RESUMO

OBJECTIVE: During infection, metabolism and immunity react dynamically to promote survival through mechanisms that remain unclear. Pro-opiomelanocortin (POMC) cleavage products are produced and released in the brain and in the pituitary gland. One POMC cleavage product, alpha-melanocyte-stimulating hormone (α-MSH), is known to regulate food intake and energy expenditure and has anti-inflammatory effects. However, it is not known whether α-MSH is required to regulate physiological anti-inflammatory responses. We recently developed a novel mouse model with a targeted mutation in Pomc (Pomctm1/tm1 mice) to block production of all α-MSH forms which are required to regulate metabolism. To test whether endogenous α-MSH is required to regulate immune responses, we compared acute bacterial lipopolysaccharide (LPS)-induced inflammation between Pomctm1/tm1 and wild-type Pomcwt/wt mice. METHODS: We challenged 10 to 14-week-old male Pomctm1/tm1 and Pomcwt/wt mice with single i.p. injections of either saline or low-dose LPS (100 µg/kg) and monitored immune and metabolic responses. We used telemetry to measure core body temperature (Tb), ELISA to measure circulating cytokines, corticosterone and α-MSH, and metabolic chambers to measure body weight, food intake, activity, and respiration. We also developed a mass spectrometry method to measure three forms of α-MSH produced in the mouse hypothalamus and pituitary gland. RESULTS: LPS induced an exaggerated immune response in Pomctm1/tm1 compared to Pomcwt/wt mice. Both groups of mice were hypoactive and hypothermic following LPS administration, but Pomctm1/tm1 mice were significantly more hypothermic compared to control mice injected with LPS. Pomctm1/tm1 mice also had reduced oxygen consumption and impaired metabolic responses to LPS compared to controls. Pomctm1/tm1 mice had increased levels of key proinflammatory cytokines at 2 h and 4 h post LPS injection compared to Pomcwt/wt mice. Lastly, Pomcwt/wt mice injected with LPS compared to saline had increased total α-MSH in circulation 2 h post injection. CONCLUSION: Our data indicate endogenous α-MSH contributes to the inflammatory immune responses triggered by low-dose LPS administration and suggest that targeting the melanocortin system could be a potential therapeutic for the treatment of sepsis or inflammatory disease.

BACKGROUND: Parenting programs are delivered in many low income countries in an effort to address the effects of insufficient cognitive stimulation, major social-emotional risk factors related to inadequate caregiver sensitivity and responsiveness, maternal depression, violence and biological risk factors. However, the outcomes of these programs are often untested because of methodological limitations in the existing evaluation tools available for cross-cultural research, and especially those related to social-emotional development and functioning. METHOD: This study takes an approach that involves organizing data from focus groups collected following caregiver participation in a parenting program, Learning Through Play (LTP) delivered in 11 low income countries. The LTP program has as its aim to increase parent knowledge and influence parental practice regarding the essential components of early development of children aged birth to 6 years. The purpose of systematically organizing focus group data was to illustrate a means by which program developers worldwide can discuss outcomes of program participation, particularly in social-emotional domains that have been overlooked in past studies on parenting programs. RESULTS AND CONCLUSION: The findings from this study suggest that qualitative outcome data can be organized to make the social-emotional outcomes of participation in parenting programs more visible.

Two thermophilic lab-scale reactors of 5 L were operated on a daily fed basis. Digester T1 received raw sludge (control) and digester T2 was fed with raw sludge plus metallic micronutrients and a bacilli additive. Raw sludge was obtained from a municipal chemically enhanced primary treatment plant. The effect of additives was clear on methane production, since on day 50, digester T2 produced 900 ml more methane than T1, an increase of 64%. On day 80, T2 reached twice the production of biogas of T1. Volatile solid removal (% VSR) in T2 increased to 29%; while T1 achieved only 15%. Acetic acid concentration in T2 diminished to 100 mg/L, which related to the higher biogas production. Based on the Mexican biosolids standard, the digested sludge reached Class A biosolids, in both digesters: fecal coliforms were reduced to less than 1000 MPN/gTS; Salmonella spp was totally eliminated and helminth egg counts were lower than one viable egg per gram of total solids.

Two additives (lyophilized bacilli and enzymes) and a solution of micronutrients (Fe, Co, Ni and Mo) were tried separately and in combination, in order to assess their effect on the anaerobic digestion of waste sludge from an enhanced primary treatment (EPT) of municipal wastewater. Three batch tests were carried out in serological bottles. In the first test, addition of bacilli increased production of methane from day 11 and at day 1 7 the production was 95% greater than the control. In that experiment, the concentration of volatile fatty acids (VFAs) was 1,391 mg/L, 40% lower than the control. In the second test, the combination of micronutrients with bacilli, reached from the first days a better methane production than the control, 167% higher in day 17. At the end of the experiment, this combination achieved a lower concentration of VFAs and a greater percentage of volatile solid removal than the rest of the treatments. The third test was based on an experimental design in order to statistically determine the best doses of bacilli additive and micronutrients. The anaerobic thermophilic digestion of sludge from aluminium sulfate EPT will be improved with the addition of Fe: 4.5 mg/g VS, Ni: 0.75 mg/g VS, Co: 0.45 mg/g VS, Mo: 0.09 mg/g VS and bacilli additive: 12 mg/g VS.

OBJECTIVES: To determine the outcome and the place of transluminal angioplasty (ATL) in the treatment of severe limb ischaemia. MATERIALS AND METHODS: Seventy two legs at stage III and IV of Fontaine's classification have been exclusively treated by endoluminal procedures. The success was valued both on the haemodynamic post-operative improvement of the run-off flow and on the clinical statut leading to the conservation at mid-term of a functional limb. Patency and survival rate had been valued by actuarial method. RESULTS: Seventy percent of the limbs were haemodynamically improved. For the global population, a 48% clinical success rate was obtained at 6 months but 30.5% of limbs were loss. Primary patency rate was respectively 79, 71 and 68% at 6, 12 and 24 month. The quality of the run-off arteries has been the most influential factor. CONCLUSION: Endoluminal treatment of chronic limb ischaemia had lead to a clinical improvement in 48% of cases. Multi-stages and distal atherosclerotic disease of this patients limits ATL indications which results depend of run-off quality.
