RESUMO

Obese patients were coached after inpatient medical rehabilitation with a 6 units comprehensive multimodal Liveonline aftercare program. In the randomized controlled study design, significant improvements in all outcome criteria were found both in treatment and control group. The improvements in the treatment group, however, were only partially superior to the control group, especially in the area of food habits. A statistically significant superiority of the treatment group concerning the primary targets waist circumference and body mass index could not be determined. The Liveonline aftercare was evaluated positively by the participants. Future research is discussed.

RESUMO

Modifications of the subjective prognosis of gainful employment during in-patient rehabilitation were assessed by means of a 3-item scale (SPE scale) in 180 orthopaedic patients. Statistically significant improvements were found as to the subjective prognosis of gainful employment and other measures of vocational ability as well. Patients showing lower risk at the beginning profited to a greater degree from rehabilitation as compared to patients with higher scores in SPE scale. To improve the effects of rehabilitation for patients with a high subjective risk as to gainful employment specific interventions are recommended. Further research is discussed.

RESUMO

Advanced stage squamous cell carcinomas of the head and neck have a particularly poor prognosis despite surgical and irradiation treatment. Over the last few years attempts have been made to improve the local control rate using primary chemotherapy. 70 patients with advanced carcinomas were treated with methotrexate, cisplatin and bleomycin using double the dose of cisplatin as in the EORTC-study. The remission rate for patients undergoing two treatment courses was 83%. Toxicity was moderate to low with additional intensive anti-emetic treatment. The combination is well suited for chemotherapy of advanced tumours of the head and neck in combined treatment programmes.

RESUMO

Analysis of 763 debatable examinations by experts and the study of 104 bulletins. It because clear that the results of language-audiometry can not at all be reproduced in the same way as the results of tone-audiometry. The reason for this is not only a different technique of measure, but obviewsly the patient accommodates himself to being examined. Therefore a sole deterioration of any language-audiogramme without the deterioration of the tone-audiogramme must be critically deliberated by any expert.

RESUMO

In a histological point of view there is in the first line the squamous cell carcinoma. Mostly a chronic otitis media is important for the genesis. Because of the hidden place of the tumor there are usually late stages to be observed. The success of therapy therefore is not all convincing as are about 25% 5-year-survival-rate. The mono-bloc-resection of the temporal bone turns out to be more and more important. As this operation involves high risks, the partial resektion of the temporal is still used. Radio-therapy before or after the operation is widely contested in its value, although there are statistically ensured results. Sole radio-therapy is only convincing with very tiny tumors, in other cases it is only palliative. Because of the histological structure of the squamous cell carcinoma the results of chemotherapy cannot be better.

RESUMO

Comparative studies of case controls of Bell's palsy, posttraumatic and postoperative palsies in relation to the clinical positive evidence of the nerve excitability test are demonstrated. While an absolutely parallel reaction of clinical function restitution and electrical excitability is observed in Bell's palsy and in posttraumatic late palsy, on the other hand this congruence will not be observed in postoperative palsies (in intact anatomical conduction of the nerve) in most of the investigated cases. In these cases the clinical restitution of isolated movements of the mimetic muscles has further improved than the excitability test demonstrates. The exact developmental mechanism remains still open.

Facial nerve anomalies found at surgery are reported, and are classified as Minor and Severe. A solitary malformation is rare. Duplication of the nerve in its tympanic segment as well as branching within the mastoid bone with an unusual route close to the mastoid cortex was also observed. Malformations of the nerve in connection with other anomalies of the ear were encountered in 20% of all ear anomalies that underwent surgery. There was no direct correlation between the degree of the ear deformity and the facial nerve malformation. Two cases of severe malformations were seen whereby in one with a minor ear deformity the nerve was seen lying at the level of the tympanic membrane. In the other instance the nerve had an additional well developed duplication in the correct position, although a lesion on the duplicated nerve showed that it was non-conductive.

RESUMO

The problems of routine mediastinoscopy such as the differential diagnosis of unrecognisable specific pathology of the mediastinum or lung, assessment of the operability of malignancy in the thorax prior to surgery, and the execution of minor mediastinal surgery are discussed. It is recommended that mediastinoscopy may also be performed where no radiological abnormality exists, and it is considered necessary that it is performed with biopsy where bronchial carcinoma has already been positively identified. Personal experience shows that previous mediastinoscopy or mediastinal irradiation are not contraindications to this procedure, but an adequate thoracic surgical unit and adequate after-care are essential prerequisites.
