RESUMO

BACKGROUND AND PURPOSE: Perfusion parameters obtained in F-18 FDG PET/CT performed for staging purposes in breast cancers may provide additional information about tumor biology as well as glucose metabolism. The aim of this study was to evaluate throughout F-18 FDG PET/CT the relationship between blood flow and glucose metabolism and histological parameters of the primary tumor, normal mammary gland, and axillary lymph nodes in breast cancer patients. MATERIALS AND METHODS: Sixty six female patients (mean age 51 y ± 12,81) were prospectively included to this study. We performed dynamic blood flow (f) study that started with 296-444 MBq (8-12 mCi) F-18 FDG injection and lasted for 10 minutes, and glucose metabolism (m) imaging one hour later. On each frame, mean activity concentration (AC) values (Bq/mL) were recorded on a spherical volume of interest (VOI) having a volume of ~ 1 cm3 on the hottest voxel of primary tumor (T), across normal breast gland (NG) and ipsilaterally axillary lymph nodes (iLN). Correlations among PET parameters and estrogen receptor (ER), progesterone receptor (PR), human epidermal growth factor receptor 2 (c-erbB2) and Ki67 index were analyzed. RESULTS: T volume (TV) ranged from 1.1 to 85.28 cm3 [median (IR): 6.44 (11.78)]. There were positive correlations between c-erbB2 and TACf and between c-erbB2 and iLNACf (p = 0.045, r = + 0.248; p = 0.050, r = + 0.242). In the ER positive (ERP) patients, TV and TACm were significantly lower than those of ER negative (ERN) (respectively p = 0.044 and p = 0.041). In patients with two positive Ki-67 indices, iLN-SUVmax was significantly higher than one-positive patients (p = 0.020). There was a negative correlation between NGACm and histological grade of tumor (p = 0.005, r = - 0.365). CONCLUSIONS: Breast cancer shows differences in progression, metastasis and survival due to its diversity in terms of molecular, biological and angiogenesis. High glucose metabolism in breast cancers is associated with tumor aggressiveness. Being able to examine tumor tissue characteristics such as blood flow and glucose metabolism with a single diagnostic technique and to reveal its relationship with histological parameters can provide a reliable pretherapeutic evaluation in breast cancers.

RESUMO

OBJECTIVE: The aim of this study is to analyze the frequency of lung injury and the sensitivity of the diethylenetriamine penta-acetic acid (DTPA) clearance test in detecting lung injury in patients undergoing radiotherapy (RT) to the thorax. MATERIAL AND METHOD: Twenty individuals scheduled for RT for lung cancer were included as the patient group. The healthy control group consisted of 20 age and gender-matched individuals who were nonsmokers with no history of comorbidities. We conducted follow-up with patients at 0-1-6 months, performing carbon monoxide diffusion test (DLCO), DTPA clearance test (excluding the first month), and high-resolution computed tomography of the thorax. The control group was followed up with DLCO between the baseline and 6th months. RESULTS: Ninety percent of the patient group was male, and the median age was 62 years. Seventy percent of the patients had squamous cell carcinoma and adenocarcinoma. Pneumonitis was detected in the patient group in the first month (100%) and fibrosis in the sixth month (%100) Both at the beginning and in the sixth month, the DLCO values of patients who received RT were lower than those of the control group ( P  = 0.001 and P  < 0.001, respectively). While DTPA clearance was similar between irradiated and non-radiated lungs at the beginning, there was a substantial decrease in the irradiated lung in the sixth month( P  = 0.001). There was no significant correlation between malignancy type, RT dose, and tumor size( P  > 0.05). CONCLUSION: The DTPA clearance test could be an alternative method for demonstrating radiation injury in patients receiving RT.

RESUMO

This study aims to investigate the impact of coating with ground calcium carbonate (GCC) on the optical properties and filler content of white top testliner (WTT) papers. The paper properties investigated include brightness, whiteness, opacity, color coordinates, and yellowness. The results showed that the amount of filler mineral used in the coating process significantly affected the optical properties of the paper. The use of 15% total solids of GCC in the coating suspension resulted in the highest level of whiteness and improved the brightness value by 6.8%. The use of 7% total solids of starch and 15% total solids of GCC reduced the yellowness index by 85%. However, the use of only 7 and 10% total solids of starch had an adverse effect on the yellowness values. The surface treatment led to a significant increase in the filler content of the papers, with a maximum of 23.8% achieved using a coating suspension with 10% total solids of starch solution, 15% total solids of GCC suspension, and 1% dispersant. The starch and GCC in the coating suspension were found to have a direct impact on the filler content of the WTT papers. The addition of a dispersant improved the uniform distribution of the filler minerals and increased the filler content of the WTT. The water resistance of WTT papers increases due to the use of GCC, while their surface strength remains at an acceptable level. The study demonstrates the potential benefits of the surface treatment in terms of cost savings and provides valuable information on the impact of the treatment on the properties of WTT papers.

RESUMO

PURPOSE: In our study, tumor heterogeneity was evaluated using the parameters skewness and kurtosis on pretreatment Ga-68 DOTA-TATE PET/CT to assess therapy responses of lesions in patients with gastroenteropancreatic neuroendocrine tumors (GEP NETs) treated with peptide receptor radionuclide therapy (PRRT). METHOD: We retrospectively studied the texture analysis with relatively simple first-order parameters skewness and kurtosis on pretreatment Ga-68 DOTA-TATE PET/CT from 22 patients (eight females, 14 males; with a mean age: 54 ± 11 years) with GEP NETs who were treated with 2-6 therapy cycles of Lu-177 DOTA-TATE. A total of 326 lesions of 22 patients were evaluated in terms of treatment response by Ga-68 DOTA-TATE PET/CT examination performed before and after treatment. Receiver-operating characteristic (ROC) curves were generated to assess skewness and kurtosis to discriminate between responder and nonresponder lesions. RESULT: A total of 326 lesions of 22 patients were evaluated and 137 lesions responded partially or completely to the treatment, 189 lesions did not respond to treatment, remained stable or progressed. The skewness and kurtosis values of the lesions which did not respond to the PRRT were significantly higher than those with response to PRRT treatment (P < 0.001 and P = 0.004, respectively). However, ROC curves provided a moderate area under the curve value for skewness and a relatively low value for kurtosis (0.619 and 0.518, respectively). CONCLUSION: Texture analysis using skewness and kurtosis of the lesions on pretreatment Ga-68 DOTA-TATE PET/CT was able to predict their responsiveness to PRRT.

RESUMO

BACKGROUND: In this study, 13 different extracts were investigated which are grown in the region of Erzurum. OBJECTIVE: The aim of this study was to screen various plant extracts that are known and used for medicinal purposes such as Ferula communis L., Rumex patientia L., Gundelia tournefortii L., Rheum ribes L., Asphodeline taurica, Polygonum arenastrum, Allium schoenoprasum L., and Ferula orientalis L. MATERIALS AND METHODS: Medicinal parts of plants such as leaves, flowers, and stems were investigated by 2,2-diphenyl-1-picrylhydrazyl, ferric reducing antioxidant power, and cupric reducing antioxidant capacity assays: Centaury and Blackthorn. Total phenolic content, total flavonoid content, and antimicrobial properties were also determined. Antibacterial and antifungal activities were investigated by the microdilution method and the agar diffusion method respectively. RESULTS: Accordingly, the results of the Rheum ribes L. plant have the highest antioxidant activity among all analyses made. But in almost all antioxidant analysis methods, the lowest antioxidant activity was found in Ferula orientalis L. According to the antibacterial analysis applied, it was found that the plant extracts were generally more effective on yeast strains than the test bacteria used; that is, most of the plants have antifungal effect. CONCLUSIONS: Due to their antimicrobial, antifungal, and antioxidant properties, the extracts of these plants might be used as natural sources in the pharmaceutical and cosmetic industries.

RESUMO

BACKGROUND: The aim of this paper was to compare the results of postoperative I-131 remnant ablation therapy using a quantitative data in the low activity (1110 MBq) and standard dose (3700 MBq). METHODS: The study included two groups of patients with low risk differentiated thyroid cancer (DTC): Group L (low dose group) included 54 patients who were treated with 1110 MBq I-131 and Group S (standard dose group) included 61 patients treated with 3700 MBq. The postoperative thyroid remnants were assessed with the pretreatment thyroid uptake test (PTUT) and the whole body scans (WBS) were performed in the 7th day after the ablation treatment. We obtained the average count per pixel from the standard region of interest analysis of the thyroid bed (Tavc), liver (Lavc), thigh (Thavc) and whole body (WBkc). At the sixth month after the treatment, WBS were performed to 106 patients (45 patients from Group L and 61 patients from Group S) to evaluate the success of ablation treatment. RESULTS: A significant difference in PTUT and Tavc was not found between the two groups (P>0.05). However, Lavc, Thavc and WBkc were significantly higher in Group S compared with Group L (P<0.001). Although the percentage of ablation was higher in Group S (49 of 61 patients, 80.3% versus 34 of 45 patients, 75.6%), the statistical difference was not significant (P>0.05). CONCLUSIONS: In low risk DTC patients, low dose radioactive iodine can ablate thyroid remnants as effectively as a higher dose with less radiation exposure to other non-target organs and the whole body.

RESUMO

AIM: To assess the additional contribution of the combined imaging approach with Ga-DOTA-TATE and fluorine-18 fluorodeoxyglucose (F-FDG) on the basis of volumetric parameters in neuroendocrine tumors (NETs). MATERIALS AND METHODS: Forty-one patients with NET (19 women, 22 men; age range: 30-79 years; mean age: 56.7±12.3 years) underwent Ga DOTA-TATE and F-FDG PET/computed tomography (CT) imaging. Within the drawn regions of interest, in addition to evaluating the maximum and mean standardized uptake values (SUVmax and SUVmean), the metabolic tumor volume (MTV) and total lesion glycolysis (TLG) in F-FDG PET/CT imaging, and the somatostatin receptor density and total lesion somatostatin receptor expression (TLSRE) in Ga somatostatin receptor imaging (SRI) were measured. The patients were graded on the basis of the proliferation index: well (G1; Ki-67 ≤2), moderately (G2; Ki-67=3-20), and poorly (G3; Ki-67>20) differentiated groups. RESULTS: Of the 41 NET patients, 22, 14, and five were in the G1 (53.7%), G2 (34.1%), and G3 (12.2%) groups, respectively. Liver metastases had significantly higher TLSRE values than the TLG values. Ki-67 levels showed a positive correlation with the primary tumor MTV and TLG values. Cg-A levels had a positive correlation with the volumetric parameters of the whole-body tumor burden (P<0.001). CONCLUSION: Ga SRI and F-FDG PET/CT provide complementary information on treatment protocol and response assessment. While assessing the prognosis and tumor aggressiveness, lesions and whole-body tumor burdens can be calculated on the basis of volumetric parameters by F-FDG PET/CT using MTV and TLG, and by Ga SRI using somatostatin receptor density and TLSRE.

RESUMO

Peptide receptor radionuclide therapy with Lu or Y is promising with successful results in somatostatin receptor-positive tumors. In all radiation therapies, knowledge of the radiation dose received by the target, and other organs in the body is essential to evaluate the risks and benefits of any procedure. We report a case of liver metastases from a rectal neuroendocrine tumor, which was treated with Y DOTANOC. Posttreatment whole-body planar images were acquired through Bremsstrahlung radiations of Y on a γ-camera, and thoracolumbar PET/CT images were acquired on PET.

RESUMO

OBJECTIVE: The aim of this study was to evaluate the diagnostic and prognostic efficacy of gated single-photon emission computed tomography (GSPECT) in patients with acute chest pain and to compare quantitative GSPECT parameters and the coronary angiographic SYNTAX score. MATERIALS AND METHODS: A total of 168 patients who presented with clinical symptoms of acute chest pain were enrolled in the study. Study participants were divided into two groups according to the risk of acute coronary syndrome (ACS): low-intermediate and high risk. All participants underwent rest or stress-GSPECT (R/S-GSPECT). Coronary angiography was performed in all high-risk patients and the SYNTAX score was determined. All patients were followed for 24 ± 3 months and monitored for the occurrence of major adverse coronary events (MACE). RESULTS: Among patients with low-intermediate ACS risk, R-GSPECT and S-GSPECT were associated with 100 and 86% sensitivity, 99 and 98% specificity, 100 and 98% negative predictive value, 80 and 86% positive predictive value, and 98 and 97% accuracy, respectively. At follow-up, MACE occurred in 16 patients. Among high-risk patients, GSPECT quantitative parameters were the most significant predictors of MACE in Cox regression analysis. CONCLUSION: R/S-GSPECT, a noninvasive diagnostic method, is associated with an excellent safety profile and exceptional diagnostic and prognostic accuracy in cases of ACS.

RESUMO

The purpose of this study is to compare the strain echocardiographic and scintigraphic parameters for evaluating of the left ventricular (LV) functions in patients with anterior myocardial infarction (MI). Fifty-four patients (male/female: 36/18; mean age 62 ± 13 years) with anterior MI were prospectively enrolled. All patients were performed gated myocardial perfusion scintigraphy gated single-photon emission computed tomography (GSPECT) and echocardiography (EC). GSPECT data were processed and analyzed using 4D-MSPECT (4DM, Invia Medical Imaging Solutions, Ann Arbor, MI, USA). The echocardiographic strain (S) and strain rate (SR) values were calculated. The results obtained by these techniques were compared each other. A total of 918 segments of LV wall were evaluated. In all patients, 385 segments were automatically scored as normokinetic, 206 as hypokinetic, 122 as akinetic, 205 as dyskinetic and 300 as normal thickening, 348 as decrease thickening and 270 as no thickening. The means of S and SR values in thickening and motion score groups according to GSPECT were statistically different from each other (P < 0.001). There was a negative significant correlation between LV wall thickening sum score and S and SR and between LV wall motion sum score and S and SR (P < 0.001). There was a good correlation between GSPECT and echocardiographic LV-ejection fraction (r = 0.7, P < 0.001). GSPECT and strain EC are similar in quantitative grading of the severity of regional and global myocardial dysfunction in patients with anterior MI and these techniques provide valuable diagnostic information.

RESUMO

UNLABELLED: Primary liver carcinosarcoma is a very rare disease. There have been only a few cases described to date, none of which has been imaged by bone scintigraphy. A 69-year-old man who developed right back pain and weight loss was admitted to our hospital. Tenderness of the right upper abdomen, hepatomegaly, and a giant mass were the main physical examination signs. Abdominal ultrasonography showed a large lobulated heterogeneous echogenic solid mass with multiple cystic areas of varying size and a highly echogenic region that had posterior acoustic shadowing within the mass in the right lobe of the liver. Labeled erythrocyte imaging and (99m)Tc-nanocolloid liver-spleen scan were performed to exclude hemangioma, showing a large intrahepatic photon deficient area. (99m)Tc-methylene diphosphonate (MDP) bone scintigraphy revealed heterogeneous, irregular and dense activity accumulation thought to be osteoblastic activity in the liver mass. Postsurgical pathological diagnosis revealed "carcinosarcoma". CONFLICT OF INTEREST: None declared.

RESUMO

Assessment of left ventricular (LV) function in patients with myocardial infarction (MI) provide useful diagnostic and prognostic information. Up to date, single photon emission tomography (SPET), positron emission tomography (PET), multidetector computed tomographic angiography, echocardiography (EC) and magnetic resonance imaging (MRI), have been used to examine LV parameters. However, due to limitations of some imaging methods, new studies are directed to improve myocardium function evaluation. In conclusion, SEC and GSPET can be applied to semi-quantitatively assess LVEF and regional wall motion abnormalities in a noninvasive manner. These techniques can provide strong diagnostic and prognostic information related to anterior myocardial infarction. In addition to this, nitrate enhanced GSPET allows to identify stunning and hibernating myocardium. New methods of reconstruction on GSPET systems will better improve image quality using lower count rates.

RESUMO

P-glycoprotein (Pgp) overexpression has been shown to be correlated with resistance to chemotherapy in patients with malignant breast tumors. The aim of our study was to investigate the role of technetium-99m-tetrofosmin (99mTc-TF) as a functional imaging agent reflecting Pgp expression in these tumors. We prospectively studied 28 patients (26 females, 2 males; mean age, 53.07±9.88 years; range, 38 to 70 years) with breast cancer to ascertain the relationship between the degree of accumulation (lesion/nonlesion=L/NL) and percentage washout (WO%) rate of 99mTc-TF and expression of Pgp in tumor tissues. All patients received 555-740 MBq of 99mTc-TF intravenously at the arm controlateral to the suffering breast. Planar images were obtained 10 and 120 min post injection from prone lateral and anterior views with an acquisition time of 5 min. Visual and semiquantitative measurements were performed. The L/NL ratios and WO% rates were calculated semiquantitatively. Immunohistochemical studies were performed on paraffin sections using a monoclonal antibody, JSB-1. The L/NL ratios and WO% rates were related with the level of Pgp determined immunohistochemically. Our results showed an inverse correlation between the L/NL ratios of 99mTc-TF and the density of Pgp expression in tumor tissues, whereas there was no appreciable correlation between the tumor WO% rates of 99mTc-TF and the level of Pgp expression (P=0.275). The values for the L/NL ratios were significantly lower for those tumors expressing Pgp at high levels than those with intermediate or no Pgp expression (P<0.002 and P<0.04). In conclusion, although our results warrant further studies, our data strongly suggest that 99mTc-TF imaging is useful to noninvasively determine the presence of multidrug resistance in patients with breast cancer.

RESUMO

The patient with a history of bone pain and muscle weakness, was thought to have oncogenic osteomalacia as a result of biochemical investigations and directed to Nuclear Medicine Department for a whole-body bone scintigraphy and 111In-octreotide scintigraphy. There was no focal pathologic tracer uptake, but generalized marked increase in skeletal uptake on bone scintigraphy. Octreotide scintigraphy showed accumulation of octreotide in the region of the left lobe of the thyroid gland in the neck. Thereafter, parathyroid scintigraphy was performed with technetium-99m labeled metroxy-isobutyl-isonitryl (99mTc-MIB) and MIBI scan demonstrated radiotracer uptake at the same location with octreotide scintigraphy. The patient underwent left inferior parathyroidectomy and histopathology confirmed a parathyroid adenoma. Somatostatin receptor positive parathyroid adenoma may show octreotide uptake. Octreotide scintigraphy may be promising and indicate a possibility of using somatostatin analogues for the medical treatment of somatostatin receptor positive Conflict of interest:None declared.

RESUMO

UNLABELLED: We report the case of a 64-year-old man followed up for two years as suffering from differentiated thyroid cancer (DTC). In the patient's follow up, despite thyroglobulin level and I-131 whole body scan results being normal, metastases were identified at the 4th thoracic vertebra corpus by MR. Histopathological findings were carcinoma metastases. F-18 FDG PET/CT showed increased metabolic activity in the right renal mass, bilaterally in the surrenal gland, multiple lymph nodes in the thoracic and abdominal para-aortic region and in multiple vertebral and pelvic bones. An excisional biopsy of the right renal mass was reported as renal cell carcinoma. Immunohistochemical staining performed retrospectively to the first thyroid preparation showed renal cell carcinoma metastases. Consequently, any patient who presents with a thyroid nodule can also be considered as possibly suffering from metastatic disease. F-18 FDG PET/CT can provide valuable information in finding the primary focus and metastases. CONFLICT OF INTEREST: None declared.

RESUMO

PURPOSE: Although liver biopsy remains the gold standard in the staging of liver fibrosis in chronic hepatitis C virus (HCV) infection, several noninvasive methods are under evaluation for clinical use. The aim of this study was to evaluate the utility of hepatobiliary function through technetium-99m-N-(-3-bromo-2,4,6-trimethylacetanilide) iminodiacetic acid (Tc-mebrofenin) scintigraphy in evaluating liver fibrosis in patients with chronic HCV infection. METHODS: We studied 62 patients with HCV (18 men, 44 women). The patients were allocated into three groups according to histopathological score: group 1: portal/periportal fibrosis (21 patients; eight men, 13 women); group 2: bridging fibrosis (23 patients; seven men, 16 women); and group 3: incomplete/complete cirrhosis (18 patients; three men, 15 women). As a control group, 20 healthy volunteers (six men and 14 women) were studied. Hepatocyte mebrofenin uptake rate, the time required for maximal hepatic activity (Tmax), and the time required for peak activity to decrease by 50% (T1/2max) were calculated using Tc-mebrofenin cholescintigraphy. Scintigraphic parameters were correlated with biochemical parameters and liver histopathology. RESULTS: The uptake rates were significantly decreased in all groups with fibrosis compared with the controls (P<0.05). The correlation between the severity of fibrosis and Tc-mebrofenin uptake rate was strongly significant (r=-0.81, P<0.0001). Tmax and T1/2max were significantly prolonged in groups 2 and 3 compared with the controls. Histopathology score was correlated moderately with Tmax and T1/2max (r=0.61, P<0.0001 and r=0.52, P<0.0001, respectively). CONCLUSION: The assessment of hepatobiliary function by Tc-mebrofenin scintigraphy may be a good choice for assessing the severity of liver fibrosis in patients with HCV.

RESUMO

Nasal packing may influence the mucociliary clearance of the nose in the postoperative healing phase. In an attempt to overcome some of this problem, a haemostatic septal suturing technique was conceived. In this prospective study, we aimed to investigate the effects of nasal packing and septal suturing technique on mucociliary clearance by rhinoscintigraphy. Forty-eight adult patients who had undergone septoplasty were included in the investigation. Preoperatively, the patients were allocated into three groups: group 1, fingerstall packs filled with gauze and smeared with vaseline were used (11 male, 4 female); group 2, silicon septal splint packs were used (11 male, 4 female); group 3, haemostatic septal sutures were used (14 male, 4 female). Mucociliary clearance was measured by rhinoscintigraphy in all patients before surgery and 6 weeks after surgery. The nasal mucociliary clearance was presented as the velocity (mm/min) of nasal mucociliary transport of the (99m)Tc-MAA droplet. The mean velocity of nasal mucociliary clearances before and after surgery for group 1, group 2 and group 3 were 1.85 +/- 0.67 versus 2.43 +/- 0.78 mm/min, 2.36 +/- 0.80 versus 2.92 +/- 0.96 mm/min and 2.03 +/- 0.58 versus 2.62 +/- 0.65 mm/min, respectively. A significant difference in nasal mucociliary clearance was observed after surgery in all groups (p < 0.001). No significant differences were found between the groups regarding mucociliary clearance before and after surgery. Patients with septal deviation have a prolonged mucociliary transit time as compared with postoperative. Nasal packing did not significantly influence the mucociliary clearance in the postoperative healing phase.

RESUMO

Leptin is a product of the obese gene and plays an important role in the regulation of body weight and food intake. Weight and appetite are frequently altered in depression. So far, inconsistent results have been reported in terms of leptin levels in depression. Therefore, the authors investigated serum leptin levels in patients with depression and in healthy controls, and whether there was any alteration throughout antidepressant treatment. Female patients showed significantly higher leptin levels than those of the control females both before and after the response to antidepressant treatment, whereas no difference was found between the male patients and the male controls. The improvement from depression with antidepressant treatment caused a further elevation on the leptin levels, in both female and male patients. These findings confirm an increase in leptin levels in depressive patients and presence of a sexual dimorphism. Moreover, clinical response to antidepressant treatment seems to have an additional increasing effect on leptin levels.

RESUMO

Increasing environmental restrictions require a reduction in the use of chlorine-containing bleaching agents mainly chlorine gas. There are several approaches to this goal, including changes in the cooking process and the influent treatment. The aim of this study is to improve the physical and optical properties of chemithermomechanical pulp (CTMP) which has low brightness and strong tendency towards aging in chlorine-free bleaching sequences. For this reason, initially 7 different sets of single stage hydrogen peroxide bleaching operations were performed. After the analysis of the data obtained, the optimum bleaching conditions were as: H2O2 ratio on oven dry (o.d) raw material: 2%, NaOH ratio (on o.d raw material): 1.5%, MgSO4 ratio (on o.d raw material) : 0.5%, Na2SiO3 ratio (on o.d raw material) : 3%, EDTA ratio (on o.d raw material) : 0.5%, reaction time: 60 minutes, reaction temperature: 70 degrees C and concentration: 16%. Then, 11 different sets of two and multistage H2O2 (P), NaBH4 (B), and Na2S2O4 (S) bleaching operations were carried out. Optimum bleaching conditions were found in bleaching of BPSP with 4 stage sequences. As a result, CTMP brightness was observed to increase from 47% to 81.37% elrepho while yellowness of bleached pulps decreased from 321 per thousand to 164.01 per thousand elrepho.

RESUMO

Depressive patients with psychotic features demonstrate distinct biological abnormalities in the hypothalamic-pituitary-adrenal axis (HPA), dopaminergic activity, electroencephalogram sleep profiles and measures of serotonergic function when compared to nonpsychotic depressive patients. However, very few functional neuroimaging studies were specifically designed for studying the effects of psychotic features on neuroimaging findings in depressed patients. The objective of the present study was to compare brain Single Photon Emission Tomography (SPECT) images in a group of unmedicated depressive patients with and without psychotic features. Twenty-eight patients who fully met DSM-IV criteria for major depressive disorder (MDD, 12 had psychotic features) were included in the study. They were compared with 16 control subjects matched for age, gender and education. Both psychotic and nonpsychotic depressed patients showed significantly lower regional cerebral blood flow (rCBF) values in the left and right superior frontal cortex, and left anterior cingulate cortex compared to those of controls. In comparison with depressive patients without psychotic features (DwoPF), depressive patients with psychotic features (DwPF) showed significantly lower rCBF perfusion ratios in left parietal cortex, left cerebellum but had higher rCBF perfusion ratio in the left inferior frontal cortex and caudate nucleus. The present study showed that DwPF have a different rCBF pattern compared to patients without psychotic features. Abnormalities involving inferior frontal cortex, striatum and cerebellum may play an important role in the generation of psychotic symptoms in depression.
