INTRODUCTION: Disturbances in testicular integrity have the potential to cause severe concerns about masculinity, body image, and sexual function. Testicular volume replacement surgery with prosthesis has been known about since 1941, although esthetic procedures for testicular augmentation have not been satisfactory. AIM: To describe an unprecedented testicular augmentation surgical technique that is specially focused on preserving testicular function and providing a favorable esthetic outcome. METHODS: We present a case of a 45-year-old man with body dysmorphic disorder. Surgical treatment using a new technique by placing a chin implant on the testicle was offered and accepted. RESULTS: Good symmetry between both testicles was achieved. No complications were reported. One month after the procedure, the patient started a satisfactory sex life. Functional monitoring was normal during the year after the procedure. CONCLUSIONS: This technique is unprecedented in literature, it represents a safe and effective therapeutic alternative; nevertheless, more experience in performing this procedure is needed.

UNLABELLED: The objective of the present study was to determine the efficacy and safety of a fixed dose of vardenafil in the treatment of patients with erectile dysfunction (ED). MATERIAL AND METHODS: This was an open label, prospective and multicentric trial. After a 4-week wash out period, all patients received 20 mg of vardenafil given on demand for 12 weeks. Primary efficacy variables were the erectile function domain of the International Index of Erectile Function (IIEF), answers to questions 2 and 3 of the Sexual Encounter Profile (SEP) and the Global Assessment Question (GAQ). All adverse events were recorded and reported. RESULTS: 229 patients were screened. 177 received at least one dose of vardenafil and were included in the safety analysis. Mean age was 54.4 years old. Etiology of ED was organic or mixed in 77% of the patients. Erectile function domain of the IIEF changed from a basal mean score of 14.8 to 25.5 at the end of the study. 80.5% of the patients reported erections of rigidity and duration enough for satisfactory sexual intercourse and 93.3% improved their erections at the end of the study. Adverse events were mild to moderate and the most common were headache, dyspepsia, rhinitis and facial flushing. The drop out rate due to adverse events was 1.7%. CONCLUSIONS: This multicenter study confirms the high efficacy of this new phosphodiesterase type 5 inhibitor, vardenafil. There was a low rate of discontinuations due to adverse events and a favorable safety profile. The results of this study are similar to the results of other studies conducted in other parts of the world.

Objetivo: Determinar la prevalencia de la disfunción eréctil en población urbana de México, en varones entre los 18 y los 40 años. Material y métodos: Se llevó cabo un estudio epidemiológico, transversal y analítico, mediante una encuesta en hogares aplicada a personas entre 18 y 40 años, realizada por encuestadores profesionales, previamente adiestrados y estandarizados. El trabajo fue coordinado por el Capítulo de Disfunción Eréctil de la Sociedad Mexicana de Urología y por la Unidad de Investigación Médica de la empresa de investigación Gallup México. Se seleccionó una muestra aleatoria en población urbana de tres ciudades (Distrito Federal, Guadalajara y Monterrey), tomando como base los datos sociodemográficos del censo nacional, con un total de 1,800 encuestados. Resultados: La prevalencia de disfunción eréctil para este grupo de edad fue de 9.7 por ciento: 7.9 por ciento correspondió a disfunción eréctil leve, 1.2 por ciento moderada y 0.6 por ciento severa. En relación con la percepción que tuvieron los entrevistados sobre su eyaculación, se encontró que el 80.2 por ciento la refirieron como normal, 12.8 por ciento como precoz y el 7.0 por ciento como retardada. De las variables sociodemográficas, de comorbilidad y hábitos de vida, sólo la escolaridad mostró una dependencia, a expensas de las personas con educación primaria, en las categorías de disfunción eréctil leve y severa. Las respuestas subjetivas sobre su capacidad para tener una erección y experimentar orgasmo, así como sobre la evaluación de su estado emocional, fueron variables que permitieron hacer una correcta predicción en el 90 por ciento de los casos. Conclusiones: La DE es producto de una amplia variedad de condiciones y resultó ser un buen indicador sintomático de otros aspectos de la vida del individuo. La prevalencia de DE en este grupo de edad, resultó ser importante.
