RESUMO

This young cardiac transplant patient developed painless acute pancreatitis within 10 days of implantation of a biventricular assist device (BIVAD). Historical, physical, laboratory, and imaging data allowed conservative management leading to a favorable outcome. Acute pancreatitis after cardiac transplantation is common with a significant mortality rate. Immunosuppression may play an important role in this process as well as infectious and pancreaticobiliary etiologies. Whereas acute pancreatitis is a well-documented complication of cardiac transplantation, this event has not previously been reported in patients who have received a BIVAD. The mechanism by which BIVAD placement may result in pancreatitis is unknown.

RESUMO

The rapid growth of rural enterprises has transformed the Chinese countryside. Although rural industrialisation has resulted in increased financial well-being, it has also contributed to decreased environmental quality. While China has strong environmental protection laws, this paper will demonstrate that they are not being effectively implemented in a rural region in Zhejiang Province. This is due to a number of social, political, and economic barriers that prevent agencies from effectively enforcing environmental policies and regulatory mechanisms. This paper investigates the implementation of China's environmental policies through a case study in Yuhang County, Zhejiang Province. It demonstrates that the implementation of environmental impact assessment, discharge fees, and limited time treatment is limited by inadequate technology, low finances, limited human resources, poor public environmental awareness, faulty data, inferior agency reports, organizational conflict, relations based on guanxi, and low discharge fee prices.

The long-term outcome of total knee arthroplasty and femoral or tibial osteotomy is related to the ability of the surgeon to achieve the desired alignment based on preoperative planning. This study evaluates the effect of axial rotation on measured tibiofemoral angles and the angle formed between the anatomic and mechanical axes of the femur in lower extremities with valgus and varus deformities. A comparison study of the measured tibiofemoral angles indicated a statistically significant effect in models with severe vagus or varus deformity when rotated 10 degrees internally or externally. In the second part of the study, the measured angle between the anatomic and mechanical axes of the femur never varied by more than 1 degree, despite a 40 degrees are of rotation. The results of the study indicate the tibiofemoral angle measurements are more sensitive to axial limb rotation in lower extremities with valgus or varus deformity than are normally aligned limbs. In preoperative planning of total knee arthroplasty, the measured angle between the anatomic and mechanical axes of the femur is less effected by limb rotation, regardless of the degree of valgus or varus deformity.

To determine whether cerebrospinal fluid (CSF) Cl- has a role in determining the stimulus to the central respiratory chemoreceptors under conditions of constant CSF pH, CO2, and HCO3- concentrations, the ventral medullary surface of the anesthetized rat was perfused with mock CSF of various ion composition and pH. Four mock CSF perfusates were used: two normal pH control perfusions and two acidic solutions. One acidic perfusate was formulated in the traditional manner by substituting Cl- for HCO3-. The second acidic perfusate, and one of the normal pH control perfusates, had approximately 15% of the Cl- replaced with isethionate, an impermeant strong anion. When the two acidic solutions were perfused over the ventral medulla, consistently larger increases in both tidal volume and minute ventilation were observed with the isethionate-containing acidic solution, despite conditions of identical pH and PCO2. The unequal ventilatory effects of the two acidic perfusions suggest that Cl- transport may be a factor determining the stimulus to the central respiratory chemoreceptors.
