RESUMO

From January, 1988 to March, 1994, 83 patients with diagnosis of gestational trophoblastic disease, were identified. Incidence was 2.4 per 1000 births. Average age of patients was 28.9 years. Forty four point five per cent of them were multiparae and in 25.3% there was the antecedent of molar pregnancy. Seventy seven point one per cent of the cases are from low socioeconomic stratum. The diagnosis was done by ultrasound in 89.1%. Instrumental uterine curettage was done in 89.1%, with histological confirmation in 100% of the cases. From the 83 cases with molar pregnancy, 74 were classified as complete moles, four, incomplete, four, invasive and one choriocarcinoma. There was follow up in all the patients with beta fraction of chorionic gonadotropin hormone; this was negative in most of the cases for eight weeks after evacuation. Oral contraceptives were indicated in 73.4% of the cases.

RESUMO

Fifteen (18%) out of 83 patients with gestational trophoblastic disease, developed persistent trophoblastic tumor, from January, 1988 to March, 1994. Average age of patient was 25.5. There was antecedent of molar pregnancy in two cases. Beta fraction of chorionic gonadotrophin hormone at the time of diagnosis was 152,200 m UI/ml, average. During control it was in 80% of anovulatory oral cases. The treatment was based on metotrexate, and folinic acid; there was 100% remission. There was a total of 46.6% of pregnancies, after treatment; and 40% of them were normal.

RESUMO

The case of a patient with Meigs' syndrome, seen at Instituto Nacional de Perinatología, México City, is presented. The patient presented with: Hydrothorax, ascites and benign ovarian tumor (fibroma). The condition was managed by salpingo-oophorectomy; the patients was cured. Hydrothorax and ascites etiopathology, which is not clear at the present time, is mentioned.

RESUMO

The purpose of this study was to analyze 19 patients infected with Human Papilloma Virus (HPV), in order to determine the most frequent subtype. Subjects were patients of the National Institute of Perinatology (INPer), between 1991-1992. All the patients had positive cytology, cervical biopsy was done through colposcopy control. Sample characteristics were: Mean age 29 years, married; most of them multiparous (9/19) and 7 were monogamous. Six out of 12 cytologies were positive for HPV, 12 out of 19 had an inespecific inflammatory reaction and one patient had NICIII. The histopathologic study showed in 15 cases a positive reaction to HPV, 2 out of 19 had both dysplasia and condyloma, two cases had moderate and severe dysplasia. All the cases were studied with in situ hybridization with biotinilaty probes, two cases were positive to Subtype 18 and one to Subtype 16. Positive cases were selected for electron microscopy in order to confirm viral particles. This study suggests that subtype 18 and 16 positivity must be considered as high risk of cancer.

RESUMO

The case of a 24-year-old woman with fever, abdominal pain and weight loss, is presented. Right ovarian dysgerminoma was diagnosed; group A blood, negative direct Coombs. The search for free serum antibodies was positive, with specificity: auto anti 1, anti P with activity at 37 degrees C. Hysterectomy, appendectomy, omentectomy and retroperitoneal ganglia biopsy, were performed; the latter one showed metastasis. Blood transfusions were given. Thirty days postoperative, the red cell polyagglutination had disappeared, as well as antibodies.
