvious in the laboratory staff handling human serum and plasma specimens (anti-HB8 incidence--25%).

RESUMO

Through 35 days Mecadox was admixed to feed rations for chicken, 40 days of age, and inhibited immunogenesis which, generally, is reflected in lowering of antibody titre and gamma globulin level as well as in quantitative variation of the differential leucocyte count. Change of immune reactivity in chicken treated with Mecadox and deviation from the normal are attributable to changes in the organs which are responsible for immunological processes (thymus and bursa), such as reduction of ascorbic acid levels in the adrenal gland.
