RESUMO

52 women with opium addiction in remission (deprivation as a result of imprisonment) were examined. The disorders of appreciation pain were found in 50 cases (96 +/- 3%). There were clinical peculiarities of their topography (composed distal, maculary-mosaic and perioral-axial versions). These peculiarities were shown in alcoholism, neuroses, neurosis-like nonpsychogenic conditions of infection and traumatic genesis. The supposed mechanisms of described disorders are also presented.

The study covered 52 women, aged 19-30 years, who had previously taken home-made poppy preparations. In seven of them we diagnosed opium use (F 11.11 according to ICD-10) and 45 had opium abuse (F 11.21). All were detained in penitentiaries for 2-23 months. The control group consisted of 20 healthy females. A significant frequency of brain stem and extrapyramidal disturbances was found in the opium abuse group. Brain stem signs included tendon anisoreflexia (100-2%), nystagmus (87 5%), ataxia (87 5%) and corneal hyporeflexia (48 7%). Among the most frequent extrapyramidal disturbances were a positive pushing test (90 4%); test with pendular swinging of the legs (85 5%) and hand tremor (82 7%). Statistical analysis revealed a relationship between the degree of extrapyramidal system damage and duration of opiate dependence.

Long-term follow-up of the subjects who have suffered closed brain injury showed that in comparison to matched controls the injured young subjects demonstrated abnormalities in lipid metabolism and coagulation tending to atherosclerosis. The limits of the values by which the patients significantly differed from the controls as well as 7 specific for the patients signs of lipid metabolism and coagulation impairment have been determined. From 1 to 5 specific indications registered in 67 +/- 5% patients allowed the diagnosis in them of the injury sequelae. The mechanisms of the above atherosclerotic shifts may work through neuroregulatory derangements common for brain injury and atherosclerotic encephalopathy and enhancement of atherosclerosis course by the craniocerebral damage.

The immunity indicators (T general, T active and B lymphocytes, IgA, IgM and IgG, phagocytosis intensity and activity) and blood content of medium-size molecules (MSM) were studied in patients with brain tumors. Immunity disturbances were revealed in 96 +/- 4% of the patients whereas the increase of the content of MSM was detected in all the patients. The ratio of the absolute count of B lymphocytes to the count of T general lymphocytes (B/T) was offered for use. The magnitude of the ratio equal to 0.47-0.85 turned out characteristic of malignant tumors. The presence of MSM ranging within 0.390-0.480 units was common to the same tumors. The simultaneous demonstration of the characteristic magnitudes of the B/T and MSM in the patient permits one to reliably differentiate between malignant and benign brain tumors. The high magnitudes of MSM are prognostically unfavourable as regards the patients' survival after surgery.

RESUMO

On the basis of 309 cases of visceral cancer with intracranial metastases studied pathomorphologically, the author describes the clinical features of relatively rare forms of intracranial cancer metastasis: those of isolated carcinomatosis of the cerebral dura mater in 14 patients and carcinomatosis of all the three meninges in eight patients constituting 4.5 and 2.6 percent from the total number, respectively. The clinical picture of disease is shown to depend on the form of metastases (nodular or diffuse), their localization on the basal or the convex surface of the cranium and brain, and the phase of disease. Correlation of clinical signs in metastatic isolated carcinomatosis of the dura mater with those of isolated carcinomatosis of the two inner meninges resulted in identifying a series of symptoms differing significantly in frequency. Informative clinical indices permitting differentiation between carcinomatosis of the meninges and intracranial cancer metastases are provided.
