RESUMO

Previous research has found that lower limb muscle asymmetries increase with age and are linked to fall and injury risks. However, past studies lack a wide variety of muscle function modes and measures as well as comparison to a comparable younger age group. The purpose of this study was to examine age-related lower limb muscle function asymmetries across a variety of muscle action types and velocities in young and old adults. Lower limb balance, strength, power, and velocity were evaluated with concentric, isometric, isotonic, and eccentric muscle actions during a single-leg stance test and on single- and multi-joint dynamometers in 29 young (age = 21.45 ± 3.02) and 23 old (age = 77.00 ± 4.60) recreationally active men and women. Most (15 of 17) variables showed no statistical (p > 0.05) or functional (10% threshold) limb asymmetry for either age group. There was a significant main effect (p = 0.046; collapsed across groups) found for asymmetry (dominant > non-dominant) for the isotonic peak velocity variable. There was a significant (p = 0.010) group × limb interaction for single-joint concentric peak power produced at a slow (60 deg/s) velocity due to the non-dominant limb of the young group being 12.2% greater than the dominant limb (p < 0.001), whereas the old group was not asymmetrical (p = 0.965). The findings of this investigation indicate there is largely no age-related asymmetry of the lower limbs across a range of muscle function-related variables and modes, with a couple of notable exceptions. Also, the significant asymmetries for the isotonic peak velocity variable perhaps show the sensitivity of this uncommonly used measure in detecting minimally present muscle function imbalances.

PURPOSE: To examine the effects of neuromuscular fatigue and recovery on maximal and rapid torque characteristics in young and old men for the leg extensors and flexors. METHODS: Twenty-one young (age = 24.8 years) and 19 old (72.1 years) men performed maximal voluntary contractions (MVCs) before and at 0, 7, 15, and 30 min following an intermittent submaximal fatigue task. Outcome measures included endurance time, maximal (peak torque; PT) and rapid (absolute and normalized rate of torque development; RTD and nRTD) torque characteristics. RESULTS: The old men had greater endurance times than the young men. Differential recovery patterns were observed for PT, and early and late RTD phases between the leg extensor and flexor muscle groups such that the early rapid torque variables and the flexors demonstrated slower recovery compared to later rapid torque variables and the extensors. The normalized RTD variables were reduced less after the fatigue task and differential muscle and age effects were observed where the flexors were reduced more at the early phase (nRTD1/6) compared to the extensors, however, for the later phase (nRTD2/3) the young men exhibited a greater reduction compared to the old men. CONCLUSIONS: Dissimilar fatigue recovery patterns across different phases of RTD, lower limb muscles, and age groups may have important fatigue-related performance and injury risk implications across the adult lifespan.

OBJECTIVES: To examine the effects of a combined eccentric overload and aquatic-based plyometric training program on muscle function/performance measures and soreness versus an eccentric-only training protocol using a minimal dose training paradigm. METHODS: Twenty-five participants were randomized into either an eccentric-only training group (ECC) or a combined eccentric and aquatic plyometric group (ECC + AQP). The ECC group performed eccentric training once per week for 6-weeks while the ECC + AQP group performed the same eccentric training but with an additional aquatic plyometric training session. RESULTS: There was no group × trial interactions for any of the variables. However, the training elicited large improvements in eccentric strength in both ECC (27%; ES = 1.33) and ECC+AQP (17%; ES = .86) groups. Isometric strength improved moderately for ECC and ECC+AQP groups (17.2%, ES = .53;9%, ES = .45). A moderate increase was observed for depth jump height for both ECC and ECC+AQP groups (13.1%, ES = .48;8.8%, ES = .36). No changes were observed for countermovement jump or sprint time and muscle soreness did not differ between groups. CONCLUSIONS: Minimal dose multi-joint eccentric overload training improved strength and depth jump outcomes after 6-weeks regardless of the training condition but adding a minimal dose aquatic plyometric protocol does not improve muscle function-based outcomes.

The depth jump (DJ) is commonly used to evaluate athletic ability, and has further application in rehabilitation and injury prevention. There is limited research exploring sex-based differences in DJ ground reaction force (GRF) measures. This study aimed to evaluate for sex-based differences in DJ GRF measures and determine sample size thresholds for binary classification of sex. Forty-seven participants from mixed-sex samples of NCAA athletes and young adults performed DJs from various drop heights. Force platform dynamometry and 2-dimensional videography were used to estimate GRF measures. Three-way mixed analysis of variance was used to evaluate main effects and interactions. Receiver operating characteristic (ROC) curve analysis was used to evaluate the combined sensitivity and specificity of dependent measures to sex. Results revealed that reactive strength index scores and rebound jump heights were greater in males than females (p < 0.001). Additionally, young adult females showed greater peak force reduction than young adult males (p = 0.002). ROC curve analysis revealed mixed results that appeared to be influenced by population characteristics and drop height. In conclusion, sex-based differences in DJ performance were observed, and the results of this study provide direction for future DJ investigations.

The reliability of isokinetic peak torque (PT) has been reported mostly using a short-term (<~10 day) inter-trial testing time frame. However, many studies and programs utilize a long-term (several weeks to months) inter-trial testing period. Additionally, the methods by which the PT value is selected and reported from a multiple rep testing scheme have not been well investigated for both reliability and PT absolute performance comparisons. The purpose of this study was to investigate the long-term reliability of isokinetic and isometric PT of the leg extensors with an emphasis on the differences among several PT score selection methods. Thirteen men and women (age = 19.5 years) underwent two testing trials separated by 28.8 (±1.8) days. Testing included maximal voluntary contractions of three sets of three reps for two isokinetic contraction conditions of 60 (Isok60) and 240 (Isok240) deg/s velocities, and three sets of one rep of isometric contractions for the leg extensors. The PT score was derived from seven different methods (see text for descriptions). Reliability as assessed from intraclass correlation coefficients (ICCs) varied widely across contraction conditions and PT score selection parameters. The Isok60 velocity overall had lower reliability (ICCs = 0.48-0.81) than Isok240 (0.77-0.87) across the conditions whereas the isometric PT variables showed moderate reliability (0.71-0.73). Overall the set 1 PT score selection parameters were generally lower (p ≤ 0.05) than those that involved sets two and three. Systematic error (p ≤ 0.05) was shown for 6 out of the 17 PT selection variables. On a subjective interpretation basis, when taking everything into account the best overall combination of time/trial efficiency, reliability, best/highest PT score parameter, and reduced risk of systematic bias appears to be the PT variable that uses the average of the highest two reps of the first two sets of three reps-i.e., averaging the highest two values of the six total reps from the first two sets.

Relatively few investigations have examined the transfer effects of multiple-joint isokinetic eccentric only (MJIE) resistance training on non-specific measures of muscle strength. This study investigated the transfer effects of a short-term MJIE leg press (Eccentron) resistance training program on several non-specific measures of lower-body strength. Fifteen participants performed Eccentron training three times/week for four weeks and were evaluated on training-specific Eccentron peak force (EccPF), nontraining-specific leg press DCER one-repetition maximum (LP 1 RM), and peak torques of the knee extensors during isokinetic eccentric (Ecc30), isokinetic concentric (Con150) and isometric (IsomPT) tasks before and after the training period. The training elicited a large improvement in EccPF (37.9%; Cohen's d effect size [ES] = 0.86). A moderate transfer effect was observed on LP 1 RM gains (19.0%; ES = 0.48) with the magnitude of the strength improvement being about one-half that of EccPF. A small effect was observed on IsomPT and Ecc30 (ES = 0.29 and 0.20, respectively), however, pre-post changes of these measures were not significant. Con150 testing showed no effect (ES = 0.04). These results suggest a short term MJIE training program elicits a large strength improvement in training-specific measures, a moderate strength gain transfer effect to DCER concentric-based strength of a similar movement (i.e., LP 1 RM), and poor transfer to single-joint knee extension measures.

Purpose: This study investigated the effects of flywheel squat training on lower body muscle function adaptations and sport-specific performance in collegiate club water polo players. Methods: Thirteen collegiate club water polo athletes (5 women, 8 men) performed flywheel squat training for 4 weeks. Isokinetic knee extension (KE) peak power (PP) and peak torque (PT), flywheel squat peak power (FPP) and mean power (FMP), countermovement jump (CMJ), in-water jump height (WJH) and foot speed were assessed at baseline (Pre1), 4 weeks (Pre2), and 8 weeks (Post) with the first 4 week block being a control period and the second 4 week block being the experimental training. Throughout the training period muscle soreness was assessed using a VAS scale, and FPP and FMP were assessed during every other session. Results: Isokinetic KE PP and PT increased from Pre1 to Post, and FPP and FMP increased between Pre1 and Post, and Pre2 and Post. CMJ and foot speed were unchanged. WJH displayed a change between Pre1 and Post. FPP increased 19% from session 2 to 4 and FMP increased 27% from session 2 to 6, and each remained elevated through session 8. Conclusion: 4 weeks of flywheel squat training in collegiate club water polo players elicited large gains (47-52%) in flywheel-specific squat power, but did not influence sport-specific performance measures including CMJ, WJH, and foot speed. Water-based exercises and stretch-shortening cycle movements (plyometrics) in combination with effective resistance training programs, which may include flywheel-based training, are likely needed for marked sport skill improvements.

This study investigated the age-related changes in antagonist muscle coactivation of the biceps femoris (BF) during an acute recovery period following a leg extensor fatiguing protocol. Twenty-three young (mean ± SD: age = 25.1 ± 3.0 years) and twenty-three old men (age = 71.5 ± 3.9 years) participated. Surface electromyography (sEMG) was recorded from the BF muscles for antagonist muscle coactivation. Testing involved participants performing leg extension isometric maximal voluntary contractions (MVCs) and isokinetic MVCs at 240°·s-1 at baseline (Pre) and again after the fatigue protocol at 0 (Post0), 7 (Post7), 15 (Post15), and 30 (Post30) minutes post fatigue. Root mean square (RMS) values were computed from the BF sEMG and were calculated as the first 200 ms from onset for the isometric (IsomCoact200ms) and dynamic isokinetic 240°·s-1 (DynCoact200ms) MVCs, and for the final 10° of the leg extension (DynCoact10°) on the isokinetic 240°·s-1 MVCs. Two-way ANOVAs [age group (young vs. old) × time (Pre vs. Post0 vs. Post7 vs. Post15 vs. Post30)] showed that DynCoact200ms had an effect for time (p = 0.018), with greater antagonist coactivation in Pre than Post0 (p = 0.009) and recovering by Post7 (p = 0.011) with no group differences. DynCoact10° had no age × time interaction (p = 0.070), but had a main effect for time (p = 0.020) with the Post0 being lower than the Pre. However, for this variable the young group showed a more severe Pre to Post0 fatigue decline (-45.9%) than the old group (-6.7%) indicating this may be a more sensitive variable for capturing age-related antagonist coactivation post-fatigue responses. Leg extensor fatigue affects some BF coactivation sEMG variables more than others, and any altered post-fatigue coactivation response recovers rapidly (<7 min) from baseline levels.

ABSTRACT: Crane, JS, Thompson, BJ, Harrell, DC, Bressel, E, and Heath, EM. Comparison of high versus low eccentric-based resistance training frequencies on short-term muscle function adaptations. J Strength Cond Res 36(2): 332-339, 2022-Eccentric resistance training is beneficial for improving a number of performance and health metrics. However, the recommendations on eccentric training frequency have not been established. This study investigates the effects of volume-matched resistance training frequency comparing 1 vs. 3 training days per week of isokinetic multiple-joint eccentric training on strength and lower-body function adaptations during a 4-week training period. Thirty subjects were assigned to either 3 days per week (high-frequency [HF]) or 1 day per week (low-frequency [LF]) training conditions for 4 weeks. An eccentric dynamometer was used for the training and testing. Eccentric strength and vertical jump (VJ) measures were taken at Pre, Mid (2 weeks), and Post (4 weeks) intervention. Soreness (visual analog scale [VAS]) and rate of perceived exertion (RPE) were taken throughout the training period. There was no group × trial interaction for eccentric strength (p = 0.06) or VJ (p = 0.87). For eccentric strength, all trials were significantly different (p < 0.001) from each other. For VJ, there was a main effect for trial such that VJ increased from Pre to Post (p < 0.001) and Mid to Post (p < 0.01). High frequency reported lower RPE (p < 0.01) and soreness (p = 0.04) compared with LF. Both HF and LF protocols elicited large (36.8 and 27.4% strength increases, respectively) and rapid neuromuscular adaptations for improved strength. Eccentric-based workload may be dispersed across a given period to allow for reduced soreness and perceived exertion levels without compromising neuromuscular adaptations. Some eccentric training transfer to functional (VJ) task may also be observed, independent of training frequency.

Changes in knee extension and flexion maximal and rapid torque characteristics during a collegiate women's soccer season. J Strength Cond Res 36(5): 1389-1395, 2022-The purpose of this study was to assess the changes in maximal and rapid torque capacities of the knee extensor and flexor muscles over the course of a competitive season in NCAA Division II women's soccer players. Eighteen female soccer athletes performed 2 maximal voluntary isometric contractions (MVICs) of the knee extensor and flexor muscles before, during, and at the end of the competitive season. Peak torque (PT) and rate of torque development (RTD) at 50 (RTD50), 200 (RTD200), and 100-200 (RTD100-200) milliseconds were extracted from each MVIC for both legs. The rapid (RTD50) to maximal force ratio (RTD:PT), hamstring-to-quadriceps (H:Q) strength ratio, and bilateral strength differences were also calculated. Results indicated that PT, RTD50, and RTD200 decreased 11-21% from the preseason to the midseason for the knee extensors (p < 0.02) and RTD50 increased approximately 11% from the midseason to the end of season for the knee flexors (p < 0.01). Rate of torque development-to-PT ratios for the knee extensors and flexors increased 12-25% at the end of the season (p < 0.05). Also, H:Q strength ratios using PT, RTD50, and RTD200 increased 12.5-24% after the season started (p = 0.001-0.04). There were no bilateral strength differences (dominant vs. non-dominant limbs) across the season (p > 0.05). These findings suggest that participation in a soccer season can change maximal and rapid torque production of the knee extensors and flexors. Coaches and clinicians should consider incorporating a season-long strength training and maintenance plan for soccer players with the aim to improve athletic performance and minimize the risk of musculoskeletal injuries to the lower extremities.

Effects of strength and conditioning on maximal isometric strength, motor unit behavior, and concentric isokinetic peak torque in middle-school boys. J Strength Cond Res 36(5): 1318-1326, 2022-It has long been theorized that improvements in muscle strength in young athletes are mediated by motor unit adaptations. The ability to decompose surface electromyographic signals obtained during isometric contractions now allow for such research questions to be answered. We examined changes in isometric and concentric isokinetic strength, as well as vastus lateralis motor unit behavior, after 16 weeks of strength training and conditioning in middle-school aged boys. Nine boys (mean ± SD age = 12 ± 1 years) participated in training. Five boys (age = 13 ± 1 years) served as control subjects. The training subjects performed 90 minutes of high-intensity, multi-joint exercise twice per week. Assessments of unilateral maximal voluntary isometric contraction (MVIC) force of the knee extensors, concentric peak torque at velocities of 60, 180, and 300°·s-1, and vastus lateralis motor unit data during 50 and 80% MVIC tests were performed. Strength training and conditioning did not improve MVIC force. Greater training-induced strength increases were observed at faster isokinetic velocities, with a large effect size at 300°·s-1 (d = 0.813). The slopes and y-intercepts of the mean firing rate vs. recruitment threshold relationship and the action potential amplitude vs. recruitment threshold relationship were unaffected by training. Sixteen weeks of middle-school strength and conditioning did not enhance maximal isometric strength or vastus lateralis motor unit control, but improvements were observed during rapid isokinetic muscle actions. Given the lack of training (multi-joint) vs. testing (single-joint) specificity, we propose that motor unit adaptations in youth are task specific.

BACKGROUND: The psychomotor vigilance task (PVT) is a commonly used test that effectively assesses neurobehavioral alertness. The originally developed PVT is 10 minutes in duration, which presents practical and logistical issues, particularly when administered to large samples or on a repetitive basis. More recently the PVT has been used in both 3- and 5-minute formats. While both of these durations have been shown to be field sensitive to identify impairments from sleep- and fatigue-related interventions, the 5-minute version has been suggested to be more valid than the 3-minute. However, while these have shown field-validity in a number of working populations, there is a paucity of data reporting the test-retest reliability statistics of the 5-minute PVT, particularly in working-aged females. The purpose of the study was to examine the test-retest reliability of a comprehensive set of PVT variables for the 5-minute PVT in a population of working-aged females (20-63 years). NEW METHOD: Participants reported to the laboratory on two separate days and performed a 5-minute PVT on each occasion. Outcome measures included the mean reaction time (MRT), fastest and slowest 10% of reaction times (F10RT% and S10RT%, respectively), standard deviation of reaction times (SDRT) as well as error-based metrics including major and minor lapses, anticipations, and false starts. In addition, total errors (ERR) were computed as a composite of all types of errors. Reliability statistics were reported as intraclass correlation coefficients (ICCs), standard error of measurement (SEM, SEM%), and minimal difference to be considered real (MD, MD%). Systematic error was also evaluated between sessions. RESULTS: Overall high reliability was shown for the MRT (ICC = 0.79, SEM% = 4.14%) and F10RT% (ICC = 0.83, SEM% = 4.43%) variables, with moderate relative reliability (based on ICCs) for the false starts, ERR, and sleepiness scale (ICC = 0.50 - 0.70) variables but these all exhibited poor absolute reliability (based on SEM% values of 32.60 - 168.69%). Poor relative reliability was found for the SDRT and S10RT% variables (ICCs < 0.50) but the S10RT% variable had good absolute reliability (SEM% = 7.12%). The minor and major lapses and anticipations variables had too few of event occurrences for a confident determination of the reliability. CONCLUSIONS: The finding that the MRT variable displayed systematic error (P = 0.01) indicating that a learning curve may have been present, but the F10RT% did not show systematic error, suggests the F10RT% may be the most reliable PVT variable in a 5-minute duration test. These findings provide researchers and practitioners with reliability statistics that may help in determining which variable(s) to use, and which to avoid when specifically conducting 5-minute PVT assessments, particularly in a population of working-aged females. These results suggest that the 5-minute PVT can be used in place of the 10-minute version, if used appropriately.

The ability of older adults to perform activities of daily living is often limited by the ability to generate high mechanical outputs. Therefore, assessing and developing maximal neuromuscular capacity is essential for determining age-related risk for functional decline as well as the effectiveness of therapeutic interventions. Interventions designed to enhance neuromuscular capacities underpinning maximal mechanical outputs could positively impact functional performance in daily life. Unfortunately, < 10% of older adults meet the current resistance training guidelines. It has recently been proposed that a more "minimal dose" RT model may help engage a greater proportion of older adults, so that they may realize the benefits of RT. Eccentric exercise offers some promising qualities for such an approach due to its efficiency in overloading contractions that can induce substantial neuromuscular adaptations. When used in a minimal dose RT paradigm, eccentric-based RT may be a particularly promising approach for older adults that can efficiently improve muscle mass, strength, and functional performance. One approach that may lead to improved neuromuscular function capacities and overall health is through heightened exercise tolerance which would favor greater exercise participation in older adult populations. Therefore, our perspective article will discuss the implications of using a minimal dose, submaximal (i.e., low intensity) multi-joint eccentric resistance training paradigm as a potentially effective, and yet currently underutilized, means to efficiently improve neuromuscular capacities and function for older adults.

Since the reactive strength index (RSI) and reactive strength index-modified (RSI-mod) share similar nomenclature, they are commonly referred as interchangeable measures of agility in the sports research literature. The RSI and RSI-mod are most commonly derived from the performance of depth jumping (DJ) and countermovement jumping (CMJ), respectively. Given that DJ and CMJ are plyometric movements that differ materially from biomechanical and neuromotor perspectives, it is likely that the RSI and RSI-mod measure distinct aspects of neuromuscular function. The purpose of this investigation was to evaluate the association and agreement between RSI and RSI-mod scores. A mixed-sex sample of NCAA division I basketball athletes (n = 21) and active young adults (n = 26) performed three trials of DJ from drop heights of 0.51, 0.66, and 0.81 m and three trials of countermovement jumping. Using 2-dimensional videography and force platform dynamometry, RSI and RSI-mod scores were estimated from DJ and CMJ trials, respectively. Linear regression revealed moderate associations between RSI and RSI-mod scores (F = 11.0-38.1; R2 = 0.20-0.47; p < 0.001-0.001). Bland-Altman plots revealed significant measurement bias (0.50-0.57) between RSI and RSI-mod scores. Bland-Altman limit of agreement intervals (1.27-1.51) were greater than the mean values for RSI (0.97-1.05) and RSI-mod (0.42) scores, suggesting poor agreement. Moreover, there were significant performance-dependent effects on measurement bias, wherein the difference between and the mean of RSI and RSI-mod scores were positively associated (F = 77.2-108.4; R2 = 0.63-0.71; p < 0.001). The results are evidence that the RSI and RSI-mod cannot be regarded as interchangeable measures of reactive strength.

Many diseases, disabilities, and mental health conditions associated with aging can be delayed or prevented through regular exercise. Several barriers to exercise, many of which are exacerbated in rural communities, prevent mid-life and older adults from accessing its benefits. However, recently, a racquet sport named pickleball has become popular among older adults, and it appears to overcome some of these barriers. We conducted a feasibility study to evaluate the impact of a six-week pickleball intervention on measures of muscle function, cognitive function, perceived pain, and cardio-metabolic risk, as well as several psychosocial factors contributing to adherence in sedentary rural participants. Participants improved their vertical jump, cognitive performance, and reported a decrease in self-reported pain, suggesting improved physical and cognitive health across the sample. Participants also reported high levels of satisfaction and demonstrated good adherence over the duration of the study. Perhaps of greatest value was the overwhelmingly positive response from participants to the intervention and follow-up interviews reporting a desire to continue pickleball play beyond the study period. Overall, pickleball appears to be a promising intervention to, (1) elicit functional- and cognitive-related improvements, and (2) motivate mid-life and older adults to adhere to exercise sufficiently long to benefit their health.

OBJECTIVE: The purpose of this study was to examine the effects of a competitive soccer season on rapid activation properties of the knee extensors and flexors in Division II female soccer players. METHODS: Eighteen collegiate female soccer players participated in the present study, however, due to injuries during the season a final sample of 16 players were included for study analysis. Participants performed two maximal voluntary isometric contractions (MVICs) of the knee extensors and flexors before, during, and at the end of a competitive college soccer season. Electromyography root mean square (EMG RMS; µV), rate of EMG rise (RER; %Peak EMG•s-1), and electromechanical delay (EMD; ms) were examined on both legs for the knee extensors and flexors. RESULTS: EMG RMS at early time intervals (0-50, 0-100, and 50-100 ms) and RER at 0-75 ms for the knee extensors and flexors significantly increased from the pre-season to the end of the season (P≤0.010-0.026, η2=0.36-0.81). EMD of the knee flexors significantly decreased at the mid-season and the end of the season compared to the pre-season (P<0.001, η2=0.95). CONCLUSIONS: These findings may have important implications for monitoring improvements on thigh neuromuscular activation and developing lower extremity injury prevention strategies during a competitive collegiate female soccer season.

Background: The female nurse exhibits a multitude of personal and environmental characteristics that renders this population especially prone to fatigue. The consequences of fatigue in nurses are widespread and impactful at the personal, organizational, and societal levels. These include high injury rates and burnout in the nurse and poor patient and organizational outcomes. Objective: This article discusses the implications of fatigue in female nurses, including the impacts of fatigue across multiple entities (e.g., worker, patient, organization). It also reviews the current state of the research, including recent work on nurse fatigue and work schedule characteristics, and key areas for future work that would help empirically establish approaches to counter the detrimental and widespread effects of fatigue. Method: A narrative literature review was conducted resulting from an analysis of the literature limited to peer-reviewed studies. Results: A confluence of factors combines to elevate the prevalence and risk of fatigue in the female nurse. Numerous measures have established that performance-based fatigue results from nursing work schedules in nurses. Data also demonstrate that fatigue accumulates across successive shifts. Recent evidence supports the use of objective fatigue measures, including psychomotor reaction time and muscle function-related variables. Current gaps in the literature are delineated in the text. Conclusions: Strategic and well-designed research studies, as well as recent technological advances in fatigue tracking tools have the potential to help workers, administrators, and organizations develop fatigue management programs that could reduce the heavy burdens of fatigue on a multitude of health, safety, and economical outcomes.

BACKGROUND: Reactive balance is the last line of defense to prevent a fall when the body loses stability, and beneficial effects of various exercise-based interventions on reactive balance in older adults have been reported. However, their pooled evidence on the relative effects has yet to be described. OBJECTIVE: To review and evaluate the comparative effectiveness of various exercise-based interventions on reactive balance in older adults. METHODS: Nine electronic databases and reference lists were searched from inception to August 2021. Eligibility criteria according to PICOS criteria were as follows: (1) population: older adults with the mean age of 65 years or above; (2) intervention and comparison: at least two distinct exercise interventions or one exercise intervention with a no-exercise controlled intervention (NE) compared in each trial; (3) outcome: at least one measure of reactive balance; (4) study: randomized controlled trial. The main network meta-analysis was performed on data from the entire older adult population, involving all clinical conditions as well as healthy older adults. Subgroup analyses stratified by characteristics of participants (healthy only) and reactive balance outcomes (simulated slip or trip while walking, simulated forward falls, being pushed or pulled, and movable platform) were also conducted. RESULTS: Thirty-nine RCTs (n = 1388) investigating 17 different types of exercise interventions were included in the network meta-analysis. Reactive balance training as a single intervention presented the highest probability (surface under the cumulative ranking (SUCRA) score) of being the best intervention for improving reactive balance and the greatest relative effects vs. NE in the entire sample involving all clinical conditions [SUCRA = 0.9; mean difference (95% Credible Interval): 2.7 (1.0 to 4.3)]. The results were not affected by characteristics of participants (i.e., healthy older adults only) or reactive balance outcomes. SUMMARY/CONCLUSION: The findings from the NMA suggest that a task-specific reactive balance exercise could be the optimal intervention for improving reactive balance in older adults, and power training can be considered as a secondary training exercise.

PURPOSE: This narrative review provides an overview of the current knowledge of B-mode ultrasound-derived echo intensity (EI) as an indicator of skeletal muscle quality. METHOD: PubMed and Google Scholar were used to search the literature. Advanced search functions were used to find original studies with the terms 'echo intensity' and/or 'muscle quality' in the title and/or abstract. Publications that conceptually described muscle quality but did not include measurement of EI were not a focus of the review. RESULT: Importantly, the foundational premise of EI remains unclear. While it is likely that EI reflects intramuscular adiposity, data suggesting that these measurements are influenced by fibrous tissue is limited to diseased muscle and animal models. EI appears to show particular promise in studying muscular aging. Studies have consistently reported an association between EI and muscle function, though not all chronic interventions have demonstrated improvements. Based on the existing literature, it is unclear if EI can be used as a marker of muscle glycogen following exercise and nutritional interventions, or if EI is influenced by hydration status. Inconsistent methodological approaches used across laboratories have made comparing EI studies challenging. Image depth, rest duration, participant positioning, probe tilt, and the decision to correct for subcutaneous adipose tissue thickness are all critical considerations when interpreting the literature and planning studies. CONCLUSION: While some areas show conflicting evidence, EI shows promise as a novel tool for studying muscle quality. Collaborative efforts focused on methodology are necessary to enhance the consistency and quality of the EI literature.

BACKGROUND: Declines in muscle morphology and function are commonly reported as a consequence of aging. However, few studies have investigated the influence of age on a comprehensive set of muscle function-related measures (i.e., reactive strength, power, etc.) that focuses on a dynamic performance task such as the vertical jump across the adult life span. This study aimed to examine the effects of age on muscle morphology characteristics (muscle cross-sectional area [CSA] and echo intensity [EI]) and vertical jump height, power, and reactive strength index (RSI) in females. METHODS: Twenty-six young (22±2 years), 30 middle-aged (36±5 years), and 23 older (71±5 years) females participated in this study. Muscle CSA and EI were determined from ultrasound scans of the vastus lateralis. Countermovement jumps were used to assess jump height, RSI, movement time, and peak power (Pmax). RESULTS: Muscle CSA, jump height, and Pmax were higher for the young compared to the old and middle-aged (P≤0.027) and for the middle-aged compared to the old (P<0.001). Movement time and EI values were lower (P≤0.004) and RSI values were higher (P<0.001) for the young and middle-aged compared to the old; however, no differences were observed between the young and middle-aged (P=0.367-0.620). CONCLUSIONS: Of all the variables assessed in this study, RSI exhibited the greatest decline (76%) between the young and old females. Such findings highlight the importance of reactive strength when assessing age-related changes in neuromuscular performance.
