Reconstruction of lower eyelid and periorbital district: an orbicularis oculi myocutaneous flap.

The upper eyelid has long been considered a good source of skin graft, but it is infrequently used as a unipedicled flap, based either medially or laterally, to reconstruct the skin of the lower eyelid. Here we describe a procedure in which we use the upper eyelid skin and underlying orbicularis muscle as a myocutaneous flap to reconstruct the lower eyelid and periorbital region.

Reconstruction of the upper lip and philtrum.

The procedure used to repair upper lip defects depends on various factors: size and location of the defect, the conditions of neighbouring tissues, and the aetiology of the deformity. A single-stage procedure rarely gives a satisfactory cosmetic outcome in patients who have also lost the philtrum or a philtral crease. Here, we describe a single-step procedure for the reconstruction of the upper lip and philtrum that resulted in a good aesthetic appearance.

The introduction of sutures using Polyglecaprone 25 (Monocryl--Ethicon SpA) has facilitated the use of intradermal suture with separate stitches owing to its flexibility and softness, with extremely satisfactory aesthetic results. The authors report their experience regarding the use of this new material from May 1995 to April 1996 in 250 cases of intradermal suture with separate stitches. The paper also describes a few technical details and emphasis their advantages.

[Hearing changes in patients with cleft lip and palate: clinico-statistical contribution]. / Alterazioni uditive in soggetti portatori di schisi labio-maxillo-palatina: contributo clinico-statistico.

The otological complications which may occur in subjects with cleft lip and palate due to middle ear and tubal impairment related to velopharyngeal musculature are discussed. Three groups of patients aged 3-15 years have been submitted to clinical examination, audiometry and impedance measurement. In 2/3 of cases there was hearing impairment between 20-40 dB and 72% out of children younger than 8 years a pathological tymponometric curve was observed. These results confirm the positive relationship between cleft palate and hearing impairment and suggest that these children must be followed-up very carefully in oder to prevent middle ear and tubal alterations.

The use of a myocutaneous trapezium flap as a definitive cure for recurrent dermatofibrosarcoma protuberans of the nucha is proposed. It is also emphasised that the reconstructive technique suggested involves no functional sacrifice of the muscle, which may be repositioned once it ceases to function as a cutis vector.
