RESUMO

Basis on study through integrated comparative assessment of clinical, biochemical survey data revealed that in patients with impaired metabolism of uric acid in a greater percentage of common biliary sludge, a violation of the rheological properties of bile, a violation of cholate-cholesterol ratio index, which indicates an increased risk of bile stones. The study found that despite the high levels of uric acid there is a violation of the spectrum of bile acids, cholic and deoxycholic growth acid reduction taurocholic acid. Thus, application of ursodeoxycholic acid, rosuvastatin and allopurinol in these study patients with NAFLD dosages in combination with hyperuricemia improves the clinical symptoms and normalization of biochemical parameters and normalizes the spectrum of biliary acids.

RESUMO

Quantum pharmacology allows to study the mechanisms of action of cardiovascular drugs, to predict pharmacological activity and identify the most pronounced pharmacodynamic efficacy and therapeutic activity of new compounds. Calculation of quantum-pharmacological parameters for molecules of beta-blockers (propranolol, atenolol, metoprolol, carvedilol) in aqueous media, research its hydrophobic interaction with receptors allow to form a theoretical basis for the development of new generations of more effective and safe medicines for hypertension treatment. Increased hydrophobicity leads to poor solubility of carvedilol in water and high--in the lipids. The clinical pharmacology of the drug is shown by such indicators as the therapeutic dose, half-life and degree of metabolism in the liver. Due to enhanced interaction with adrenergic receptor effective dose of carvedilol is an order of magnitude lower than other beta-blockers, even with the relatively low bioavailability. Reduced bioavailability of carvedilol versus atenolol, metoprolol and propranolol is caused by elevated metabolism during the first pass through the liver, which is also due to the hydrophobicity of the drug. High solubility in lipids appears to extend the half-life of carvedilol. QSAR studies make an important contribution to the study of the properties of chemical compounds and their pharmacological activity. Software, used for computation of studied properties, has a significant role. A large number of descriptors allows a qualitative and quantitative assessment of the molecules of chemical compounds and prediction of their influence on cardiovascular system.

The article is devoted to features of mikrotsirulyatory changes in the gastric mucosa in patients with chronic H. pylori-associated erosive gastritis with non-alcoholic steatohepatitis after eradication therapy with and without addition of pentoxifylline. It is proved that, when added to eradication therapy pentoxifylline, there is a more effective speaker and the statistical significance of the normalization of microcirculatory changes in the mucosa of the body and antrum as stasis, thrombosis, microbleeds and expand the lumen of the capillaries and venules. Also significantly more often to increase in the number of vessels in the mucous membrane of the body and antrum of the stomach, the average size and the relative amount of blood vessels in the mucous membrane of the antrum.

RESUMO

In the study of psychological status of patients with different clinical variants of irritable bowel syndrome we found each variant to be characterized by worry, but the anxiety is typical only of IBS with diarrhea, while depression is typical of IBS with constipation in contrast to other IBS variants (nonspecific IBS and mixed IBS ones).

RESUMO

In the spontaneous hypertensive rats (SHR)--line NISAG--most cardiac hystiocytes of the left ventriculum have signs of ultrastructure damage to a variable extent. First of all the contractile and energetic function of cardiac hystiocytes has different abnormalities. Administration of Lipril (20 mg/kg daily, during three months) showed the normalization of the ultrastructure of myocardium of SHR. This was evident through decreased in overcontractility of muscular fibrils till complete or partial disappearance of structuring sarcomers. Most mitochondria preserve ultrastructural organization. There are practically no damages in the endothelium, except non-significant vasoconstriction. The comparative analysis of the ultrastructural changes of cardiac hystiocyte of SHR after 3 months of treatment of Lipril showed that current medication has the recoverable effect on the structural organization of cardiac hystiocytes, mitochondrion and blood vessels.

An analysis of risk factors (kitchen salt overconsumption, smoking, overweight, stress, alcohol abuse, hypokynesia) affecting the development and course of arterial hypertension was carried out. Consideration of these risk factors in the treatment of patients with arterial hypertension makes pharmacotherapy of this disease more effective.

RESUMO

A total of 32 patients with chronic antral gastritis without erosion were examined together with 25 patients with chronic erosive antral gastritis. The conclusion reached was that in the above conditions, one could see different strains of Helicobacter pylori. In chronic erosive antral gastritis vs erosion-free chronic antral gastritis H. pylori are found with higher frequency in the antral segment of the stomach, the mucous membrane exhibiting a higher degree of infectious lesion, gastroesophageal reflux occurring far more frequently.

RESUMO

The aim of the study was gaining insights into the peculiarities of the general adaptive reactions (GARs) dynamics during the treatment of duodenal peptic ulcer disease (DPUD) in servicemen of regular army as well as to compare the GARs spectra in the groups of patients depending on the following: disease severity, efficacy of Helicobacter pylori-eradicative therapy and rate of ulcer healing. It was also the purpose of the study to elucidate both the GARs prognostic value in the duration of the disease and the expediency of their correction in the complex treatment of this category of patients. Examined in the study were 70 servicemen. It has been established that both the disease severity and the efficacy of therapy are closely linked with the character of GARs; so we concluded that the study of the GARs dynamics in servicemen with DPUD makes it possible to follow up the duration of the disease as well as to correct the complex therapy and estimate the prognosis of the disease. The expediency of GARs correction in patients with duodenal ulcer has been ascertained.

As may as 155 patients with exacerbated osteoarthrosis were examined to compare the incidence of development of erosive-and-ulcerous lesions in the gastroduodenal system under exposure to different schemes of nonsteroid-antiinflammatory drug (NAID)-therapy and over different time ranges of its administration and to assess prophylactic efficacy of the drug diclobrou (enterosoluble form of diclofenac natrium). There has been endoscopically validated usefulness of employing enterosoluble and injectable dosage forms during the first days of NAID-therapy. With signs of synovitis still in place, subsequent therapy treatments are to be administered with selective inhibitors of cyclooxigenase-2 or a simple dosage form of diclofenac should be combined with misoprostol.
