The in vitro study of the influence of photodynamic action (with the use of photosensitizer "Photosens", laser and non-laser irradiation) on the strains of the main representatives purulent wound microflora was carried out. Staphylococcus aureus, S. epidermidis, Pseudomonas aeruginosa, Escherichia coli, Proteus mirabilis cultures have been isolated from wound secretions and identified. The photosensitizer was shown to produce no bactericidal effect by itself. Irradiation with laser and non-laser light sources induced a sharp decrease in the number of viable cells in the cultures under study. The irradiation of the photosensitizer led to its activation, manifested in bactericidal action. The results thus obtained confirm good prospect of using photodynamic therapy for the treatment of purulent wounds.

Immobilization of cellulose and polyacrylic acid on a grafted copolymer increased significantly the stability of proteolytic enzymes to inactivation by urea. Materials containing immobilized proteolytic enzymes and urea and displaying a combined biological activity were obtained.

Study results of the laser irradiation impact on the blood lipid and phospholipid components and membrane erythrocyte in patients with diabetes mellitus and pyo-necrotic injuries of lower extremities when treated by laser intravenously or epicutaneously. The results prove, that the laser irradiation of low intensity favourably affects the blood components in a complex treatment of diabetes mellitus. Intravenous irradiation positive dynamics are more easily observed.

Joint immobilization of a proteolytic enzyme (terrilytin, protease C, or collytin) and an antimicrobial compound or the bacteriolytic enzyme lysozyme in the structure of polyvinyl alcohol was studied. Changes in the activity of immobilized enzymes upon x-ray sterilization were studied. These materials were highly effective in wound healing.

Properties of joint immobilization of a proteolytic enzyme (terrilytin, trypsin, collytin, or protease C) and an anti-microbe compound to cellulose copolymer containing carboxyl groups were investigated. It was established that the molecule of anti-microbe substance containing a few base groups enhanced stability of the enzyme immobilized, most likely due to additional fixation of the enzyme macromolecule. Changes in activity of the materials containing both protease and anti-microbe compound were studied upon gamma-sterilization and subsequent prolonged storage. The materials with combined biological action were demonstrated to accelerate markedly wound cleansing and healing.

In the experiment on 218 white rats, it was established that preliminary influencing upon the site of a future incision with the infrared laser radiation in constant regime within the range of from 1 to 150 mW (dosage of radiation per 1 field of 0.06-9.3 J/cm2) facilitated pronounced stimulation of the processes of collagen formation and significant increase in strength of a forming scar. In increasing the power of radiation, the reparative processes in a wound are slowed and disturbed; in power of 250 mW, a wound heals by second intention, in average, after (32.6 +/- 3.2) days.

On the basis of experimental studies conducted on 535 albino Wistar rats and clinical observations over 125 patients, and from complex appraisal of morphological, biochemical, and tensiometric data the conclusion is drawn that optimal operative cutting of tissues is achieved by means of far infrared carbon dioxide laser beam with a wavelength of 10.6 microns. Comparison of these characteristics showed the strength of the postoperative scar to be much less in intraoperative tissue cutting with a steel scalpel or high-energy beam of an AIG laser with a wavelength of 1.06 microns. Low-intensity laser radiation increased the strength of the postoperative scar in the early periods in all groups. Tissue cutting with a high-energy carbon dioxide laser beam with a wavelength of 10.6 microns in combination with pre- and postoperative exposure of the zone of the incision to low-intensity 0.89 micron wavelength laser beam considerably increases the strength of the postoperative scar and is the method of choice in the treatment of aseptic wounds.

The article analyses the results of treatment of 150 patients with purulent diseases of the soft tissues of various localization, in which after surgical treatment the purulent focus immobilized trypsin (50 patients), trypsin with lysozyme immobilized on gauze (50 patients), and antiseptics (50 patients) were used for topical treatment of the formed wounds. The results of the treatment showed that topical prolonged enzyme therapy of purulent wounds with the use of trypsin immobilized on gauze and that with lysozyme promotes cleansing of the wounds, reduces microbial dissemination, intensifies reparative processes, and reduces the terms of treatment.

Spongy dressing materials made from alginic acid salts, specifically immobilized with terrilitinum (teralginum) were found to reduce the period of wound purification from suppurative and necrotic tissues and to facilitate more rapid, viz. 1.5-fold compared to the control, patient recovery, particularly after preliminary wound treatment with defocussed CO2 laser beam. In the group on teralginum, the cytological study demonstrated more intensive wound purification from microflora, more active growth and differentiation of reparation cells, specifically of fibroblasts, which influences the results of accelerated wound healing.

The results of treatment of 370 patients with purulent soft tissue diseases have shown that laseroenzymotherapy of the suppurating wounds contributed to their rapid cleaning, maintenance of microbial colonization at a stable low level, activation of reparative processes. This permitted to prepare a wound surface for closure at the nearest days after the operation.

The results of treatment of 245 patients with purulent diseases of the soft tissues are discussed. The course of the wound process was most favourable with the use of immobilized trypsin on gauze in the management of wounds formed after surgical debridement of purulent foci by means of CO2 laser. The method made it possible to accelerate the processes of necrolysis, provide a stable low level of dissemination of bacteria in the wound tissues, reduce the term of preparation of the wound for the application of secondary sutures and the terms of healing.

The suture material and the method of its application play an important role in the prevention of wound complications. With the use of Nichrome wire thread as the suture material in surgical interventions, the number of postoperative complications significantly was decreased. The Nichrome thread is also used expediently in operative interventions on patients with purulent diseases of the soft tissues, including cases after primary surgical treatment of purulent foci by carbon dioxide laser.

The results of ++clinico-bacterial examination of 168 patients provide evidence for the efficacy of CO2-laser treatment of a purulent wound; it reduces contamination of the tissues, 1,000-10,000-fold as a result of which surgical debridement of the purulent wound is completed in most cases by the application of a primary suture. The use of immobilized proteolytic enzymes in the postoperative period facilitates further cleansing of the wound, prevents the development of secondary infection, accelerates the change of the wound process to the phase of regeneration, promotes an uneventful postoperative course and healing of the wound within a lesser period as compared to the control group.

A bioluminescent rapid method (a three-hour test) has been developed to assess the susceptibility of microflora to antibiotics. It is based on comparison of intracellular ATP content in a sample (pus) after 3 h incubation at 37 degrees C in liquid culture medium (dilution: sample/culture medium--1:9) on a shaker in the presence and absence (control) of a single antibiotic concentration. A criterion has been offered for quantitative estimation of antibiotic susceptibility of microorganisms. The authors have optimized the measurements of intracellular ATP by the rapid bioluminescent method using immobilized firefly luciferase-based ATP reagent. The results of the method are in good correlation with those of the standard agar diffusion technique.

The article analyses treatment of 312 patients among which 156 were treated with the use of a CO2 laser and 156 by the traditional method. Study of the results showed that with the use of the CO2 laser the duration of in-patient treatment after radical operation was reduced by 5 days and out-patient rehabilitation by 13 days. the number of postoperative complications reduced considerably, single-stage radical operation could be carried out in the acute period of the disease, an operation for excision of a suppurating cyst could be completed by applying primary sutures with complete closure of the wound in 71.7% of cases, and that a recurrence of the disease could be prevented. Wide introduction of CO2 laser in surgical treatment of epithelial passages and cysts of the coccyx is recommended.

The article discusses experience in surgical treatment of 480 patients with the most common diseases of the anorectal region (acute and chronic haemorrhoids, acute and chronic paraproctitis, polyps, chronic fissures of the anal canal) in which special laser tools were used. The carbonic acid laser Skalpel-1 with a wave length of 10.6 microns and power of 30 W was used as the source of laser radiation. Methods of operations with the use of these tools are described in detail. The advantages of laser instruments in the treatment of patients with the above listed diseases are shown on a rich clinical material.
