OBJECTIVES: This study assessed the impact of hepatic impairment on the pharmacokinetics (PK) of paliperidone and its enantiomers. METHODS: A single 1 mg dose of paliperidone immediate-release (IR) was administered to subjects with moderate hepatic impairment (n = 10) and demographically matched individuals with normal hepatic function (n = 10). RESULTS: Plasma protein binding was lower in hepatically impaired subjects resulting in a 27% higher unbound fraction of paliperidone compared with healthy individuals. After correcting for the difference in plasma protein binding, unbound exposures were comparable between groups. All other PK parameters were similar between the two groups. Paliperidone IR was equally well tolerated in both groups. CONCLUSIONS: The impact of moderate hepatic impairment on paliperidone PK is not considered clinically relevant as the PK profile of unbound paliperidone is similar for subjects with moderate hepatic impairment and those with normal hepatic function. Dosage adjustments of paliperidone are not required in subjects with mild or moderate hepatic impairment.

Fungus balls in the upper urinary tract are uncommon infectious processes that usually occur in diabetic patients. We report a case of an obstructing fungus ball of the renal pelvis, which was identified and removed successfully by percutaneous techniques.

To compare the relative prognostic accuracy of the Gleason classification, the Whitmore staging, and the Broder grading systems, 111 patients with prostate cancer undergoing radical surgery were assessed utilizing these systems. The assessments were correlated with the presence or absence of disease six months to eight years after surgery. Of the three systems the Gleason classification system was the least accurate.

Urinary immunoglobulin A levels were measured in the urine of 13 patients with histologically diagnosed bladder cancer present, six patients with culture proven urinary tract infection, and 14 control individuals. There were statistically significantly higher immunoglobulin A levels in the urine of patients with bladder cancer than in either individuals with urinary tract infection or control individuals. Urinary immunoglobulin A may be a potentially useful test for the diagnosis and follow-up of bladder cancer patients.
