OBJECTIVE: Orbital varix is a rare distensible orbital venous malformation. Most patients present with unilateral intermittent periorbital pain and positional proptosis that is exacerbated by the Valsalva maneuver. Complications include hemorrhage and thrombosis, leading to sudden painful proptosis and visual disturbance. OBSERVATIONS: A 42-year-old female with a history of bilateral intermittent painless proptosis that was accentuated by a postural head-down position presented with acute painful proptosis in her right eye. Ophthalmic examination revealed right eye proptosis with a bluish mass at the right upper eyelid, and another bluish mass at the left lower eyelid that was prominent during the Valsalva maneuver. Computed tomography scans of the orbits indicated right orbital varix thrombosis and left orbital varix. Surgical excision of the right thrombosed varix was performed along with intralesional bleomycin injection in the left orbital varix. Histopathological examination confirmed the right thrombosed varix diagnosis. The patient had good clinical improvement at the 6-month follow-up. LESSONS: Orbital varices have a spectrum of clinical manifestations, from asymptomatic to severe visual loss. Most cases are successfully treated conservatively; however, in complicated cases, interventions with a multidisciplinary team approach such as sclerotherapy, embolization, and surgical excision should be considered.

PURPOSE: This study aimed to evaluate the ability of susceptibility-weighted imaging (SWI) to detect cortical venous reflux (CVR) in patients with intracranial non-cavernous dural arteriovenous fistulas (DAVFs), which can be helpful to differentiate benign and aggressive DAVFs. MATERIAL AND METHODS: Twenty-seven patients (8 women and 19 men) with 33 non-cavernous DAVFs were divided into benign and aggressive groups. Presence of CVR and pseudophlebitic pattern (PPP) and location of fistula on SWI were determined. Digital subtraction angiography was used as the reference standard. Interobserver agreement for the presence of CVR and PPP and location of DAVF on SWI was evaluated using the kappa statistic. Statistical comparisons between the benign and aggressive DAVFs were performed. RESULTS: Sensitivity, specificity, positive predictive value, and negative predictive value of SWI for detecting CVR was 73.7%, 85.7%, 87.5%, and 70.6%, respectively. Corresponding values for detecting PPP were 95.2%, 83.3%, 95.2%, and 83.3%, respectively. SWI correctly identified DAVF location in 78.9%. Prevalence rates of CVR and PPP on SWI were significantly higher in aggressive DAVFs than benign ones. CONCLUSION: SWI exhibited high sensitivity and specificity for detection of CVR, a characteristic used to differentiate benign and aggressive lesions. CVR and PPP on SWI are signs of aggressive DAVFs that guide to perform angiography confirmation and prompt treatment to avoid serious complication.

PURPOSE: The aim of the study was to assess the usefulness of apparent diffusion coefficient (ADC) values for differentiating between SCC and malignant salivary gland tumors of the head and neck. MATERIALS AND METHODS: This retrospective cross-sectional study was conducted on 29 patients with SCCs and 10 patients with malignant salivary gland tumors who underwent pretreatment MRI of the head and neck. The minimum and average ADC values of the tumors were measured, with normalized tumor to spinal cord ADC ratios generated. Comparisons of ADC values and normalized ADC ratios between the two tumor types were performed using the unpaired t-test. RESULTS: The minimum and average ADC values and normalized average ADC ratios of SCCs (753.17 ± 214.47 × 10-6 mm2/s, 848.79 ± 250.13 × 10-6 mm2/s, and 0.92 ± 0.25, respectively) were significantly lower than those of malignant salivary gland tumors (1084.90 ± 242.60 × 10-6 mm2/s, 1305.90 ± 270.99 × 10-6 mm2/s, and 1.58 ± 0.31, respectively; all p < 0.001). A cutoff value of 1.31 for the normalized average ADC ratio was used to distinguish between SCCs and malignant salivary gland tumors and achieved an area under the curve of 0.93, sensitivity of 96.6%, specificity of 90%, and accuracy of 94.6%. CONCLUSION: ADC value measurement could help differentiate between SCCs and malignant salivary gland tumors.

PURPOSE: Differentiating between atherosclerosis, vasculitis, and dissection is a diagnostic challenge because of inconclusive findings on conventional imaging and some overlap in the vessel wall patterns. The aim of this study was to determine whether vessel wall MRI patterns can differentiate between these vasculopathies. METHODS: We retrospectively reviewed 3T high-resolution vessel wall imaging studies of patients diagnosed with atherosclerotic plaques, vasculitis, and dissection. The patterns of involvement, wall enhancement, and T1 and T2 signals, as well as the specific patterns, were assessed and compared between the three diseases. RESULTS: Fifty-nine patients with atherosclerosis (n = 33), vasculitis (n = 13), and dissection (n = 13) were enrolled. There were significant differences in the pattern of involvement between the three groups (P < 0.001), with concentric wall thickening in vasculitis patients (84.6%) and eccentric wall thickening in atherosclerosis (97%) and dissection (92.3%) patients. There was also a significant difference in the specific pattern (P < 0.001), with intimal flap (76.9%) and intramural hematoma (23.1%) in dissection patients and intraplaque hemorrhage (18.2%) in atherosclerosis patients. Furthermore, subgroup analysis showed a significant difference in the wall enhancement pattern between atherosclerosis and vasculitis patients (P < 0.05). Finally, there was a significant difference in the location of involvement between the three groups (P < 0.001). CONCLUSION: By using the pattern of involvement, wall enhancement, and specific patterns, vessel wall MRI can help differentiate between atherosclerosis, vasculitis, and dissection.

PURPOSE: A multimodality approach is generally considered for pediatric low-grade gliomas (LGG); however, the optimal management remains uncertain. The objective of the study was to evaluate treatment outcomes of pediatric LGG, focusing on long-term survival and factors related to outcomes. METHODS: A retrospective review of 77 pediatric LGG cases treated at Ramathibodi Hospital, Thailand between 2000 and 2018 was performed. The inclusion criteria were all pediatric LGG cases aged ≤ 15 years. Diffuse intrinsic pontine gliomas and spinal cord tumors were excluded. RESULTS: The median follow-up time was 8.2 years (range, 0.6-19.7). The median age at diagnosis was 6.2 years (interquartile range, 3.6-11.4). Treatments modality included tumor surgery (93%), chemotherapy (40%), and radiation therapy (14%). The 10-year overall survival (OS) and 10-year progression-free survival were 94% and 59%, respectively, for the entire cohort. The 10-year OS was 100% in three subgroups of patients: pilocytic subtype, WHO grade 1 tumors, and recipient of gross total resection. After multivariable analysis, no tumor surgery had a significantly unfavorable influence on overall survival. CONCLUSIONS: With a multimodality approach, pediatric LGGs had excellent outcome. Gross total resection is the standard primary treatment. Chemotherapy is the alternative standard treatment in incomplete resection cases, unresectable patients, or patients with progressive disease. Radiation therapy should be reserved as a salvage treatment option because of late complications that usually affect patients' quality of life.

OBJECTIVE: This study aimed to assess the value of dual-energy computed tomography for differentiation of inverted papilloma from squamous cell carcinoma (SCC)/lymphoma. METHODS: Twenty-eight patients with pathologically diagnosed inverted papilloma or SCC/lymphoma underwent contrast-enhanced dual-energy computed tomography. Qualitative features (laterality, location, enhancement pattern, border, necrosis, hemorrhage, calcification, bone destruction, pterygopalatine fossa extension, adjacent invasion, and perineural spreading) and quantitative features (iodine density and spectral attenuation curve slope) were analyzed. Optimal cutoff thresholds of diagnostic efficacy were generated. RESULTS: Fifteen patients had inverted papilloma, and 13 had malignancy (5 SCC and 8 lymphoma). Computed tomography findings of bilateral lesions, sphenoid sinus involvement, pterygopalatine fossa extension, and adjacent invasion were significantly associated with SCC/lymphoma. The iodine density was significantly higher in SCC/lymphoma (2.46 ± 0.22 mg/mL) than in inverted papilloma (1.42 ± 0.46 mg/mL; P = 0.001). An iodine density threshold of 1.74 mg/mL had a sensitivity, specificity, negative predictive value, positive predictive value, and accuracy of 92.3%, 86.7%, 92.9%, 85.7%, and 90.3%, respectively. The spectral attenuation curve slope was significantly higher in SCC/lymphoma (4.35 ± 0.27 HU/keV) than in inverted papilloma (2.72 ± 0.88 HU/keV; P = 0.001). A spectral attenuation curve slope threshold of 3.34 HU/keV had a sensitivity, specificity, negative predictive value, positive predictive value, and accuracy of 92.3%, 86.7%, 92.9%, 85.7%, and 90.8%, respectively. CONCLUSIONS: Squamous cell carcinoma/lymphoma had a significantly higher iodine density and spectral attenuation curve slope than inverted papilloma. Using optimal quantitative measurement thresholds provides high diagnostic efficacy.

PURPOSE: To analyze radiological characteristics of the extraocular muscles (EOMs) in myasthenia gravis (MG) patients with ocular manifestations. PATIENTS AND METHODS: This retrospective case-control study included all MG cases with ocular manifestations, who attended a neuro-ophthalmology clinic at a university hospital, Bangkok, from April 2009 to June 2018. They experienced double vision and ophthalmoplegia. Control participants had normal eye movements. Orbital scans were thoroughly reviewed. We measured muscle thickness (MT) of the superior rectus, inferior rectus, medial rectus and lateral rectus muscles in both eyes using magnetic resonance imaging or computed tomography scan. The sum of the muscle thickness at all sites was calculated (MTsum). Comparisons of the mean MT of each muscle type and the mean MTsum between the MG and control groups were performed by using Student's t-test. MRI signal intensities of the EOMs were also recorded. RESULTS: Twenty MG cases and 20 controls were included in the study. The mean MTsum was 23.7 (standard deviation 4.8) mm in the MG group and 32.6 (3.5) mm in the controls. There were statistically significant differences between the two groups with respect to the mean MT and mean MTsum (p <0.001). In the MG group, there was a negative correlation between the MTsum and disease duration (p= 0.03). By using coronal T2-weighted orbital MRI with fat suppression (T2W/FS), the most frequent finding was isointensity with central hypointensity of the EOMs in the MG group. CONCLUSION: Atrophic EOMs were frequently found in the MG group, particularly in chronic cases. Isointensity with central hypointensity of EOMs on T2W/FS was also common in the MG group. These findings highlight the importance of muscle involvement in MG and may be helpful for clinical decision-making.

OBJECTIVE: The aim of this study was to differentiate suprasellar germinomas from chiasmatic/hypothalamic gliomas (CHGs) using apparent diffusion coefficient (ADC) values and magnetic resonance imaging (MRI) characteristics. MATERIALS AND METHODS: A cross-sectional study was conducted on 11 patients with suprasellar germinomas and 11 patients with CHGs who underwent pretreatment MRI. The ADC values (minimum and average ADC values) of the tumors were measured and the MRI characteristics were evaluated. RESULTS: The average and minimum ADC values of suprasellar germinomas were significantly lower than those of CHGs (p = 0.016 and 0.004 respectively). The selection of 941.15 × 10-6 mm2/s as a cutoff value of the minimum ADC value was used to differentiate suprasellar germinomas and CHGs; the best results were obtained with area under the curve of 0.889, sensitivity of 87.5%, specificity of 77.8% and accuracy of 82.4%. MRI characteristics suggested the diagnosis of suprasellar germinomas were T2W hypointensity and involvement of pituitary gland and/or stalk. MRI characteristics suggested the diagnosis of CHGs was higher degree of contrast enhancement and presence of macrocysts. CONCLUSION: ADC values have a role in differentiating suprasellar germinomas and CHGs, especially when imaging findings on conventional MRI are inconclusive. Furthermore, some MRI features are in favor of differentiation between these tumor entities including tumor location, cyst pattern, T2W hypointensity, degree of contrast enhancement, stalk and pituitary gland involvement.

PURPOSE: The distribution of ischemic changes caused by infarction of the middle cerebral artery (MCA) territories is usually measured using the Alberta Stroke Program Early Computed Tomography Score (ASPECTS). The first interpreter of the brain computed tomography (CT) in the emergency department is the on-call radiology resident. The primary objective of this study was to describe the agreement of the ASPECTS performed retrospectively by the resident compared with expert raters. The second objective was to ascertain the appropriate window setting for early detection of acute ischemic stroke and good interobserver agreement between the interpreters. METHODS: We identified consecutive patients presenting with hemiparesis or aphasia at the emergency department who underwent brain CT and CT angiography. Each scan was rated using ASPECTS by senior radiology resident, neuroradiology fellow, and later by consensus between two expert raters. Statistical analysis included determination of Cohen's kappa (κ) coefficient and intraclass correlation coefficient (ICC). RESULTS: A total of 43 patients met our study criteria. Interobserver agreements for ASPECTS varied from 0.486 to 0.678 in Cohen's κ coefficient between consensus of two neuroradiologists and a neuroradiology fellow, and from 0.198 to 0.491 for consensus between two neuroradiologists and a senior radiology resident. ICC among three raters (expert consensus, neuroradiology fellow, and senior radiology resident), was very good when 8 HU window width and 32 HU center level setting was used. CONCLUSION: ASPECTS varied among raters. However, when using a narrowed window setting for interpretation, interobserver agreement improved.

Systemic lupus erythematosus (SLE) is an autoimmune disease with multi-organ involvement and results in neurological and psychiatric (NP) symptoms in up to 40% of the patients. To date, the diagnosis of neuropsychiatric systemic lupus erythematosus (NPSLE) poses a challenge due to the lack of neuroradiological gold standards. In this study, we aimed to better localize and characterize normal appearing white matter (NAWM) changes in NPSLE by combining data from two quantitative MRI techniques, diffusion tensor imaging (DTI) and magnetization transfer imaging (MTI). 9 active NPSLE patients (37 ± 13 years, all females), 9 SLE patients without NP symptoms (44 ± 11 years, all females), and 14 healthy controls (HC) (40 ± 9 years, all females) were included in the study. MTI, DTI and fluid attenuated inversion recovery (FLAIR) images were collected from all subjects on a 3 T MRI scanner. Magnetization transfer ratio (MTR), mean diffusivity (MD), fractional anisotropy (FA), radial diffusivity (RD), axial diffusivity (AD) maps and white matter lesion maps based on the FLAIR images were created for each subject. MTR and DTI data were then co-analyzed using tract-based spatial statistics and a cumulative lesion map to exclude lesions. Significantly lower MTR and FA and significantly higher AD, RD and MD were found in NPSLE compared to HC in NAWM regions. The differences in DTI measures and in MTR, however, were only moderately co-localized. Additionally, significant differences in DTI measures, but not in MTR, were found between NPSLE and SLE patients, suggesting that the underlying microstructural changes detected by MD are linked to the onset of NPSLE. The co-analysis of the anatomical distribution of MTI and DTI measures can potentially improve the diagnosis of NPSLE and contribute to the understanding of the underlying microstructural damage.

PURPOSE: To analyze significant ultrasonographic findings of small malignant breast mass (≤10 mm) which were occult on mammography. METHODS: The study included 190 small breast masses (≤10 mm), demonstrated on breast ultrasonography, but not mammography. Histopathology (when the masses were biopsied) or serial breast ultrasonography (for at least 24 months) were used to confirm benign or malignant condition of the masses. Univariate and multivariate logistic regression analysis were used to identify significant characteristic malignant findings on ultrasonography. RESULTS: Of 190 masses, 46 were cancer, and 144 were benign. On multivariate analyses, irregular shape (odds ratio [OR], 10.4) and not circumscribed margin (OR, 31.6) were significant features to differentiate between benign and malignant breast masses. However, low width/anteroposterior ratio, echogenic halo, hypoechogenecity and posterior acoustic shadow, which were predictors for malignancy in large breast mass, were not documented in small mass. CONCLUSION: In conclusion, irregular shape and not circumscribed margin detected during ultrasonography were strong predictive signs of malignancy for small malignant breast mass.
