RESUMO

The small airway, present since the origins of humanity and described barely a century ago, has recently been discovered as the anatomical site where inflammation begins in some obstructive lung diseases, such as asthma and Chronic Obstructive Pulmonary Disease (COPD), per se. Small airway dysfuction was identified in up to 91% of asthmatic patients and in a large proportion of COPD patients. In subjects without pathology, small airway represent 98.8% (approximately 4500 ml) of the total lung volume, contributing only between 10-25% of the total lung resistance; however, in subjects with obstruction, it can represent up to 90% of the total resistance. Despite this, its morphological and functional characteristics allow its dysfunction to remain undetected by conventional diagnostic methods, such as spirometry. Hence the importance of this review, which offers an overview of the tools available to assess small airway dysfunction and the possible therapies that act in this silent zone.


La vía aérea pequeña, presente desde los orígenes de la humanidad y descrita hace apenas un siglo, se ha descubierto recientemente como el sitio anatómico donde inicia la inflamación provocada por algunas enfermedades pulmonares obstructivas: asma y enfermedad pulmonar obstructiva crónica (EPOC), per se. Se ha identificado disfunción de la vía aérea pequeña en el 91% de los pacientes asmáticos y en una gran proporción de quienes padecen EPOC. En los pacientes sin enfermedad, la vía aérea pequeña representa el 98.8% (4500 mL) del volumen pulmonar total, y solo aporta del 10 al 25% de la resistencia pulmonar total; sin embargo, en sujetos con obstrucción puede suponer el 90% de la resistencia total. A pesar de esto, sus características morfológicas y funcionales permiten que la disfunción pase inadvertida por métodos diagnósticos convencionales, por ejemplo la espirometría. Con base en lo anterior, el objetivo de este estudio fue revisar el panorama general de los métodos disponibles para evaluar la vía aérea pequeña y los posibles tratamientos asociados con esta zona silente.

BACKGROUND: Recruitment after disturbance events depends on many factors including the environmental conditions of the affected area and the vegetation that could potentially grow in such affected areas. To understand the regeneration characteristics that occurs in temperate forests, we evaluated differences in the number of seedlings from trees and shrubs along an altitudinal gradient in Sierra Norte of Oaxaca, Mexico in different biological, climatic, edaphic, light, topographic, and disturbance regimes. Here, we aimed to test the hypothesis that the environmental disturbances influence on recruitment (positive or adverse influence). We sampled the vegetation to obtain recruitment and adult data, and species composition. RESULTS: We identified three disturbance regimes: areas affected by forest harvesting, areas exposed to pest management, and undisturbed areas. We identified 29 species of trees and shrubs (9 species of the genus Pinus, 1 species of the genus Abies, 10 species of the genus Quercus, and 9 of other species of broadleaf). We found that both environmental conditions and disturbances influence the recruitment of vegetation in the study area. In particular, disturbances had a positive influence on the regeneration of oak and other broadleaf species by increasing the number of seedlings, and a negative influence on the regeneration of conifers by decreasing the recruitment. Because the recruitment of conifers is more likely in undisturbed areas (sites over 3050 m). CONCLUSIONS: Environmental factors and anthropogenic disturbances can alter the recruitment of forests. Consequently, knowing which factors are key for the recruitment of vegetation is fundamental for decision-making processes. This is particularly relevant in areas as the one in this study because it provides knowledge to local people on vegetation recovery for a proper management of their biological resources.

The study of changes in species richness and composition along rivers has focused on large spatial scales. It has been ignored that in different sections of the river (high mountain area, middle zone, and mouth of the river) the specific environmental conditions can generate different longitudinal patterns of the species richness and composition. In this study, we determine whether species richness and composition of the riparian plant communities change along a mountain river and whether these changes are related to environmental variables. We expect an increase in species richness and turnover along the river, that the upstream communities would be a subset of the downstream communities, and that such would be related to edaphic and hydrologic conditions. To test this, we sampled three strata of the riparian vegetation (upper: individuals with <1 cm of ND, middle: individuals with >1 cm of ND, low: individuals with >1 m tall) in a set of 15 sites that we place along a mountain river. Additionally, we recorded topographic, hydrological, morphological, and soil variables. We performed correlation analyzes to determine whether changes in species richness and turnover were related to increased distance to the origin of the river. Also, we obtained the nestedness and evaluated the importance of environmental variables with GLM, LASSO regression, and CCA. With the increase in distance, the species richness decreases in the upper stratum, but not in the middle and the low stratum (although the highest values were observed near the origin of the river), the turnover increase in all strata and the upstream communities were not a subset of the downstream communities. The changes in species richness and composition were related to topographic (altitude), hydrological (flow), and edaphic (conductivity and pH) variables. Our results indicate that at small spatial scales the patterns of richness and composition differ from what has been found at larger spatial scales and that these patterns are associated with environmental changes in the strong altitude gradients of mountain rivers.

BACKGROUND: The main limitation of the six-minute walk test (6-MWT) is that not all pulmonary function testing facilities have an indoor flat, 30-m-long corridor. Therefore, this study aimed 1) to evaluate the correlation and agreement of the distances walked in 30-m- vs. 15-m-long corridors by subjects with chronic lung diseases (CLD group) and 2) to compare the levels of oxygen saturation (nSpO2), blood pressure (BP), heart rate recovery at minute one post-exercise (HRR1), and Borg scale scores for dyspnea and fatigue between the two distances walked. METHODS: A prospective, cross-sectional study was conducted at the National Institute of Respiratory Diseases in Mexico City. Subjects with chronic lung diseases and healthy adults were invited to participate. The distance of the 6-MWT was randomly assigned based on whether the first test was in the 15-m or 30-m corridor. RESULTS: Ninety individuals were included; the correlation in meters walked between the two corridors was r = 0.96 in CLD; the 95% limits of agreement for the 6-MWT ranged from -73 to +37 m. Most subjects walked further in the 30-m corridor (82%); however, the percent predicted values for the CLD group were 3.5% lower for the 15-m corridor than the 30-m corridor. Only 10.5% of the subjects with CLD would have been falsely classified as having a normal 6-MWT (false negative). No significant differences in the nSpO2, Borg scale, BP or HRR1 were found between the two 6-MWT corridor lengths. CONCLUSION: The 6-MWT can be performed using a 15-m corridor in subjects with CLD, and the results for the distance walked, HRR1, nSpO2, and Borg scale scores are similar to between the 15-m and 30-m corridors.

Globally, climate is a fundamental driver of plant species' geographical distributions, yet we still lack a good understanding of climatic variation on tropical mountains and its consequences for elevational floristic patterns. In a seasonally dry region of southern Mexico, we analysed meteorological patterns along a large elevational gradient (0-3670 m a.s.l.) and examined their relationship with changes in floristic richness. Meteorological patterns were characterised using two data sources. First, climatic information was extracted from cartography and records from a few existing meteorological stations. Additionally, air temperature and humidity were recorded hourly during 1 year with data loggers, at sites representing 200-m elevation increments. Floristic information was extracted from a database containing 10,124 records of plant collections, and organized in 200-m elevational belts. Climatic charts distinguished three climate types along the gradient, all with marked rainfall seasonality, but these bore little correspondence with the information obtained with the data loggers. Mean annual air temperature decreased with increasing elevation (lapse rate of 0.542 °C 100 m(-1)). Thermal oscillation was minimum around 1400 m and increased towards both extremes of the gradient. Relative humidity opposed this pattern, with maxima between 800 and 1800 m, decreasing towards the highest elevations. An analysis of temperature frequency distributions revealed meteorological features undetectable from the annual or monthly means of this variable; despite an overall gradual transition of the proportions of time recorded at different temperatures, some changes did not conform to this pattern. The first discontinuity occurred between 1000-1200 m, where dominant temperatures shifted abruptly; also noticeable was an abrupt increase of the proportion of time elapsed at 0.1-10 °C between 2400 and 2600 m. Air temperature appears to be the most influential climatic factor driving elevational variation of plant species richness in this region.
