RESUMO

New methods of operative treatment of congenital dislocations in children are proposed as well as the methods of restoration of motility in the hip joint. All these developed methods tested in 300 patients (380 joints) exclude recurrent deformities and delay the development of coxarthrosis.

RESUMO

In the article is described a new method of treatment of chronic hematogenic osteomyelitis of femur in children in case of its total affection. The essence of the method consists in making a longitudinal groove along the whole length of a bone by means of chisels and milling cutters till the medullar canal is reached and removal of pus, sequesters, pathologic granulations and zones of excessive osteosclerosis. The cavity after bathing is subjected to ultrasound cavitation; fibrinous film and gauze plastrons, impregnated with hemostatic sponge, are placed in it with the proximal and distal ends connection to the tubular drainage in order to ensure permanent lavage of cavity with antiseptic solution and active vacuum aspiration of its content with the subsequent application of sutures on the wound, tight bandaging of a limb and its immobilization. 6 patients have been subjected to treatment by this method with good outcomes in all cases. The results have been followed through 2.5-8 years.

RESUMO

The paper is devoted to the surgical treatment of 99 senior children (103 joints) and adolescents with dysplastic deforming coxarthroses and pathologic dislocations and subluxations of the hip, namely to the reconstruction of the hip joint by means of creation of acetabuloplastic roofs making use of the migrating osteomuscular flap graft taken from the upper flaring portion of the ilium with the anterosuperior bone and the tailor's muscle attached to it, which has allowed to reduce the terms of the treatment, to interrupt the phenomena of coxarthrosis that are present, to improve the blood supply of the joint and the nutrition of the articular cartilage and to restore the function of the joint.

RESUMO

The paper is devoted to the surgical treatment of the children with dysplastic pre- and coxarthroses, namely to the reconstruction of the supraacetabular area of the hip joint by means of incomplete semicircular osteotomy, which has allowed to reduce the terms of the treatment, to exclude a further recurrence of the deformation and to interrupt the phenomena of arthrosis that are present. In all the cases of the 50 patients who had been operated (aged 3 to 14 years with the follow-up period up to 10 years) there were good anatomical and functional results. The average mark according to the CITO scale was 4.6.

RESUMO

The paper is devoted to the surgical treatment of children with dysplastic pre- and coxarthroses, particularly to reconstruction of the supra-acetabular++ area of the hip joint using a migrating osteo-muscular+ tissue complex, i.e. a graft taken from the upper flaring portion of the ilium together with the growth area and the tailor's muscle attached to it, which allows to reduce the terms of the treatment, to exclude a further recurrence of the deformation and to interrupt the present phenomena of arthrosis. In all the 52 operated patients aged 5 to 12 years with the follow-up periods up to 6 years the anatomical and the functional results were good. The mean rating according to the Central Institute of Traumatology and Orthopaedics was 4.8 points.

RESUMO

Operative interventions were performed in patients both with the presence of hip dislocation and with residual defects of the joint after its setting. The best results were obtained after surgical interventions developed by the authors with the use of growing transplants on feeding muscular pedicles. In all 141 patients good results were obtained. The average of the scores according to CITO was 4.41.

RESUMO

The work is devoted to operative treatment of children with repeated dislocations and ankyloses of the hip joint after operative treatment of congenital dislocation of the hip. The dura mater, demineralized allocups and polymer self-resolving film were used as interlayers. Best results were obtained in arthroplasty with using demineralized allocups, polymer self-resolving film or their combinations.
