RESUMO

Requirements applicable to as well as the construction of electrodes to stimulate the cardiac function are presented in the article. The importance of pacemakers at a wide range of heart diseases is demonstrated.

RESUMO

The aim of this research was analysis of hemodynamic efficacy and pumping function of the heart under conditions of various physiologic cardiostimulation regimens at rest and exercise. A total of 112 patients were examined and implanted this or that pacemaker. The mean age was 36.8 +/- 1.4 years, there were 52 male and 60 female patients. The results indicate that the most effective were the physiologic regimens, particularly those permitting an adequate increase of the pumping function at the expense of stimulation rate increment in response to exercise. When these regimens were used physical activity increased from 50 to 75% as a result of minute volume increase by 1.5-2.2 times. These results recommend a wider use of pacemakers with an automatically changing stimulation frequencies in response to exercise.

RESUMO

Under analysis were causes of reoperations in bradyarrhythmic forms of disturbances of conductivity of the heart. 565 operations connected with electrocardiostimulation were performed on 276 patients. Reoperations in patients with the myocardial stimulation were made on 63 patients, with the endocardial stimulation--in 226 patients. Causes of reoperations were as follows: depletion of the source of ECS--31.5%; dislocation of the electrode--8.2%; damage of the electrode--15%, suppuration of the bed, decubitus of the stimulator--28.8%; competing rate--7.4%; break of the contact in the connecting system of the electrode and cardiostimulator--9.4%.
