RESUMO

Experience in operative treatment of 91 patients with spacious cysts of jaws occupying from 1/3 to capital DSE, Macedonian and more of the lower jaw volume was presented. In 36 patients keratocysts were found and in 55 patients teeth containing cysts. In the latter case it is obligatory to remove fully the cyst capsule because its epithelium is capable of metaplasia and neoplastic transformation. Patients were treated with the use of 2 stage method. 1st stage included decompression operation of cystotomy type; 2nd stage was performed 1-2 or more years later (depending upon primary cyst size) and consisted of remaining cyst capsule removal with wound closure. Benefits of 2 stage operation were described.

RESUMO

Histiocytosis of the jaws in biopsies from 17 patients aged 19 to 51 years were studied at light and electron microscopy level. Birbek's bodies were found in Langerhans cells in all the biopsies confirming the diagnosis of Langerhans cell histiocytoma. The structure of Birbek's bodies changed depending on the treatment--radiation or chemotherapy.

RESUMO

Forty-six patients (21 men and 25 women) with keratocysts were treated at Dental Surgery Chair of the Moscow Medical Stomatological Institute. Cystectomy was carried out in 26 patients, two-staged treatment in 20. Histologic examination of the operative material confirmed the diagnosis. A two-staged method of treatment of this disease in an outpatient setting has been developed. Cystectomy was carried out in cases with cysts up to 2-3 cm in diameter, it did not destroy the cortical plate of the jaw; it was performed similarly as total biopsy and was the final treatment modality. In cases with larger cysts, when the thickness of the cortical layer was less than 0.5 cm and there was a risk of fracture, two-staged treatment was resorted to: the first step, decompression performed similarly as cystostomy and the second, cystectomy a year or more after the first stage, when the size of the cyst reduced to 1.5-2 cm in diameter. The patients were followed up for up to 2-5 years after the treatment. Hence, correct and timely diagnosis of the disease and a two-staged method of treatment of keratocysts developed at out Chair bring about good results with the least injury to the patients.

RESUMO

Eighty-one patients with histiocytosis X involving the jaw bones were followed up for 30 years; the patients presented with involvement of the lungs, skin, mucosa, other bones of the skeleton, and diabetes insipidus signs. In 58 of the 81 patients eosinophilic granuloma eventuated in a generalized process: chronic xanthomatosis or Hand-Schüller-Christian's disease. This fact permits a conclusion that eosinophilic granuloma is an initial stage of the same systemic disease, histiocytosis X, and that subdivision of histiocytosis X into 3 forms, accepted in literature, is undesirable, for it erroneously presents the essence of the process. Therapeutic recommendations are given.
