RESUMO

Ninety six patients with hemorrhagic fever and the renal syndrome were followed up. Reaferon was included into the complex therapy of 57 cases. The drug was administered intramuscularly in a dose of 2 x 10(6) IU twice a day for 5 days. The use of reaferon in the complex therapy resulted in a decrease in the period of the general intoxication by 3.4 +/- 1.3 days. The use of the drug at the initial stages prevented acute renal failure. When reaferon was used at the stage of oliguria, the level of proteinuria and the risk of the further affection of the kidneys lowered. After the completion of the treatment course with the use of reaferon, a significant increase in the content of the T-helper cells was observed which could be indicative of an activation of the mechanisms providing the immunity development and pathogen elimination. Reaferon promoted a decrease in the concentration of the circulating immune complexes of the blood serum. No significant adverse reactions to the use of reaferon were recorded. The results are in favour of the reaferon use in complex therapy of patients with hemorrhagic fever and the renal syndrome, especially at the initial stages until the syndrome of acute renal failure developed.

Willebrand's factor (WF), antithrombin III (AT-III), blood 5'-nucleotidase and athrombogenic properties of the vascular wall were studied in the course of the disease in 69 patients suffering from hemorrhagic fever with renal syndrome (HFRS). The patients were chiefly under 27 years, had severe or moderate condition. It was found that WF rose 3-fold in oliguria, but returned to normal values by convalescence. AT-III remained stable. High activity of 5'-nucleotidase reached the pick in oliguria and polyuria. As shown by a cuff test, athrombogenic potential of the vascular wall remained low. A role of vascular endothelial impairment in HFRS is discussed.

In examination of 50 patients with impotence, pathological venous drainage (PVD) was revealed in 11 of them. Perfusion graded cavernosometry/cavernosography is the only method by which PVD can be detected and studied thoroughly. Analysis of the cavernosograms revealed a sphincter of the deep dorsal vein of the penis, the dysfunction of which in 10 of the 11 patients was the cause of PVD; in 5 of these patients the dysfunction was combined with excessive development of the subcutaneous veins of the penis. In one patient PVD was caused by a large venous trunk which emptied into the ostium of the left major subcutaneous vein. The 11 patients underwent operation after which they returned to a normal sex life.

Most complete information on vascular components of erection can be obtained by using three methods: a pharmacological test for creation of artificial passive erection, roentgen-contrast arterio-cavernography on both sides, perfusion graded cavernometry and cavernography. Of 50 patients examined, 5 had no disorders of vascular components of erection. In 34 patients pathology of the arterial duct was revealed, 11 patients had a pathological venous drain. 46 operations were performed on the vessels. Satisfactory and good results were observed in 31 patients (68.8%) within 3 years after operation.

RESUMO

Chemodectoma of the neck is one of the rare tumors occurring in man. The paper presents an observation of cervical chemodectoma found in a patient aged 31 years. For preoperative differential diagnosis, carotid angiography, ultrasound and radioisotopic examinations were used, which allowed the tumor located at the bifurcation of the left carotid artery to be diagnosed. The material taken during the surgery was histologically and electron microscopically examined to verify benign chemodectoma (an alveolar variant).

RESUMO

Fibrinogen levels and aortic and pulmonary-artery blood pressure were measured in 143 patients with acquired heart diseases and pulmonary hypertension and in 60 control patients. The lungs were shown to participate in the regulation of fibrinogen levels both in the controls and patients with heart diseases, as reflected in the deposition-balance-mobilization phases by positive, zero, negative arterio-venous difference of fibrinogen levels. The association between the metabolic and hemodynamic functions of the lungs is particularly obvious, if pressure gradients (pulmonary-capillary and bronchial perfusion ones), making for blood inflow into the alveolar-capillary region, are examined.

RESUMO

Sixty essentially healthy subjects were examined manometrically with blood withdrawn from the coronary sinus, pulmonary artery, aorta, veins of the right kidney and the right liver lobe. Together with the traditional parameters of the gas contents and gradients of plasma, it is proposed to use tests measuring additive O2 and CO2 parameters. It has been demonstrated that the above organs can be discriminated using tests that are additive with respect to the gas pressure. It is emphasized that the tests proposed here, when used in addition to the traditional ones, allow identification of the gas and non-gas parameters of homeostasis as a single system of tests to assess the human body function.

Experiments on six healthy test volunteers, who underwent simultaneous catheterization of different cardiovascular compartments, were carried out to study the effect of head-down tilting at an angle of -20 degrees on the central and cerebral hemodynamics and metabolism. By the third hour of exposure, the test subjects showed a decrease in the systolic arterial pressure and increases in the systolic pressure of the right ventricle, heart rate, cardiac output, and oxygen intake. They also displayed a trend for an increase in the content of adenosine triphosphate, a decrease in the content of pyruvic acid, and essentially no changes in the concentration of lactic acid and activity of lactate dehydrogenase in the arterial blood and in the blood flowing out from the brain. The oxygen arteriovenous difference for the systemic and cerebral circulation decreased by 24% and 13%, respectively. The above circulatory and metabolic changes seem to reflect processes of adaptation of the human body to functioning under conditions of cephalad fluid shifts.
