Complete obturation of the nasopharynx can cause functional problems with breathing and swallowing due to obstruction of the nasal airway. A technique is described whereby a one way valve was incorporated in an obturator to allow normal nasal inspiration whilst maintaining an oropharyngeal seal during swallowing and speech.

The use of myocutaneous flaps has revolutionized reconstructive surgery following tumor resections in the head and neck area. The platysma myocutaneous flap described almost a century ago has not received much attention, although it has been reported to be a satisfactory technique. Its successful use in 2 patients with squamous cell carcinoma of the tongue, a man of 61 and a woman of 77, is presented.

Congenital granular cell epulis is an uncommon pedunculated tumor of the upper alveolar mucosa found only in newborns. The lesion is not usually associated with other congenital deformities, but presents a major esthetic problem and interferes with nursing. We present a case of a double epulis, 7 cm in diameter, in a 2-day-old girl. Treatment is simple excision, and no recurrences have been reported. The lesion must be differentiated from granular cell myoblastoma.

Bacteremia due to Vibrio cholerae is rare. Each of 15 cases previously reported in the English language literature occurred in the setting of immune deficiency. We describe an instance of non-serogroup O1 V. cholerae septicemia in an otherwise healthy patient. Susceptibility to such infection may have been enhanced by a prior gastrectomy for duodenal ulcer.
