RESUMO

25 specimens of the temporal bone were studied to ascertain specific anatomical features of the pockets and sinuses of the tympanic cavity provoking cholesteatoma recurrence after tympanoplasty. An original method is provided of systemic revision of the tympanic cavity along the perimeter to detect and remove cholesteatoma parts hidden in the tympanic cavity pockets and sinuses. After application of the above method the occurrence of the recurrences went down to 9.7%. Intraoperative findings show that cholesteatoma may invade each pocket and sinus of the tympanic cavity both in primary epi-, epimesotympanitis and in cholesteatoma recurrence following open and closed tympanoplasty. In both groups sinus of the facial nerve, posterior tympanic sinus and tympanic cavity suffer most frequently.

RESUMO

The operative procedure performed for marked deformity of the external nose includes the following stages: reestablishment of nasal breathing (submucosal resection of the nasal septum), plastic repair of the external nose (correction of scoliosis ny means of mobilization of the nasal bones, repair of the saddle nose using cartilage transplants). Rethy method allowed optimization of the operative procedure. A simple and reliable scheme of external nose immobilization is outlined.

RESUMO

A chronic sluggish suppuration in the middle ear, mesotympanitis, leads to impairment of sound conduction and onset of hypoacusis. Otogenic labyrinthopathy is also possible. As to complications, present-day course of mesotympanitis is close to epitympanitis. Intensive conservative therapy should be followed by revision of the middle ear system earlier than 4-6 months of remission. In minor lesions tympanoplasty with retroauricular drainage of the antrum is indicated, in serious affection surgical intervention is needed.

RESUMO

The paper reports two rare cases of sphenoidal osteoma which occur statistically in 1.9% of sphenoidal osteomas. Two women underwent computed tomography because of complaints of progressive headaches. The patients had recurrent unilateral exudative otitis media. CT localized the tumor spread into the cerebral cranium and promoted further treatment design. The authors state that indications to CT diagnosis in patients with headaches should be increased. It is also suggested to make CT-diagnosis more frequently in cases of recurrent unilateral exudative otitis.
