RESUMO

Microorganisms with multiple ecological functions can be a useful biotechnological resource in integrated pest- and disease-management programs. This work aimed to investigate the potential endophytic and virulent effects of a strain of Purpureocillium lilacinum on organic cultivation in Brazil. Specifically, the strain's ability to establish itself as an endophyte in common bean, soybean, and sunflower plants when inoculated via seed was evaluated. Furthermore, its antifungal activity against phytopathogens and its pathogenicity and virulence against insects of the order Lepidoptera, Coleoptera, and Hemiptera were evaluated. Furthermore, the strain was evaluated for its biochemical and physiological characteristics. For virulence bioassays, the experiments were conducted under a factorial scheme (2 × 3), with the following factors: (a) fungal inoculation and control without inoculum and (b) types of inocula (blastospores, aerial conidia, and metabolites). The treatments were sprayed on insect species at different stages of development. In summary, it was found that the SBF054 strain endophytically colonized the common bean, with partial recovery from the root tissues of soybean and sunflower plants, 30 days after inoculation; suppressed 86% of Rhizoctonia solani mycelial growth in an in vitro assay; and controlled eggs, nymphs, and Euschistus heros adults. These multifunctional abilities are mainly attributed to the strain's mechanisms of producing metabolites, such as organic acids, soluble nutrients, and hydrolytic enzymes.

The present study aimed to evaluate the influence of titanium surface nanotopography on the initial bacterial adhesion process by in vivo and in vitro study models. Titanium disks were produced and characterized according to their surface topography: machined (Ti-M), microtopography (Ti-Micro), and nanotopography (Ti-Nano). For the in vivo study, 18 subjects wore oral acrylic splints containing 2 disks from each group for 24 h (n = 36). After this period, the disks were removed from the splints and evaluated by microbial culture method, scanning electron microscopy (SEM), and qPCR for quantification of Streptococcus oralis, Actinomyces naeslundii, Fusobacterium nucleatum, as well as total bacteria. For the in vitro study, adhesion tests were performed with the species S. oralis and A. naeslundii for 24 h. Data were compared by ANOVA, with Tukey's post-test. Regarding the in vivo study, both the total aerobic and total anaerobic bacteria counts were similar among groups (p > 0.05). In qPCR, there was no difference among groups of bacteria adhered to the disks (p > 0.05), except for A. naeslundii, which was found in lower proportions in the Ti-Nano group (p < 0.05). In the SEM analysis, the groups had a similar bacterial distribution, with a predominance of cocci and few bacilli. In the in vitro study, there was no difference in the adhesion profile for S. oralis and A. naeslundii after 24 h of biofilm formation (p > 0.05). Thus, we conclude that micro- and nanotopography do not affect bacterial adhesion, considering an initial period of biofilm formation.

Torrefaction emerges as an industrial process that increases the energy content of conventional biomass. Primary and secondary sludge are the main solid residues generated in the Effluent Treatment Plants of bleached kraft pulp mills, and can be considered as biomass. Typically, these wastes are sent to industrial landfills. The present study aimed to evaluate the technical feasibility of transforming the primary sludge (PS), secondary sludge (SS) and mixed sludges (MIX) into torrefied biomass for energy generation. Three temperatures (260, 290 and 320 °C) and three residence times (20, 40 and 60') were used in the sludge torrefaction process. Increasing the torrefaction temperature and residence time of the sludges produced several benefits on their physical and chemical properties. They promoted an increase in the heating value, due to the elimination of less energetic compounds and the concentration of the fixed carbon content; caused a reduction of moisture, with a consequent increase in the lower heating value of the sludges; and led to a high energy yield and an increased energy density, important parameters in sludges energy generation. The treatment at 320 °C for 60' obtained increases of 76%, 27% and 41% over the reference, for PS, SS and MIX, respectively.

Mining is an essential human activity, but results in several environmental impacts, notably the contamination of ground and surface water through the presence of toxic substances such as metals and sulfates in mine drainage. Permeable reactive barriers (PRB) have been applied to remediate this environmental impact, but the high costs associated with the maintenance of this system are still a challenge. The main objective of this study was to evaluate the use of kraft pulp mill alkaline residues, known as dregs and grits, as material for PRB, and to determine their capacity for retaining copper and sulfate. The work was carried out in laboratory adsorption kinetics assays, batch assays and column tests. Tests for elemental characterization, point of zero charge, acid neutralization capacity, total porosity, bulk density and moisture of the dregs and grits were conducted. The results showed high retention of Cu due to a chemical precipitation mechanism, notably for dregs (99%) at 5 min in adsorption kinetics. The grits presented similar results after 180 min for the same assay. Sulfate retention was effective at pH below 5, with an efficiency of 79% and 89% for dregs and grits, respectively. Dregs presented the best results for acid drainage remediation, notably with a solid:liquid (S:L) ratio of 1:10.

INTRODUÇÃO: Os aparelhos intraorais (AIO) possuem indicação para tratamento da Síndrome da Apneia Obstrutiva do Sono (SAOS) inclusive em pacientes com baixa adesão ao CPAP. A polissonografia com o AIO pode confirmar benefício terapêutico. MÉTODOS: Compararam-se os resultados polissonográficos de um AIO semiflexível em pacientes sob uso inadequado do CPAP por meio de estudo-piloto retrospectivo, incluindo 17 pacientes (11 homens e 6 mulheres) com 53,7 +- 7,8 anos, IMC de 27,5 +- 4,1kg/m2 e índice de apneia- -hipopneia basal (IAH) de 35,0 +- 19,8/h. Confirmados o uso inadequado ou recusa do CPAP, os pacientes receberam um aparelho com propulsão semiflexível (A-QUALITY) e, após titulação completa, novas polissonografias foram comparadas aos registros basais e com CPAP. Utilizou-se ANOVA para medidas repetidas e post-hoc Bonferroni (p < 0,05). RESULTADOS: Houve redução semelhante no IAH com AIO (7,7 +- 1,7/h) e CPAP (6,1 +- 1,6/h), ambos comparados ao basal (p < 0,001). O índice de dessaturação de O2 foi reduzido com AIO (2,4 +- 0,6/h) e CPAP (1,3 +- 0,6/h), ambos comparados ao basal (15,7 +- 3,8), (p <0,001). O índice de despertares também foi minimizado com AIO (7,2 +- 1,9/h) e CPAP (4,2 +- 0,7/h), ambos comparados ao basal (18,9 +- 5,3), (p<0,001). A eficiência do sono foi maior com o AIO comparado ao CPAP (87,2 +- 2,1 x 75,6 +- 3,9) (p<0,05). CONCLUSÃO: O tratamento com o aparelho selecionado resultou em melhora nos registros polissonográficos nessa amostra e pode ser indicado como alternativa ao CPAP em pacientes subtratados (AU)


INTRODUCTION: Intraoral appliances (IOA) are indicated for treatment of Obstructive Sleep Apnea Syndrome (OSAS) even in patients with poor adherence to CPAP. Polysomnography with IOA may confirm therapeutic benefit. METHODS: Polysomnographic results of a semiflexible IOA in patients under inadequate use of CPAP were compared by a retrospective pilot study including 17 patients (11 men and 6 women) aged 53.7 +- 7.8 years, BMI of 27.05 +- 4.1kg/m2 and basal apnea-hypopnea index (AHI) of 35.0 +- 19.8/h. Confirmed the inappropriate use or refusal of CPAP, the patients received a semiflexible propulsion device (AQUALITY) and, after complete titration, new polysomnographies were compared to baseline and CPAP registers. ANOVA was used for repeated and post-hoc Bonferroni measurements (p <0.05). RESULTS: There was a similar reduction in AHI with OA (7.7 +- 1.7/h) and CPAP (6.1 +- 1.6/h), both compared to baseline (p <0.001). The O2 desaturation index was reduced with IOA (2.4 +- 0.6 h) and CPAP (1.3 +- 0.6/h), both compared to baseline (15.7 +- 3.8) (p <0.001). Awakening rates were also minimized with IOA (7.2 +- 1.9/h) and CPAP (4.2 +- 0.7/h), both compared to baseline (18.9 +- 5.3) (p <0.001). Sleep efficiency was higher with IOA compared to CPAP (87,2 +- 2,1 x 75,6 +- 3,9) (p <0.05). CONCLUSION: Treatment with the selected device resulted in improved polysomnographic records in this sample and may be indicated as an alternative to CPAP in undertreated patients. (AU)

Multiple sclerosis (MS) is an immune-mediated disease of the central nervous system. Its treatment has focused on inflammation control as early as possible to avoid disability. Autologous hematopoietic stem cell transplantation (AHSCT) has been used for treating MS since 1996, with recent decisive results regarding benefits in long-term efficacy. Five patients followed up at an MS center in Belo Horizonte, Brazil, who had relapsing-remitting MS with high disease activity, underwent AHSCT between 2009 and 2011. They were evaluated clinically, with magnetic resonance imaging, and by the EDSS every six months after transplantation, up to July 2018. The patients were four women and one man, with ages ranging from 25-50 years, and time since disease onset ranging from 4-17 years at the time of the procedure. Four patients improved, one patient was stabilized, and all patients were free of disease activity after 5-9 years. Through improving patient selection and decreasing the time from disease onset, AHSCT could stop epitope spreading and disease progression. Despite multiple other therapeutic choices being approved for relapsing-remitting MS, AHSCT continues to be a treatment to consider for aggressive MS disease.

RESUMO A esclerose múltipla é uma doença imunomediada do sistema nervoso central. Seu tratamento tem sido focado no controle da inflamação o mais cedo possível para evitar incapacidade. O transplante autólogo de células tronco hematopoiéticas (TACTH) vem sendo usado para tratar esclerose múltipla desde 1996, e recentes resultados foram decisivos a respeito do benefício na eficácia a longo prazo. Cinco pacientes seguidos num centro de esclerose múltipla de Belo Horizonte, Brasil, que apresentavam forma clínica remitente recorrente com alta atividade de doença foram submetidos a esse tratamento de 2009 a 2011. Após o transplante foram avaliados clinicamente e com ressonância magnética, e escala de EDSS, a cada seis meses até julho de 2018. Os pacientes eram quatro mulheres e um homem, com idade entre 25 e 50 anos e tempo de doença variando de 4 a 17 anos na época do procedimento. Quatro pacientes melhoraram após a primeira avaliação e um manteve-se estável. Todos os pacientes permaneceram sem evidência de doença clínica ou radiológica de 5 a 9 anos após. Com uma seleção cada vez mais criteriosa de pacientes o TACTH pode interromper a disseminação do epítopo e controlar a progressão da doença. Apesar de várias outras opções terapêuticas aprovadas para esclerose múltipla o TACTH continua a ser uma forma de tratamento a ser considerada em casos de doença muito agressiva.

Prader-Willi syndrome (PWS) is a rare complex multisystem disorder and presents several aspects related to dentistry. The purpose of this review is to present current perspectives about oral health in patients with PWS. Delay development, hyperphagia, foamy and highly viscous saliva raise the risk of caries and contribute to tooth wear. Cariogenic foods uncontrolled consumption allows to obesity and dental problems progress worsening systemic disorders. These factors can be controlled. The success in follow-ups with caries free and oral health controlled demonstrate the importance of multidisciplinary team intervention corroborated by support at home from birth to adulthood. Thereby, current perspective on the disease is that there is possibility of proper maintenance of oral health in PWS patients. Guided care interferes positively with the overall well-being and quality of life of the individual with PWS and their family. A multidisciplinary team with a focus on teaching patients and family members will help minimize eventual problems.

PURPOSE: The aim of this retrospective cohort study was to assess the effects of annual maintenance over a 7-year period on the peri-implant health of patients rehabilitated with overdentures using clinical and radiographic parameters. MATERIALS AND METHODS: In order to be considered for inclusion in the study, patients had to have been rehabilitated with overdentures that had at least two implants placed in the mandible and four implants in the maxilla. Patients were divided into two groups: group 1 comprised individuals who had undergone annual maintenance over the previous 7 years, and group 2 comprised those who had not attended any dental appointment over the previous 7 years. All patients were submitted to clinical peri-implant examinations and radiographic assessments. RESULTS: Sixty-six patients received 396 implants of the external hexagon type, 132 in the mandible and 264 in the maxilla. Group 1 (44 patients with 264 implants) had a mean probing depth of 2.72 mm, while group 2 (22 patients with 132 implants) had a mean probing depth of 3.10 mm. It can be concluded that the mean of the variable probing depth is influenced by the presence of bleeding (P = .0005) and the implementation of maintenance (P = .0188), whereas plaque and local variables were not otherwise significant (P = .0605 and .0796, respectively). CONCLUSION: In this study, it was possible to observe better clinical conditions in individuals who had attended annual appointments for maintenance purposes.

In the search for the ideal treatment of periodontal disease various non-surgical techniques should be considered. The objective of this study was to evaluate the efficacy of full-mouth scaling (FMS) by clinical and microbiological parameters. 670 individuals were evaluated with 230 subjects meeting the selection criteria and were divided into two groups; 115 subjects treated with FMS and 115 treated with weekly sessions of scaling and root planning (SRP). The patient population had a mean age of 51.67 years, with moderate chronic periodontitis. Subjects were evaluated prior to treatment (T1) and 90 days after execution of therapy (T2), with regards to: probing depth (PD), clinical attachment level (CAL), plaque index (PI), gingival index (GI), and microbial detection for the presence of Porphyromonas gingivalis (P.g.) and Prevotella intermedia (P.i.) by culture method and confirmed by biochemical tests. Subjects treated in the FMS group also rinsed with 0.12% chlorhexidine mouthwash for seven days following treatment. The results were analyzed using statistical Student's t-test and chi-square test. No statistically significant differences were observed for PD and CAL between T1 and T2 in both groups. For GI and PI significant difference was observed between the groups. For the evaluated microbial parameters was observed reduction of P.g. and P.i., but only for P.g. with a significant reduction in both groups. The full mouth scaling technique with the methodology used in this study provided improved clinical conditions and reduction of P.g. in subjects with moderate periodontitis, optimizing the time spent in the therapeutic execution.

O ameloblastoma é um tumor odontogênico benigno, localmente invasivo, capaz de infiltrar-se pelos espaços medulares do osso e, quando em estágio inicial, geralmente não apresenta indícios radiográficos ou macroscópicos. Muitas vezes, pacientes submetidos a tratamento ressectivo perdem grande parte da maxila ou mandíbula, necessitando de uma alternativa de tratamento para restabelecer a função e a estética. O objetivo deste relato de caso foi apresentar um acompanhamento de quatro anos de uma reabilitação com implantes zigomáticos em paciente submetido à ressecção de maxila anterior devido a um tumor odontogênico do tipo ameloblastoma. Quatro meses após a realização da cirurgia de ressecção do ameloblastoma na região anterior de maxila, foram instalados dois implantes com ancoragem zigomática no lado esquerdo, um implante com ancoragem zigomática no lado direito e dois implantes regulares hexágono externo no lado direito. Em seguida, foi feita uma prótese provisória imediata. Seis meses depois, os implantes foram carregados com a prótese final em resina acrílica. Após quatro anos de acompanhamento, não foram observados sintomas dolorosos, inflamação ou infecção peri-implantar, instabilidade do implante ou reabsorção óssea. No presente caso, a reabilitação da maxila atrófica com implantes no osso zigomático proporcionou bons resultados funcionais e estéticos a longo prazo. Assim, o uso de implantes zigomáticos associados a implantes padrão oferece vantagens na reabilitação de maxilares severamente reabsorvidos, principalmente quando a indicação é feita corretamente e com o conhecimento da técnica cirúrgica. Ameloblastoma is a benign, locally invasive odontogenic tumor capable of infiltrating the bone marrow spaces, and when at the initial stage, it generally does not show radiographic or macroscopic evidence. Patients undergoing resection treatment lose a large part of the maxilla or mandible, requiring an alternative treatment to re-establish the function and aesthetics. The objective of this case report was to present a 4-years follow-up of a rehabilitation with zygomatic implants in a patient submitted to anterior maxillary resection due to an odontogenic tumor of the ameloblastoma type. After 4 months of the ameloblastoma resection surgery in the anterior maxillary region, 2 implants were placed with zygomatic anchoring on the left side, 1 implant with zygomatic anchoring on the right side and 2 regular external hexagon implants placed on the right side and then it was done an immediate provisional prosthesis. Six months later, the implants were loaded with the fi nal acrylic resin prosthesis. After 4-years of follow-up no painful symptoms, inflammation or peri-implant infection, implant instability or bone resorption were observed. In the present case, the atrophic maxilla rehabilitation with implants in the zygomatic bone provided long-term functional and aesthetic results. Thus, the zygomatic implants associated with standard implants offers advantages in severely reabsorbed maxillae rehabilitation, especially when the indication is made correctly and also with the knowledge of the surgical technique. Este relato de caso demonstra a utilização de implantes cerâmicos de zircônia na região do remanescente radicular cariado 21, extraído atraumaticamente. O implante cerâmico traz uma nova opção para as reabilitações em regiões em que a estética é essencial. Como método de preenchimento alveolar foi utilizado L-Plasma Rico em Fibrina (L-PRF), juntamente com um material aloplástico biocompatível e homogêneo, que tem sido largamente difundido na Odontologia. A instalação imediata da coroa temporária realizada logo após o procedimento de enxertia fez com que todo o alvéolo em sua porção cervical fosse ocupado, servindo de proteção e vedação para o enxerto de L-PRF e o biomaterial inserido na porção vestibular do alvéolo, bem como devolvendo a função estética e bem-estar do paciente. Um planejamento virtual adequado e a preservação das estruturas de sustentação no momento cirúrgico, bem como a utilização dos implantes de zircônia em dentes de região anterior em condições desfavoráveis, aumentam a previsibilidade, sendo uma alternativa de sucesso para a reabilitação com implantes.


This case report demonstrates the use of zirconia dental implants for a decayed, atraumatically extracted tooth # 21. The ceramic implant brings a new option for rehabilitation in regions where aesthetics is essential. In addition, the Leukocyte Platelet-Rich Fibrin (L-PRF) was added with a biocompatible alloplastic and homogeneous material, which has been widely disseminated in dentistry. The immediate installation of the temporary crown shortly after grafting procedure provided volume at the cervical portion of the alveolar socket, serving as a protection and barrier for L-PRF and the biomaterial inserted at the buccal aspect to retrieve aesthetics and well-being. Appropriate virtual planning and the preservation of supporting structures on surgery, as well as the use of the zirconia implants in anterior region under unfavorable conditions increase the predictability, being an alternative of success for implant rehabilitation.

Abstract In the search for the ideal treatment of periodontal disease various non-surgical techniques should be considered. The objective of this study was to evaluate the efficacy of full-mouth scaling (FMS) by clinical and microbiological parameters. 670 individuals were evaluated with 230 subjects meeting the selection criteria and were divided into two groups; 115 subjects treated with FMS and 115 treated with weekly sessions of scaling and root planning (SRP). The patient population had a mean age of 51.67 years, with moderate chronic periodontitis. Subjects were evaluated prior to treatment (T1) and 90 days after execution of therapy (T2), with regards to: probing depth (PD), clinical attachment level (CAL), plaque index (PI), gingival index (GI), and microbial detection for the presence of Porphyromonas gingivalis (P.g.) and Prevotella intermedia (P.i.) by culture method and confirmed by biochemical tests. Subjects treated in the FMS group also rinsed with 0.12% chlorhexidine mouthwash for seven days following treatment. The results were analyzed using statistical Student's t-test and chi-square test. No statistically significant differences were observed for PD and CAL between T1 and T2 in both groups. For GI and PI significant difference was observed between the groups. For the evaluated microbial parameters was observed reduction of P.g. and P.i., but only for P.g. with a significant reduction in both groups. The full mouth scaling technique with the methodology used in this study provided improved clinical conditions and reduction of P.g. in subjects with moderate periodontitis, optimizing the time spent in the therapeutic execution.


Resumo Na busca do tratamento ideal da doença periodontal varias são técnicas não-cirúrgicas que podem ser consideradas. O objetivo deste estudo foi avaliar a eficácia da técnica de desinfecção total de boca (FMD, na sigla em Inglês) por parâmetros clínicos e microbiológicos. Foram avaliados 670 indivíduos com 230 indivíduos atendendo aos critérios de seleção e divididos em dois grupos; 115 indivíduos tratados com FMD e 115 tratados com sessões semanais de raspagem e alisamento corono radicular (SRP, na sigla em Inglês). A população avaliada tinha idade média de 51,67 anos, com periodontite crônica moderada. Os sujeitos foram avaliados antes do tratamento (T1) e 90 dias após a execução da terapia (T2), quanto à profundidade de sondagem (PS), nível de inserção clínica (NIC), índice de placa (IP), índice gengival (IG) e detecção microbiana da presença de Porphyromonas gingivalis (P.g.) e Prevotella intermedia (P.i.) por método de cultura e confirmada por testes bioquímicos. Os indivíduos tratados no grupo FMD também realizaram bochechos com clorexidina 0,12% durante sete dias após o tratamento. Os resultados foram analisados ​​utilizando o teste estatístico t de Student e o teste de qui-quadrado. Não foram observadas diferenças estatisticamente significativas para PS e NIC entre T1 e T2 em ambos os grupos. Para IG e IP observou-se diferença significativa entre os grupos. Para os parâmetros microbianos avaliados foi observada redução de P.g. e P.i., mas apenas para P.g. com uma redução significativa em ambos os grupos. A técnica FMD com a metodologia utilizada neste estudo proporcionou condições clínicas melhoradas e redução da P.g. Em indivíduos com periodontite moderada, otimizando o tempo gasto na execução terapêutica.

Objetivo: comparar as técnicas de lateralização (LNAI) e transposição do nervo alveolar inferior (TNAI) com a taxa de disfunção neurossensorial, bem como a taxa de sucesso dos implantes dentários. Material e métodos: uma pesquisa foi realizada no período de 1987 a 2016, no banco de dados Medline/PubMed, com as palavras-chave "implantes dentários", "nervo alveolar inferior", "lateralização", "transposição", "neurossensorial" e "taxa de sucesso". Foram incluídos artigos retrospectivos e prospectivos em seres humanos, e que apresentassem relatos de disfunção neurossensorial e taxa de sucesso dos implantes dentários na região posterior da mandíbula, após procedimentos de lateralização ou transposição do nervo alveolar. Resultados: dos 142 artigos encontrados na primeira fase de seleção, apenas 26 foram incluídos na técnica LNAI, com um total de 524 procedimentos, sendo 228 na técnica de TNAI e 296 na LNAI, que foram incluídos neste estudo. O número de implantes instalados pela técnica de TNAI foi de 625, e pela técnica de LNAI foi de 401. As técnicas de TNAI apresentaram um índice inicial maior de disfunção neurossensorial, quando comparadas à técnica de LNAI. A taxa de sucesso dos implantes para a té cnica de TNAI ficou entre 80% e 100%, e para a té cnica de LNAI fi cou entre 90,5% e 100%. Conclusão: a LNAI vem sendo uma boa indicação para a reabilitação de mandíbulas atróficas verticalmente, e sua execução com o piezo diminui o risco de distúrbio sensorial. As altas taxas de sucesso dos implantes podem ser atribuídas à possibilidade da instalação de implantes longos com ancoragem bicortical, favorecendo a estabilidade primária e a biomecânica.


Objective: to compare lateralization techniques (LNAI) and transposition of the inferior alveolar nerve (TNAI) with the rate of sensorineural dysfunction, as well as the success rate of dental implants. Material and methods: a survey was carried out between 1987 and 2016 in the Medline/PubMed database, with the key words "dental implants", "inferior alveolar nerve", "lateralization", "transposition", "neurosensorial", "success rate". Retrospective and prospective articles in humans that presented reports of sensorineural dysfunction and success rate of dental implants in the posterior region of the mandible after procedures of lateralization or transposition of the alveolar nerve were included. Results: in the fi rst phase of the selection, 142 articles were retrieved, but only 26 articles on LNAI, with a total of 524 procedures, 228 on TNAI technique and 296 the LNAI, were included in the study. The number of implants installed by the TNAI technique was 625 and by the LNAI 401. The TNAI present a higher initial index of sensorineural dysfunction than the LNAI and the success rate of the implants was for the TNAI between 80% to 100%, and for the LNAI between 90.5% and 100%. Conclusion: LNAI has been a good indication for the rehabilitation of atrophic mandibles vertically and its execution with piezo reduces the risk of sensory disturbance. The high success rates of implants can be attributed to the possibility of installing long implants with bicortical anchoring favoring primary stability and biomechanics.

Prader-Willi syndrome described in 1956 has a genetic origin, affecting both genders, varying in presence and intensity from individual to individual. A precocious diagnosis, before the manifestation of symptoms, has brought some improvement in the quality of life of the carriers in the last years. The objective of this case report was to describe the treatment realized in a 3-year-old boy who presented grade II obesity, difficulty of locomotion, hypotonia, and history of cardiopathy. A dental treatment under general anesthesia was defined, allowing an oral adequation in a single section, in which it was planned the extraction of the element 74 and atraumatic restorative treatment (ART) technique in the other teeth. The precocious intervention in this 3-year-old patient by the therapy realized with ART under general anesthesia was done with success, avoiding unnecessary extractions, preserving dental elements, and maintaining the oral cavity in adequate function.

Bone resorption of the posterior mandible can result in diminished bone edge and, therefore, the installation of implants in these regions becomes a challenge, especially in the presence of the mandibular canal and its contents, the inferior alveolar nerve. Several treatment alternatives are suggested: the use of short implants, guided bone regeneration, appositional bone grafting, distraction osteogenesis, inclined implants tangential to the mandibular canal, and the lateralization of the inferior alveolar nerve. The aim was to elucidate the success rate of implants in the lateralization technique and in inferior alveolar nerve transposition and to determine the most effective sensory test. We conclude that the success rate is linked to the possibility of installing implants with long bicortical anchor which favors primary stability and biomechanics.

A realização de enxertia óssea convencional visando corrigir defeitos ósseos, para posterior colocação de implantes dentários é uma opção viável e previsível, mas que aumenta o tempo e a morbidade dos tratamentos implantodônticos. Sendo assim, a realização da implantação mesmo em áreas com deficiente volume ósseo, associada à colocação de biomateriais e membranas reabsovíveis é uma opção para reduzir a duração e a morbidade do tratamento. De acordo com a literatura, esse método quando bem indicado e executado, proporciona resultados semelhantes à enxertia convencional. Diante do exposto, o objetivo do presente trabalho foi relatar e discutir o caso clínico de um paciente que apresentava perda óssea em região anterior de mandíbula e que foi tratado através de implantação, colocação de biomaterial e membrana reabsorvível sobre a superfície exposta do implante, no mesmo momento cirúrgico. Os resultados pós-operatórios demonstram estabilidade, tecidos moles mantidos em posição e volume gengival adequado, o que vem a evidenciar o sucesso do plano de tratamento executado.


The performance of conventional bone grafting aiming to correct bone defects, for later placement of dental implants is a viable and predictable option, but it increases implantodontic treatments time and morbidity. Thus, performing the implantation even in areas with poor bone volume associated with the placement of biomaterials and resorbable membranes is an option for reducing treatment duration and morbidity. According to the literature this method when well indicated and performed provides similar results to conventional grafting. Considering all this, the objective of this study was to report and discuss the case of a patient with bone loss in the anterior jaw, who was treated through implantation, placement of biomaterial and resorbable membrane on the exposed surface of the implant at the same surgical time. The postoperative results have demonstrated stability, soft tissue held in place and adequate gingival volume, which shows the success of the treatment plan implemented.

Vários fatores biológicos têm sido estudados como biomarcadores da condição periodontal. O óxido nítrico (NO) faz parte de uma família de radicais livres envolvidos com a homeostasia, resposta imunológica, atividade cardiovascular e neurotransmissão. O NO pode ser produzido pelo organismo humano via óxido nítrico sintase (NOS) dependente ou via NOS independente. No mecanismo NOS independente a produção de NO envolve um ciclo entero-salivar de redução do nitrato (NO3-) em nitrito (NO2-) e a sua subsequente conversão em NO. Os estímulos inflamatórios presentes na doença periodontal também são capazes de induzir a formação de NO e há relatos na literatura de que o mesmo possa atuar interferindo na progressão da periodontite. No osso, assim como em outros tecidos, a produção de NO pode ser estimulada por lipopolisacarídeos (LPS) bacterianos. Nesse mecanismo tem sido sugerida uma importante participação das bactérias bucais. O objetivo deste trabalho foi avaliar por meio de revisão da literatura a relação do óxido nítrico, nitrato e nitrito com a condição periodontal e se procedimentos terapêuticos periodontais podem interferir com esses fatores. O óxido nítrico é um importante mediador de várias atividades biológicas. Os níveis de NO2- parecem estar aumentados na periodontite, comparativamente à gengivite e saúde gengival. Apesar da produção de NO ter um papel primário bactericida, é provável que essa produção em altas concentrações resulte em danos aos tecidos periodontais do hospedeiro. Nas lesões periodontais quantidades consideráveis de NO são geradas principalmente por macrófagos, neutrófilos polimorfonucleares, linfócitos e fibroblastos induzidos pelas citocinas e por LPS. A oferta dietética de NO3- influencia a síntese de NO e a produção de NO2-. Não está completamente elucidada a real influência do tratamento periodontal sobre os níveis de NO2.(AU)


Several biological factors have been studied as biomarkers of periodontal condition. Nitric oxide (NO) is part of a family of free radicals related to homeostasis, immunological response, cardiovascular activity, and neurotransmission. NO can be produced by human body via nitric oxide synthase (NOS) dependent or an pathway independent of NOS. In the independent pathway NO production involves an enterosalivary cycle by which nitrate (NO3-) is reduced to nitrite (NO2-) followed by its subsequent conversion to NO. The inflammatory stimuli present in periodontal disease are also able to induce the formation of NO and there are reports in the literature that the same may act by interfering on the progression of periodontitis. In bone, like other tissues, NOproduction may be stimulated by lipopolysaccharide (LPS) from bacteria. In this mechanism it has been suggested an important role of oral bacteria. The aim of this study was to evaluate through a literature review the relationship between NO, NO3- and NO2- and periodontal condition. Also whether periodontal therapeutic procedures can interfere in NO, NO3- and NO2- or not. NO is an important mediator in various biological activities. NO2- levels seem to be high in periodontitis when compared to gingivitis and gingival health. Although NOproduction has a bactericidal primary role, probably in high concentrations its production results in damage to host periodontal tissues. In periodontal lesions greater amounts of NO are generated primarily by macrophages, polymorphonuclear neutrophils, lymphocytes and fibroblasts and induced by cytokines and LPS. NO3-from diet influences the synthesis and production of NO and NO2-. It is not completely understood the real influence of periodontal treatment on the levels of NO2- (AU)
