[Use of WOBE-MUGOS E for prevention and correction of experimental doxorubicin side-effects]. / Ispol'zovanie VOBE-MUGOS E dlia profilaktiki i korrektsii pobochnykh reaktsii doksorubitsina gidrokhlorida v éksperimente.

Examined in treating rats with Guerin's carcinoma with doxorubicin was the possibility that the polyenzymic drug preparation wobe-mugos E had hepatoprotective, cardioprotective and myeloprotective effects. In intramuscular administration wobe-mugos E has not been found to stimulate the tumour growth, it exhibited a manifest hepatoprotective and myeloprotective actions with respect to adverse reactions of doxorubicin but failed to diminish cardiotoxicity of the latter. The protective effect of the above polyenzymic drug was of a dose-dependent character.

[The mechanisms of the curative action of systemic enzyme therapy]. / O mekhanizmakh lechebnogo deistviia sistemnoi énzimoterapii.

Employment of the authors' highly sensitive biochemical method allowed the fact of resorption of proteolytic enzymes after their peroral ingestion to be confirmed, with a question having been raised as to further fate of proteinases and mechanisms of curative action of combined enzyme preparations. The analysis of relevant data from published literature and the results of the author's own studies permitted identifying the most important moments of realization of the systemic enzyme therapy effects, which are as follows: 1) activation of receptors on the surface of enterocytes; 2) formation of activated alpha 2-macroglobulin (alpha 2-M) under the action of proteinases from the intestines, and realization of catalytic activity of those enzymes involved in this restrictor of proteinases; 3) modulation of cell activity by complexes alpha 2-M-proteinase; 4) activation of the system of mononuclear phagocytes; 5) influence on the balance of bodily endogenous proteolytic systems.

[Systemic enzyme therapy in chronic glomerulonephritis]. / Sistemnaia fermentnaia terapiia pri khronicheskom glomerulonefrite.

AIM: To try polyenzyme drug vobenzim in chronic glomerulonephritis (CGN). MATERIALS AND METHODS: 174 CGN patients were randomized into 2 groups. The study group of 39 patients received conventional pathogenetic treatment plus vobenzim (initial dose 18-24, maintenance 12-15 dragees a day). Control group of 135 patients were given only pathogenetic therapy. 67 patients appeared ineligible. RESULTS: 3-6 months after the treatment the response was seen in 61.5% patients of the study and 17.8% patients of the control group. Side effects of vobenzim in CGN treatment were absent. CONCLUSION: Enzyme therapy with vobenzim in CGN is safe and effective.

[Effect of blood plasma inhibitors on activity of serine and cysteine proteinases]. / Vliianie ingibitorov plazmy krovi na aktivnost' serinovykh i tsisteinovykh proteinaz.

Data on study of action plasma inhibitors on activity of pancreatic proteolytic enzymes (trypsin, chymotrypsin) and plant proteinases (papain, bromelain), included in composition of enzyme mixes, used for orally application are submitted. It is established, that serine proteases are more sensitive to inactivation of plasma inhibitors, than cysteine enzymes. Main inhibitor of the papain and bromelain is alpha-2-macroglobulin in complex with which they preserve significant part of initial activity. A high-sensitivity method of determination of activity enzyme combinations, enabling to detect nanograms of them in presence of plasma inhibitors is offered. It can be used for study pharmacokinetic and optimization of enzyme mixes application in clinical practice.

[Systemic enzyme therapy in orthopedics and traumatology]. / Systemna enzymoterapiia v ortopediï i travmatolohiï.

In 57 patients the preparations of systemic enzyme therapy vobenzim and flogenzim were applied during conservative or operative treatment of locomotor system injuries. Anti-edema, antiinflammatory and analgetic action of preparations was established.

[Late ventricular potentials and left ventricular performance in patients with hypertrophic cardiomyopathy]. / Pozdnie zheludochkovie potentsialy i funktsional'nye sostoianie levogo zheludochka u bol'nykh s gipertroficheskoi kardiomiopatiei.

Twenty patients with hypertrophic cardiomyopathy were examined. They were found to have late ventricular potentials in the disease, which were characterized by an increase in average QRS complex duration, a decrease in voltage, and a rise in the duration of last 40 msec of the QRS complex filtered. Such patients generally exhibited lower exercise tolerance in severe asymmetric left ventricular hypertrophy, which caused a significant deterioration of cardiac pump function.

[Importance of collateral circulation for compensating for myocardial ischemia during physical loading in ischemic heart disease patients]. / Znachenie kollateral'nogo krovotoka dlia kompensatsii ishemii miokarda pri fizicheskoi nagruzke u bol'nykh ishemicheskoi bolezn'iu serdtsa.

Exercise-induced electrocardiotopographic ST depression was analysed with reference to the presence of a collateral blood flow, angiographic assessment of the coronary bed and a history of myocardial infarction in 94 coronary patients. The presence of a collateral blood flow is shown to contribute to a more rapid elimination of post-exercise myocardial ischemia, irrespective of the number of affected coronary arteries and the history of myocardial infarction. Myocardial ischemia developing in response to exercise is particularly extensive in case of multiple coronary arterial lesions and the presence of a collateral blood flow.

An analysis of physical exercise tolerance in CHD patients with the equal number of affected coronary arteries with relation to a coronary bed region involved in a pathological process, showed that its decrease was mostly pronounced in vasoconstriction of the left coronary arterial system and was determined by a decrease in the blood flow in this region.

[Diagnosis of coronary artery lesions during the exercise test according to electrocardiotopographic data in patients with ischemic heart disease]. / Diagnostika porazheniia koronarnykh arterii pri fizicheskoi nagruzke po dannym élektrokardiotopografii u bolnykh ishemicheskoi bolezn'iu serdtsa.

It has been shown that electrocardiotopography applied during the exercise test in patients with coronary heart disease makes the test more sensitive without changing its specificity as compared with the ECG in common 12 leads. The depression of the ST segment occurs on the electrocardiotopogram in the areas of the precardial zone, corresponding to an isolated or pair lesion of the definite coronary arteries. Upon the involvement of all the three main arteries of the heart the site of the depression is indefinite, but its degree rises, being maximal in patients with stenosis of the left coronary artery and right coronary artery. In these patients, the displacement of this segment spreads to all the areas of the precardial zone.

The area of exercise-associated ischemia in coronary patients was shown to depend on both total heart arterial damage and functional status of the myocardium. Stress related electrocardiographic ST depression in a limited number of leads was associated with satisfactory myocardial function. Generalized exercise-induced ischemia developed where left-ventricular myocardial function was disturbed significantly. Changes in erythrocyte calcium concentrations were the greatest in those patients whose myocardial function was disordered considerably.

Comparison of exercise-induced variations in the end portion of electrocardiographic ventricular complex and the echocardiographic pattern showed that amplitude changes and T-wave inversion in response to exercise developed where hemodynamic support was less economical in patients with stage-IIA essential hypertension. In essential hypertension, the spread of myocardial ischemic area, as determined by the number of ECG leads showing post-exercise ST displacement, was related to the type of hemodynamic response. The number of ECG leads with displaced ST is a factor to be considered in the assessment of exercise test results.
