RESUMO

Although sagittal fractures of the maxilla are not common, they can be observed in some cases following severe trauma to the facial skeleton. 18 of the 23 cases reported had combined mandibular and midfacial fractures. Sometimes in severe cases they may demonstrate instability with conventional methods of treatment; thus for adequate stabilization they may need a palatal splint, direct wiring (internal fixation in the buttresses), intermaxillary fixation and cranial suspension.

RESUMO

The majority of extraoral facial fistulas are of odontogenic origin. Although the differential diagnosis of draining lesions should include various types of skin infection, infected tumour, specific infections, failed wound healing, foreign body, salivary gland fistula, sebaceous cysts and developmental cysts and fistulas. This report documents a group of patients with extraoral draining skin lesions and illustrates that their diagnosis is very important because clinically they can resemble many pathologic entities. One hundred and twenty-eight cases of extraoral fistulas are presented in this paper. This study aims to analyse a number of fistula cases so as to give a clear idea about relative frequency of etiology and to attract to some cases of non odontogenic etiology.

RESUMO

A rare case of a schwannoma localized on the posterior pharyngeal wall is presented. It concerns a young man with an inflammatory ulcerated mass in the posterior wall of the pharynx causing severe difficulty in swallowing. Although schwannomas of the lateral pharyngeal wall are common, only one case located on the posterior wall has been described. The origin of these tumours at this particular site is thought to be the sympathetic nervous plexus of the posterior pharyngeal wall. In presenting our case, we comment on the origin of posterior pharyngeal wall schwannomas.

RESUMO

A case of osteoblastoma of the mandibular ramus is presented with an over 2 years history and with a single clinical sign,--progressive enlargement of the cheek and parotid region. Investigation into the limited literature regarding these rare cases confirmed the specific findings in our case, i.e. that destruction of the bone cortex by the expansion of the tumour, along with osteogenesis, is not unusual.

RESUMO

UNLABELLED: Two cases of carcinoma of the frontal sinus (undifferentiated carcinoma of squamous epithelium and transitional cell carcinoma) are described. Clinically, there are difficulties in the differential diagnosis from infections processes, while, roentgenologically, the image is confused with those of mucocele, pyocele and osteomyelitis. The clinical symptoms, as exophthalmos, diplopia, pain and swelling of the supraorbital region, indicate an extension of the tumor into the orbit and the destruction of the anterior wall of the frontal sinus. MANAGEMENT: Radical removal and radio-therapy. PROGNOSIS: Very bad.

RESUMO

A case of congenital ear dysplasia, combined with cleft palate and anomalies of the cervical spine and eyes, is described. The case belongs to the group of cervico-ocular-auditory dysplasias (Klippel-Feil syndrome). A strange feature in our case was the co-existence of conductive hypakousia in the right ear, due to congenital fixation of the stapes footplate, with perceptive deafness in the left ear, due to dysplasia of the cochlea and internal auditory meatus. The conductive hypakousia in the right ear was successfully treated by stapedectomy. A review of the literature on the Klippel-Feil syndrome has shown that, in some of them the conductive deafness was unexplained, while in others the deafness was due to dysplasia of the ossicular chain or to a combination of dysplasias of the labyrinth and middle ear in the same ear. The co-existence of middle ear dysplasia in one ear with contralateral dysplasia of the inner ear and internal auditory meatus is rare.
