RESUMO

Objective: On 6 February 2023, 2 earthquakes with magnitudes of 7.7 and 7.6 mW occurred in Pazarcik and Elbistan districts of Kahramanmaras province (Turkey) and affected 11 provinces in total, especially Hatay, Malatya, and Adiyaman. Here, we report 3 earthquake victims in order to define patients profiles. Case reports: Three patients who were taken to the orthopedic and neurological rehabilitation program in our inpatient Physical Medicine and Rehabilitation unit after the earthquake are presented. Our first case, a 46-year-old man, remain-ed in the debris for 8.5 h. His left leg was left in the wreckage, and he later developed a drop foot on the left due to compartment syndrome, which developed with pain and swelling in the left calf. There is no fasciotomy procedure. Venous Doppler was reported to be within normal limits. Our second case is a 52-year-old man who was in the debris for 36 h and has a left braxial plexus injury and a left drop foot. He has Buerger's disease in his medical history. The third case is an 8-year-old girl trapped in rubble during the earthquake with a right orbital fracture and a left foot transmetarsal amputation. She has no neurological signs. Discussion: The patients that apply to our Physical Medicine and Rehabilitation clinic are especially amputees, patients with traumatic brain injury, spinal cord injury, peripheral nerve damage, plexus damage, multiple fractures, joint limitations and soft tissue loss in the musculoskeletal system. After the earthquake, we encountered patients from a wide variety of spectrums in our clinic. In addition to primary musculoskeletal injuries, they also experience systemic problems affecting the musculoskeletal system. Additionally, thrombosis, infection, renal failure, and multiple organ failures may also occur. Conclusion: Countries should develop guidelines for disaster preparedness and establish coordination units that can take quick action, make decisions, and communicate in times of disaster. A good definition of patient clinical profiles after the earthquake will ensure early intervention and prevent permanent disability and functional losses.

Objectives: This study aims to evaluate the efficacy and safety of thiocolchicoside (TCC) ointment treatment compared to placebo in patients with chronic mechanical low back pain (LBP) accompanied by acute muscle spasms. Patients and methods: A total of 292 adult patients (106 males, 186 females; mean age: 38.5±11.2 years; range, 18 to 64 years) were randomized to TCC group (n=147) and placebo group (n=145) in 12 centers between March 2020 and March 2021. Eight patients from each group were excluded from the analysis. The primary endpoint was pressure pain threshold (PPT) on Day 3, which was measured using a pressure algometer. Secondary endpoints were PPT on Day 7, patient, and physician Visual Analog Scales-pain (VAS-pain) on Days 3 and 7, and safety. Results: The PPT values on Day 3 was not significantly different between the treatment groups (p=0.701). Similarly, TCC and placebo group had similar VAS-pain scores over trial period (p=0.577 or higher for comparisons). Significantly higher PPT values and lower VAS-pain scores on Days 3 and 7 were observed in both groups (p<0.001 for all). In patients with a PPT value of ≥3.87, TCC arm had higher PPT on Day 3 compared to placebo (p=0.029). Three patients (two in the TCC arm and one in the placebo arm) discontinued the trial due to an adverse event. Conclusion: Topical TCC can be an appropriate option in a subset of patients with mild chronic LBP accompanied by muscle spasms. In a subset of patients with milder pain intensity, topical TCC may improve pain earlier. The results of this trial are compatible with the treatment approaches used in daily practice.

OBJECTIVES: Fibromyalgia syndrome (FS) is a chronic pain disorder usually affecting women in their fertile period of life. However, the relationship between FS and pregnancy has not been studied in depth. The effect of FS on the course of pregnancy is poorly investigated in the current literature. Here we aimed to investigate the effects of FS to menarche age, gravidity, parity and duration of breastfeeding. METHODS: One hundred and eighty-seven non-pregnant females attending between March 2015-June 2015, to Malatya State Hospital Physical Medicine and Rehabilitation Outpatient Clinic, were included in this prospective study. One hundred eleven (111) of them were diagnosed with FS according to the American Rheumatology Association (ACR) 2010 criteria and were defined as group 1; group 2 comprised of seventy six (76) non-pregnant healthy volunteers. All participants were asked about their menarche age, marriage age, gravidity, parity, duration of breastfeeding by conducting a basic questionnaire survey. Patients' body mass index (BMI) were recorded. Depression parameters were evaluated by Beck Depression Scale (BDS). RESULTS: The average age of the patients was 39.04±9.21 (FS) and 38.47±9.65 (Control) years; first menarche age was at 13.28±1.38 (FS) and 13.59±1.54 years (Control), and marriage age was 20.1±3.62 (FS) and 20.69±3.90 years (Control), respectively. No statistically significant difference was found (p=0.598) between BMI values (FS, 27.76±4.95; Control 26.90±4.56 kg/m(2)). The results from both groups were similar in terms of gravidity, parity, and breastfeeding duration, with no statistically significant differences (p=0.252, 0.093, 0.075, respectively). The only significant difference was found in the depression parameter. The BDS results were statistically different between the groups, found higher in FS group (p=0.000). CONCLUSION: FS occurs as a result of symptoms such as mood disorder, anxiety, cognitive and sleep disorders, and also hormonal changes; no exact cause has yet been established. The syndrome usually occurs during fertile period of young female. According to the findings of our study, FS has no negative effect on the outcome of gravidity, parity, and duration of breastfeeding in Turkish women. Further studies about the effects of FS on the course of pregnancy are required.
