RESUMO

A total of 103 patients with cholelithiasis and those operated on one year ago were investigated using modified cholescintigraphy permitting the determination of a size, duration and spreading of the gastroduodenal reflux. Dynamic cholescintigraphy was shown to be an effective method of the diagnosis and quantitative estimation of the gastroduodenal reflux in patients with cholelithiasis and patients after cholecystectomy. In patients with the postcholecystectomy syndrome the gastroduodenal reflux was detected more frequently, and its quantitative indicators were lower than those in patients with cholelithiasis.

RESUMO

Function of the hepatobiliary system in 35 patients with cholelithiasis of mild and average grave types and in 36 examinees after cholecystectomy without complaints and with the post-cholecystectomy syndrome was assessed with dynamic hepatocholescintigraphy which was shown to be an effective method for assessment of the hepatobiliary system in patients with cholelithiasis and after cholecystectomy. In some patients with cholelithiasis a degree of hepatobiliary disorders, including the frequency of dyskinetic disturbances, grows progressively with the severity of pathology (from a mild clinical type to an average grave one) and is most expressed in patients suffering from the postcholecystectomy syndrome.

Function of the hepatobiliary system was studied in 33 patients with thyroid diseases (11 with diffuse euthyroid goiter, 15 with diffuse toxic goiter, of them 7 with thyrotoxicosis of a mild degree and 8 with thyrotoxicosis of an average severity, and 7 patients with primary hypothyrosis) and in 14 healthy persons of the control group by a radionuclide method using a gamma-chamber and 99mTc-HIDA, a hepatotropic radiopharmaceutical. It was established that absorptive-excretory liver function and the concentration ability of the gall bladder decreased in noticeable thyrotoxicosis and moderate hypothyrosis. Hypermotor dyskinesia of the gall bladder and hypertension of the biliary tract sphincters were revealed in more than half of the patients. Similar changes though quantitatively less pronounced were noted in the patients with euthyroid goiter. Thyrotoxicosis of a mild degree was characterized by the affection of motor function of the gall bladder (hypermotor dyskinesia) and frequent hypertension of Oddi's sphincter. Possible mechanisms of the development of the above changes were discussed.

RESUMO

The motor activity of the gall bladder was studied using cholescintigraphy with 99m Tc-HIDA in 57 patients with chronic cholecystitis and chronic hepatitis and in 9 controls. A comparative analysis of the curves activity-time based upon the elements of images of the external contour and the entire zone of the gall bladder, made it possible to reveal differences in the type of reaction of the gall bladder to the use of cholagogic stimulators (cholecystokinin i.v. and cholagogic breakfast). A method of the processing of the results made it possible to determine the number of contraction phases of the gall bladder during its emptying as well as the true latent period and the period of primary reactions of the biliferous apparatus after taking a food stimulus.
