RESUMO

Objective: To determine the craniocervical posture in 120 subjects, 90 patients with temporo- mandibular joint disorders (TMD) of varying degrees were selected and compared with those of 30 healthy individuals. Methods: True lateral radiographs in the natural head position were obtained from TMD patients and healthy individuals. Linear measurements from the opisthion to the intersecting point of the craniovertebral angle, the opisthion to the tip of the spinous process of the seventh cervical vertebrae, and the body of Atlas to the lower border of the seventh cervical vertebrae were obtained. Data were analyzed by one way ANOVA followed by Scheffe's post hoc test. Results: Significant increases in craniovertebral, odontoid plane, and individual vertebral angles were noted in the TMD patients when compared with the healthy individuals. Conclusion: Significant postural changes in the skull in relation to the cervical vertebrae with dorsiflexion as a compensatory strategy were observed in this study.

RESUMO

Pangasianodon hypophthalmus (sutchi catfish) is a fresh water catfish extensively being cultured in the South East Asian countries in the recent years. The present study provides the first report on the effects of gutting on the quality characteristics of aquacultured sutchi catfish stored in ice. pH of whole ungutted and gutted catfish didn't show significant difference (p > 0.05) during ice storage period. Total Volatile Base Nitrogen (TVB-N), Alpha Amino Nitrogen (AAN), Free Fatty Acids (FFA) and Thio Barbituric Acid Reactive Substance (TBARS) were lower in gutted fish compared to whole ungutted fish at any particular day during ice storage. However, gutted fish expressed higher rate of primary lipid oxidation than ungutted fish. Textural degradation of the fish muscle as indicated by hardness, cohesiveness, springiness and chewiness was lower in gutted fish. Results of sensory evaluation revealed that gutting has significantly improved the sensory quality of the fish. However, microbiological analysis revealed higher Total Plate Count (TPC) and Enterobactereaceae count in gutted fish. The shelf life of gutted and whole ungutted sutchi cat fish as determined by microbiological analysis was 16-18 days and 18-20 days respectively while storage in ice.

RESUMO

Metastatic deposits within the breast may be difficult to distinguish from primary breast carcinoma. Radiological features and immunohistochemistry especially for steroid hormone receptors and expression of gross cystic disease fluid protein may be helpful in differentiating these two conditions. In this report, we present a case of signet ring cell stomach cancer with metastasis to the breast and discuss the differential diagnostic options.

RESUMO

Bleeding during and after cardiac operations and the effects of cardiopulmonary bypass hemodilution commonly result in blood transfusions. Excessive microvascular bleeding can result in re-exploration and prolonged hospitalization. Nearly 20% of all blood transfusions in the United States are associated with cardiac surgery. The risks associated with the use of allogeneic blood product transfusion include mistransfusion, immunologic complications, and transmission of infectious diseases. The large demand for blood products places significant pressure on the national blood supply, resulting in frequent shortages. The variability in transfusion practice of cardiac surgery patients suggests that sound blood management and a conservative approach to this population can result in reduced transfusions without increasing morbidity or mortality and avoiding complications associated with allogeneic blood transfusion.

RESUMO

Nearly 20% of blood transfusions in the United States are associated with cardiac surgery. Despite the many blood conservation techniques that are available, safe, and efficacious for patients undergoing cardiac surgery, many of these operations continue to be associated with significant amounts of blood transfusion. Although surgical bleeding after cardiopulmonary bypass is a common problem as reflected by the substantial use of blood products, it is the individual physician and institutional behavior that have been identified as reasons for transfusion and not necessarily patient comorbidity or blood loss. Transfusion rates in cardiac surgery remain high despite major advances in perioperative blood conservation, with large variations among individual centers. The adoption of available blood conservation techniques, either alone or in combination in patients undergoing cardiac surgery, could result in an estimated 75% reduction of unnecessary transfusions. The success of previously reported blood conservations programs in cardiac surgery should call for a reevaluation of allogeneic transfusion practices in patients undergoing cardiac surgery. By applying the numerous reported blood conservation strategies for the management of patients presenting for cardiac surgery, we can preserve our dwindling blood resources and help alleviate some of the direct costs of blood as well as the indirect costs of treating noninfectious and infectious complications of transfusion.
