[The treatment of purulent-destructive complications of acute surgical diseases and damages of the peritoneal cavity organs].

There were presented the data concerning the results of application of the puncture miniinvasive methods of treatment under ultrasonographic investigation control in 43 patients, suffering purulent-destructive complications of acute diseases and damages of the peritoneal cavity organs. Methods of the puncture sanation and drainage were applied. There was established high efficacy of the methods as the compound of detoxicational therapy and ultimate treatment of false pancreatic cyst and the abdominal cavity abscess.

[Applications of laparoscopy for duodenal ulcer perforation]. / Primenie laparoskopicheskikh operatsii po povodu perforativnoi iazvy dvenadtsatiperstnoi kishki.

Experience of treatment of 142 patients, operated on in clinic in 2001 yr for duodenal perforative ulcer was summarized. In 130 patients operation was performed by an open access method and in 12--by laparoscopic one. Laparoscopy was applied for the perforative aperture closure (in 5 patients) and for excision of ulcer with duodenoplasty (in 7). The operation duration have constituted (78 +/- 4.2) min at average. Postoperative complications were absent, all the patients survived. Duration of the patient treatment conducted in stationary after performance of laparoscopic operation have constituted (6.4 +/- 0.6) days at average and after application of open method--(9.6 +/- 5) days.

[The choice of timing and methods in the surgical treatment of thoracic bone injuries in combined closed thoracic injuries during acute and early periods of traumatic disease]. / Vybor srokov i metodov khirurgicheskogo lecheniia povrezhdeniia kostei grudnoi kletki pri sochetannoi zakrytoi travme grudi v ostrom i rannem periodakh travmaticheskoi bolezni.

Clinico-statistical analysis of the treatment results of 624 injured persons for the period from 1990 till 1995 year was conducted. In 509 (81.6%) of them the combined closed thoracic trauma was revealed, in 15 (18.4%)--the isolated closed affection of the skull, thorax, abdomen and extremities have occurred. The anatomic-functional model of the injury severity and prognosis outcome estimation was elaborated, an optimal timing and surgical treatment methods of the thoracic cage bones injury was substantiated, what have promoted to lower lethality from 50%--in injured persons, operated on without taking into account the trauma severity and prognosis outcome, to 31.6%--in operated on in the postponed order.

[Surgical tactics in perforated stomach and duodenal ulcers]. / Khirurgicheskaia taktika pri perforativnoi iazve zheludka i dvenadtsatiperstnoi kishki.

The results of treatment of 410 patients with a perforated gastric and duodenal ulcer were summarized. A dependence of lethality and type of the operation performed on time elapsing from the moment of perforation, presence of concomitant diseases is shown.

[Surgical treatment of acute intestinal obstruction]. / Khirurgicheskoe lechenie ostroi neprokhodimosti kishechnika.

The experience with treatment of acute ileus for the 5 years in 1032 patients is summarized. Operated on were 269 patients. The different types of operative interventions with a wide use of intubation, adsorbents, laparostomy were performed. The postoperative lethality was 9.2%.

[Pylorus-reconstructing operations in complicated gastric and duodenal ulcer]. / Pilorovosstanavlivaiushchie operatsii pri oslozhnennoi iazvennoi bolezni zheludka i dvenadtsatiperstnoi kishki.

The results of treatment of 185 patients with complicated gastric and duodenal ulcer disease have been analysed. The results of operative treatment in 132 patients followed up for the period of from 1 to 7 years were studied. In gastric ulcer, resection of the stomach with preservation of the pyloric sphincter, or its reconstruction by means of the flaps taken from the antrum, is indicated; in duodenal ulcer--vagotomy, ulcer excision added by pylorus-preserving and pylorus-reconstructing methods of drainage operations.

[Radical treatment of penetrating duodenal ulcer]. / Radikal'noe lechenie penetriruiushchei iazvy dvenadtsatiperstnoi kishki.

The results of treatment of 450 patients with duodenal ulcer have been analysed. Of them, in 198 (44%), ulcer penetration into the adjacent organs was revealed. An excellent and good results were noted in 80.3% of the patients. Vagotomy with an ulcer excision and subsequent restoration of the pyloric sphincter function in penetrating duodenal ulcer permits to reduce lethality and achieve stable recovery of the majority of patients.

[Treatment of complicated retroampullar duodenal ulcers]. / Lechenie oslozhnennykh zaampul'nykh iazv dvenadtsatiperstnoi kishki.

Of the 450 patients with complicated duodenal ulcer disease, in 23 (5.8%) the ulcer was located distally to the duodenal ampulla. On the basis of studying the results of surgical treatment, a conclusion is made, that the selective proximal vagotomy in combination with the ulcer excision and duodenoplasty is an operation of choice in complicated retroampullary ulcers. The operative interventions correcting the disturbances in the duodenal patency in preserved function of the pyloric sphincter contribute to prevention of duodenogastric reflux and alkaline reflux-gastritis.

[Duodenogastric reflux after organ-sparing operations in complicated duodenal ulcer]. / Duodenogastral'nyi refliuks posle organosokhraniaiushchikh operatsii po povodu oslozhnennoi iazvennoi bolezni dvenadtsatiperstnoi kishki.

On the basis of treatment of 105 patients operated on for complicated duodenal ulcer disease, it was established that performance of the organ-preserving operation with the ulcer excision supplemented with the pylorus-restoring and pylorus-preserving intervention permitted to reduce considerably the incidence of duodenogastric reflux.

[Treatment of complicated giant duodenal ulcers]. / Lechenie oslozhnennykh gigantskikh iazv dvenadtsatiperstnoi kishki.

Examined were 305 patients with complicated duodenal ulcer disease, in 28--the giant ulcers were revealed. The operative treatment of such patients should be early and aimed at preservation, or restoration of the pyloric sphincter function. This permits to achieve good functional results.
