RESUMO

The article analyses 41 fractures of the medral epicondyle of the humerus in children with incarceration of the fragment in the cavity of the elbow joint. The clinical and X-ray signs of this complication are described in detail. Open reduction is the optimal method of treatment. During the operation the ulnar nerve is examined, the cavity of the elbow joint is cleaned, and stable osteosynthesis of the medial epicondyle is performed. Restorative treatment can be started in the early postoperative period if a screw-awl has been applied. The function of the elbow joint is restored completely in 6 to 8 weeks in fresh cases, but in delayed treatment and neglected cases rehabilitation takes 6 months or longer.

RESUMO

The authors analyse the treatment of 153 children aged 4.5-15 who were performed 213 tendon- and myoplasties for persistent lesions of the peripheral nerves. The peculiarities of rehabilitative therapy in this category of patients are described. Three groups of muscles used for grafting and requiring special approach during the rehabilitation period have been distinguished. The rehabilitative treatment is divided into three periods: preoperative preparation, working out of the new function and securing of the new motor act. The peculiarities of the rehabilitative treatment in each period have been substantiated.

RESUMO

The authors attract attention to frequent association of fracture of the inner epicondyle of the humerus with dislocation of the bones of the forearm, which is of great practical importance. On this basis classification of fractures of the inner epicondyle of the humerus is presented. The authors believe that in treating this injury the method of choice should consist in open reposition, which allows to achieve accurate reposition, sanation of the ulnar joint cavity and stable fixation of the fragment. Osteosynthesis by screw-awl provides for reliable fixation of the fragment and allows for early development of the movements in the ulnar joint. Surgical treatment mostly gave excellent and good results and complications were few. The complications observed were mainly connected with late admission of the patients.

RESUMO

The article analyzes results of treatment of 77 children with dislocations of the forearm bones complicated by a fracture of the internal epicondyle of the humeral bone. The method of operative treatment with the fixation of the epicondyle with the help of a screw-stylus which, while fixing the fragments, allows early development of movements in the elbow articulation. Long-term results were followed-up in 70% of the patients treated by this method. Timely diagnosis and operative treatment gave good results almost in all the patients.

RESUMO

The authors studied long-term (from 2 to 9 years) results of treatment of 178 children with intra- and periarticular localization of fractures of the upper extremities. The age of the children was from 4 to 15. It has been established that the further development of the injured joint is dependent on the character of the primary displacement, exact arrangement of fragments and correct terms of immobilization being of certain significance for the consolidation and further restoration of the anatomical integrity of the bone and for the joint functioning.
