Hydrophilic acrylic intraocular lens clouding: a clinicopathological review.

PURPOSE: An analysis is made of a serious late complication (opacification) of different models of hydrophilic acrylic intraocular lenses (IOLs). METHODS: Seven lenses were explanted from seven patients treated for cataracts with phacoemulsification and implantation of different types of hydrophilic acrylic IOLs (five SC60B-OUV from MDR, one Aquasense from OII, and one H60M Hydroview lens from Bausch & Lomb) who developed important vision impairment in the late postoperative period (1 to 3 years) due to lens opacification. The explanted lenses were subjected to exhaustive study involving photographic analysis, scanning electron microscopy, and energy dispersive X-ray spectroscopy. RESULTS: Light and scanning electron microscopy revealed diffuse, variable-size granular deposits within the optic of the SC60B-OUV lens, and on the anterior and posterior optic surfaces of the H60M Hydroview lens, though without affecting the haptics in any of the models. Dispersive energy X-ray spectroscopy of the deposits revealed the presence mainly of calcium and phosphorus salts. CONCLUSIONS: Hydrophilic acrylic IOL opacification is a serious late complication of unknown etiology. The condition is more frequent among diabetic patients, and the only management option is IOL explantation and replacement with a lens of some other material. More frequent and longer follow-up is required of those patients wearing lenses for which cases of opacification have been documented, particularly in the presence of predisposing factors (diabetes, uveitis). Caution is required with new lenses, avoiding their generalized use until they have successfully passed the test of time.

[Contrast sensitivity to intraocular lens TECNIS Z-9000]. / Sensibilidad al contraste en las lentes TECNIS Z-9000.

PURPOSE: To evaluate the increase in contrast sensitivity to intraocular lens TECNIS Z-9000 (Pfizer) compared with other intraocular lenses. METHOD: We implanted a TECNIS Z-9000 lens in 18 patients who had another type of intraocular lens implanted in their other eye. We then tested contrast sensitivity under both mesopic and photopic conditions using the VCTS (Vision Contrast Test System) provided by Vistech Consultants Inc. RESULTS: There was an improved contrast sensitivity at low and medium frequencies, following TECNIS-Z 9000 lens implantation, however this improvement did not reach statistical significance. There were significant differences at high frequencies, which are directly related to the quality of visual function. CONCLUSIONS: TECNIS Z-9000 lens implantation (with a modified anterior surface) achieves a significant improvement in contrast sensitivity, meaning in practice an increase in the quality of visual function.

Objetivo: Evaluar el incremento en la sensibilidad al contraste de las lentes TECNIS Z-9000 (Pfizer) respecto a otras lentes.Método: Se implantó la lente TECNIS Z-9000 en 18 pacientes que ya habían sido intervenidos de un ojo con otro tipo de lente intraocular. Se les midió la sensibilidad al contraste en condiciones fotópicas y mesópicas mediante el test VCTS (Vision Contrast Test System) de Vistech Consultants Inc.Resultados: Aunque la tendencia es hacia una mejoría de la sensibilidad al contraste en las frecuencias bajas y media, no existen diferencias significativas. Sí que demuestran de forma significativa la mayor sensibilidad al contraste en las frecuencias altas, que son las que están directamente relacionadas con la calidad de la función visual.Conclusiones: Las lentes TECNIS, mediante una modificación de su cara anterior, consiguen de manera significativa una mejoría de la sensibilidad al contraste, que se traduce en un incremento de la calidad de la función visual

Cataratas congénitas y lentes intraoculares: nuestra experiencia / Congenital cataracts and intraocular lenses: our experience

Durante los últimos años hemos tratado mediante cirugía con implantación unilateral de una lente intraocular a 19 pacientes afectos de catarata congénita monocular o binocular de predomínio unilateral. Se analizan los resultados visuales obtenidos que han sido muy buenos en el grupo binocular y mediocres en el monolateral, aunque en este último grupo, todos los pacientes obtuvieron benefícios con la cirurgia al mejorar la visión y disminuir o desaparecer los sintomas que conllevaron al diagnóstico de catarata (estrabismo, nistagmus, leucocoria, sensibilidad aumentada a la luz etc). Se comentan las complicaciones quirúrgicas que temos tenido y los factores que intervienen en el prognóstico visual de estos pacientes

Se estudia un grupo de pacientes escogidos de forma arbitrária que fueron intervenidos de catarata con implantación de lentes intraoculares y en los que se ha utilizado la sustancia viscoelástica conocida como Hialuronato sódico (Healon). Se analizan las ventajas y desventajas de esta cirugía cuando dicha sustancia ha sido introducida, haciendo hincapié en las complicaciones relacionadas con el uso de la misma, así como tambiém la técnica quirúrgica de su eliminación al finalizar la intervención. Se llega a la conclusión de que el Healon es de una gran ayuda en la cirurgía de lentes intraoculares y que desde su aparición ha dismiunida el número de contraindicaciones a la implantación, la cual se ha visto especialmente favorecida en los casos que precisen una marcada protección endotelial (ojos con queratoplastias, cataratas traumáticas y congénitas en niños, etc.)
