RESUMO

By using up-to-date mathematical methods, the authors studied exercise tolerance in 324 hypertensive patients. New approaches are validated to estimation of motor activity and its intensity in hypertensive subjects. The mathematical model provides reliable orientation in physical performance in the above patients as well as in their reference to the functional classes. The model is thought appropriate for wide introduction into outpatient clinical practice.

RESUMO

Current mathematic analysis was used to elaborate a mathematic model of physical work fitness which was employed for rehabilitation of 151 patients with essential hypertension. Using the model suggested one can obtain data on physical work fitness of patients and carry out effective hypotensive treatment under conditions of a district polyclinic.

RESUMO

The authors relate the results of multidimensional multivariate algorithmic diagnosis of clinical examination of 200 persons made over time with the aid of the methods of mathematic statistics. Factorial analysis performed in accordance with the data obtained as a result of examining normal persons, those with the prenosological neurotic manifestations, neurotic reactions, and patients with neurotic conditions enabled one to distinguish 10 factors covering the most informative symptomatology reflecting formation of the neurotic condition. Proceeding from the factorial mathematic model obtained the analysis allowed demonstration of the relationship between the symptoms, syndromes and the clinical structure at each stage of their formation. Based on the data obtained prerequisites are being made for the design of the methods of computer-aided algorithmic diagnosis of the early stages of forming the neurotic conditions.

RESUMO

A detailed 402 patient survey explored the psychological concomitants of arterial hypertension. Non-linear correlation analysis was used to uncover the strong association of psychic and somatic aspects in patients with arterial hypertension and to suggest new methods for their ambulatory rehabilitation. Aspects of patient compliance were considered, taking into account the patients' psychic status. Particulars of doctor--patient relationship were discussed and strong emphasis was placed on close psychological affinities between the two as the prerequisite for fostering patient compliance. A multi-dimensional study, is was concerned with assessing the efficiency of both drug therapy (hypotensive and psychotropic drugs and an association of the two) and other types of therapy (autogenous training, therapeutic relaxation exercise) for the rehabilitation of patients with arterial hypertension by means of polyclinic-based ambulatory care.

RESUMO

Interpersonal relationships in the course of long-term treatment were examined in 87 hypertensive patients and 10 district physicians. Patient's original ideal physician image (sex, age, professional experience, emotional characteristics, role attitude, etc.) and its influence on patient's co-operation in the rehabilitation program are analysed. Major causes of refusal to go on with regular outpatient treatment are discussed.

Individual attitude to disease was evaluated in 249 individuals with marginal arterial hypertension and patients with essential hypertension (EH). Anxiety, hypochondriac and neurasthenic types of response to raised arterial blood pressure were predominant. Hypertensive patients, prone to crises, mostly showed anxiety and neurasthenic response to arterial hypertension, while anosognosic response was less common, as compared to people with occasional hypertensive crises. In hypertensive males, as opposed to females, anosognosic perception of the disease prevailed, while the cardiophobic variant was less common.

RESUMO

A study of 402 individuals with marginal arterial hypertension (MAH) and patients with essential hypertension (EH), stages 1, 2 and 3, demonstrated a variety of psychopathologic syndromes (hypochondria, anxiety, hysteria, depression, cardiophobia) in 52.8, 77.3, 82.5 and 80%, respectively. Specific personality features have been identified as pertaining to MAH (hyperthymism, sthenism, demonstrativeness, psychasthenia) and EH (psychasthenia, intraversion, cycloidy).

RESUMO

Altogether 500 persons with borderline arterial hypertension and patients with essential hypertension and 58 community internists were interviewed with the help of special questionnaires as to opportunities of various rehabilitation measures in outpatient clinics. The problem of the internists' and patients' attitude towards nonmedicamentous (salt-free diet, exercise therapy and autotraining) and medicamentous (intermittent and continuous antihypertensive therapy) methods was discussed. The authors provided information on the main reasons for the rejection of patients with essential hypertension of regular treatment in a clinic.

RESUMO

The interrelationships between the psychic status according to the MMPI test findings and indices of the central hemodynamics, microcirculation and working capacity were studied in 263 persons with borderline arterial hypertension and patients with essential hypertension. Significant non-linear correlations between hypochondriac, anxiety-depressive and hysteric tendencies, on the one hand, and parameters of arterial pressure, peripheral vascular resistance, cardiac output and tolerance to physical exercise, on the other hand, were revealed in essential hypertension.

RESUMO

Bicycle ergometry was used for a study of physical working capacity in 114 women with hypertension, of them in 63 concomitant menopause was diagnosed. A noticeable decrease in physical working capacity and efficacy of functioning of the cardiovascular system during muscular work was demonstrated in patients with hypertension combined with menopause. An opportunity of restored tolerance to physical exercise in most of the patients with hypertension and in more than half of the patients with hypertension combined with menopause was shown during prolonged 6-month hypotensive therapy.

RESUMO

Sixty-two patients with essential hypertension of stage I, II and III were studied for their ability for physical work in the course of a long-term (up to 5 years) hypotensive therapy. The regular use of drugs was shown to restore effectively the hypertensive patients' tolerance to exercise whereas an intermittent irregular therapy produced no significant increase in this tolerance.

A total of 306 patients with stages I, II and III of essential hypertension were examined for the microcirculation, central hemodynamics, physical working capacity and personality traits with the use of the MMPI test. Criteria were obtained for assessing the efficacy of a long-term treatment of patients. They included positive dynamics on the part of the resistive component in the microcirculatory system, decrease in the blood pressure and peripheral resistance to the bloodstream, recovery of the physical working capacity to the level seen in practically normal subjects.

Data on close relationships between indicators of the somatic status (microcirculation, central hemodynamics, physical working capacity) and personal peculiarities of patients with hypertension were obtained by means of factor and regression analysis. Data on the mathematical simulation of physical working capacity in patients with hypertension with account of the somatic parameters and MMP test indices were presented.

Microcirculation and central hemodynamics were studied in 62 patients with essential hypertension and in subjects with borderline arterial hypertension during long-term (up to 5 years) hypotensive therapy. Systematic therapy was found to be efficacious in the aspect of the improvement of the resistive component of microcirculation and blood pressure drop. Occasional treatment did not lead to significant beneficial shifts in micro- and macrocirculation.
