Objetivo: en ciertas situaciones clínicas, puede ser necesario cambiar la orientación del sensor Osstell ISQ(TM) pero se desconocen los efectos de dicha modificación. El objetivo de la investigación fue verificar si los valores del coeficiente de estabilidad del implante (ISQ) difieren según cuál sea la orientación del sensor. Materiales y métodos: utilizando Osstell ISQ (Osstell Integration Diagnosis AB, Gotemburgo, Suecia), se midió el ISQ de 102 implantes de superficie Osteotite (Biomet 3i, Palm Beach Gardens, FL., Estados Unidos), con dos orientaciones diferentes del sensor: paralelo al SmartPeg (Integration Diagnostics AB, Gotemburg, Suecia) (vertical) y perpendicular a él (horizontal). Los valores de ISQ registrados para cada orientación se compararon mediante prueba de Wilcoxon para muestras relacionadas. Resultados: los valores de IsQ verticales difirieron de los ISQ horizontales en el 62,7 por ciento de los casos. Las diferencias entre los grupos resultaron estadísticamente significativas (P<0,05). La media de ISQ vertical (76,29) fue mayor que la de ISQ horizontal (74,78). Conclusión: si la situación clínica amerita un cambio de orientación del sensor, se deberá tener en cuenta que la medida de ISQ vertical observada podría diferir significativamente del valor que se habría obtenido realizando la medición de manera convencional.

BACKGROUND: The high success rate of dental implants has changed the quality of life for many patients. Immediate loading finds its application in some clinical cases and certainly adds another modality of treatment for the implant patient. Starting with a few implants immediately loaded with a bar overdenture in the mandible, the concept of immediate loading evolved to loading multiple implants in both the maxilla and mandible. METHODS: In this investigation, 41 consecutive patients who needed full-arch restorations were treated with 343 double acid-etched surface implants between May 1998 and March 2004. The sample included smokers and bruxers. Twenty-three mandibular and 26 maxillary cases were treated, loading the implants within 48 hours, by using resin provisional prostheses, metal-reinforced provisional prostheses, or definitive prostheses (metal-acrylic or metal-ceramic). All implants were followed for 12 to 74 months. Follow-up consisted of clinical as well as radiographic examination. Furthermore, resonance frequency analysis was done in most of the implants during the last 2 years. RESULTS: The success rate obtained was 99.42% (only two mandibular implants that are still surviving were considered failures). The bone level was measured every year. The average radiographic bone level change was 0.56 mm at 12, 0.76 mm at 24, 0.84 mm at 36, 0.82 mm at 48, 0.83 mm at 60, and 0.94 mm at 72 months. CONCLUSION: A high success rate can be achieved when double acid-etched surface implants are immediately loaded with fixed full-arch restorations in the maxilla and mandible.

BACKGROUND: The use of external hex titanium implants is widely accepted; however, in the past few years, they have been placed by using not only the traditional two-stage surgical protocol, but also the one-stage surgical protocol. The aim of the present study was to compare the success rates that can be obtained from two-stage surgeries (conventional protocol) and one-stage surgeries when double acid-etched surface external hex titanium implants are used. METHODS: A total of 654 double acid-etched surface external hex titanium implants were studied in 195 patients (109 females and 86 males) aged 18 to 79 (mean 49.20). These patients were followed-up both clinically and radiographically for 3 years. Among these implants, 338 were inserted by means of two-stage surgery and 316 were placed following the one-stage surgery protocol. RESULTS: The success rate of the whole sample of double acid-etched surface implants was 99.23%. The results showed a 99.05% success rate for one-stage surgeries and 99.40% rate for two-stage surgeries. Only three of the implants inserted by performing one-stage surgery and two of the implants placed using two-stage surgery failed before they were loaded. No implants were lost after the loading was completed. The bone level at 12 months was 0.58 mm for one-stage-placed implants and 0.54 mm for two-stage-placed implants. At 24 months the bone level was 0.77 mm and 0.68, respectively, and at 36 months 0.89 and 0.83. CONCLUSION: The similar and high success rates obtained in one- and two-stage surgeries using double acid-etched surface external hex titanium implants demonstrate that it is possible to choose the most convenient surgical procedure for each particular case without increasing the risk of failure.
