RESUMO

Context Bronchial cancer in a person under 30 years of age is quite rare. It generally occurs after 40 years of age following heavy smoking intoxication. We report a clinical case illustrating the early onset of a small cell lung carcinoma in young heavy smoker. CASE REPORT: A 30-year-old patient, current smoker for about 10 years (15 packs/year), consulted for a cough with haemoptotic sputum. Clinical and paraclinical examinations diagnosed small cell carcinoma of the right lung with some controlatéral metastatic nodules. . He was classified as stage T2bN2M1a. Unfortunately, due to lack of financial accessibility to suitable chemotherapy, the patient died after one month. CONCLUSION: Early-onset of bronchial carcinoma in young smokers calls for strengthened control of teenage tobacco use, especially in Africa, where the phenomenon has been taking on alarming proportions.

The aim of this study was to determine the prevalence of gastritis in upper endoscopies performed for clinical signs suggestive of gastritis among 100 patients in a tropical country. Chronic gastritis (CG) was present among 81%, including CG due to Helicobacter pylori (n = 61, 75.3%), reactive CG (n = 14), and autoimmune CG (n = 6). The gastritis was located in the stomach antrum (55.7%, n = 40), fundus (3.3%, n = 2), or both (41% n = 25). Median age at diagnosis was 35 years (14/80). This CG was associated with signs of stomach activity (27.9%, n = 17), atrophy (13.1%, n = 8), metaplasia (9.8%, n = 6), and dysplasia (8.2%, n = 5). The 14 patients with reactive CG had a mean age of 31 years and all were coffee-drinkers. This CG was not associated with precancerous lesions. Autoimmune CG, present in 6% of the population, was diagnosed at a mean age of 55 years (range: 45; 67). These involved fundal atrophy with metaplasia and were associated with dysplastic lesions (66.7% n = 4).

BACKGROUND: Crusted scabies, also known as Norwegian scabies, is a rare and extremely debilitating form of Sarcoptes scabiei var. hominis infestation that generally occurs in immunosuppressed patients. Herein, we report a "historic" and fatal case. PATIENTS AND METHODS: A 52-year-old woman was admitted to the emergency department presenting crusted dermatitis together with extreme deterioration of her general condition. Her general practitioner had initiated dermo-corticosteroid therapy for suspected atopic dermatitis two months earlier, and she had been confined to bed for the previous 10 days. Her son presented pruritus that became worse at night. On examination the patient was moaning, dehydrated and confused and her entire skin was hyperkeratotic, with very thick, yellowish, cracked crusts covering 40 % of her body. Tests indicated severe water and electrolytic disorders as well as Staphylococcus aureus bacteremia. A tape test established the diagnosis of crusted scabies. The severity was grade III on the Davis clinical grading scale. The patient showed signs of multi-organ failure and was transferred to intensive care, but she died during the night. DISCUSSION: This case is remarkable for its historic severity. In France, scabies infestation is a re-emerging disease and has been a public health priority since 2012. The rare hyperkeratotic form is not fully understood and frequently diagnosed late, in some cases with a fatal outcome.

RESUMO

Crusted scabies is a rare and severe form of infestation by Sarcoptes scabies var. hominis. It is characterized by profuse hyperkeratosis containing over 4000 mites per gram of skin, with treatment being long and difficult. The condition is both direct and indirectly contagious. It has a central role in epidemic cycles of scabies, the incidence of which is on the rise in economically stable countries. Recent discoveries concerning the biology of mites, the pathophysiology of hyperkeratosis and the key role of IL-17 in this severe form open up new therapeutic perspectives.

RESUMO

Dans le cadre de la mise en œuvre d'une stratégie de lutte intégrée contre les parasitoses en milieu scolaire, une étude transversale et descriptive a été réalisée dans la commune de Dangbo. L'objectif est de faire une analyse de la situation des parasitoses (paludisme, bilharziose uro génitale, géohelminthioses) chez les enfants d'âge scolaire. Patients et méthodes : L'approche par questionnaire a été utilisée auprès des élèves de huit écoles âgés de 7 à 12 ans. Des prélèvements de sang, de selles et des urines ont été effectués pour la réalisation d'une goutte épaisse, d'un examen d'urines par filtration ainsi qu'un examen des selles par la technique de KATO- KATZ. Résultats : La prévalence globale des parasitoses est de 74,80%. La parasitose la plus fréquente est la bilharziose (58,18%) suivie du paludisme (40,30%) et enfin des parasitoses intestinales (2,27). Le paludisme demeure la parasitose dont les signes et les moyens de prévention sont les mieux connus. Parmi les facteurs favorisants, nous pouvons citer, la nature de l'eau utilisée, la mauvaise connaissance des causes et des moyens de prévention des parasitoses, la défécation et l'élimination des urines dans la nature, le milieu écologique. Conclusion : Cette étude suggère la mise en place d'une stratégie intégrée de lutte prenant en compte des séances d'Information, Education, Communication sur l'aménagement et l'assainissement du milieu, les causes et les mesures de prévention afin de réduire la prévalence de ces parasitoses en milieu scolaire et dans la communauté
