RESUMO

We report a very rare case of Monteggia and Galeazzi fractures of the same forearm in a 23-year-old female traffic accident victim. Fractures of the two forearm bones were reduced and fixed with a solid assembly. Dislocation of the radial head reduced spontaneously. Inferior radioulnar instability was treated by blocking the joint with a K wire for one month. At 24 months, outcome was satisfactory with 40 degrees pronation and 70 degrees supination. Elbow and wrist flexion-extension and muscle force were the same as on the healthy side. Surgery is indicated in this rare association in adults. As it is very important to restore exactly the anatomy with solid fixation of the ulna to achieve and maintain reduction of the radial head. Surgical exploration of the humeroradial joint is not required unless reduction cannot be achieved. The same is true for fixation of the radius after Galeazzi fracture. If an inferior radioulnar instability persists, we propose temporary stabilization with a K wire. Our experience with this case would argue against first intention resection of the distal portion of the ulna as proposed by Hughston. We prefer to postpone resection which would be performed only in case of bothersome instability. A Sauvé-Kapandji procedure would then be a possible solution.

RESUMO

A child of 13 years, victim of a fall on his wrist at dorsal flexion position; he presented a scaphoid fracture associated with a displaced fracture of the distal end of the radius. The radius fracture was reduced under general anesthesia, by external manoeuvre; the consolidation was obtained after 10 weeks of plaster cast immobilization. The follow-up was of two years, with a good clinical and radiological result in spite of a non union of the Ulnar-styloid. It is important to remember this exceptionnel association in children not to forget the scaphoid fracture.

RESUMO

Dorsal trapezoscaphoid dislocations are characterised by posterolateral translation of the trapezium and the distal part of the first finger in relation to the rest of the wrist and hand. The authors found only 7 cases in the literature and they analyse the principal features in the light of a new personal case caused by violent trauma. It was easily reduced orthopaedically, but because of the instability, double pinning was performed to the adjacent bones and plaster immobilisation was maintained for six weeks. An excellent functional and radiological result was obtained and remained stable after two years.

RESUMO

The overlapping fifth toe has often a congenital origin. It may be isolated or associated with an another pathology, congenital or acquired, but the relationship between them. Cannot be ascertained. The overlapping fifth toe is responsible for an esthetic deficit and/or a functional discomfort, it justifies a correct surgery. Twenty-one feet in 16 patients aged from 2 to 30 years (especially between 5 and 15 years) have been operated on and reviewed after a mean follow-up of 4 years. The technic associated a cutaneous dorsal Z-plasty on the cutaneous retraction a medial and dorsal capsulotomy of the metatarso-phalangeal joint, an external transfer of the fifth extensor tendon or its tenotomy and, in 10 cases, a plantar skin resection 19 results were excellent, one fair result and one failure. The condition of a stable result seems to be based on a complete and spontaneous correction of the deformity at the end of the operation.
