RESUMO

PURPOSE: To investigate if and how the positions of condyles in centric relation (CR) varied from each other using different CR records. MATERIALS AND METHODS: Condylar positions in CR were repeatedly recorded by four licensed clinicians in 81 stomatognathically healthy, fully dentate subjects with six different types of CR records. The maxillary casts were mounted in articulators after individual facebow transfer, and the mandibular casts were mounted with a central bearing point (CBP) registration on the tip of the Gothic arch. Mean values from three registrations of the centric condylar positions with each of the six methods were calculated, and comparative calculations were made. RESULTS: The median spatial distances of the condylar positions between the CBP and all other CR records was 0.73 ± 0.24 mm (0 to 3.19 mm). The median spatial values between the condylar positions of the different CR records ranged from 0.48 to 0.79 mm. Statistically, the positions of almost all types of registration were slightly different from each other. CONCLUSION: Different CR records reproduce slightly different positions of the mandible, which is especially true for CBP registration, whereas reproducibilities and SDs between registrations are similar. Unrefined wax-wafer registrations using bimanual manipulation have proven particularly successful with regard to precision and time requirements, as has CBP registration with regard to condylar position.

BACKGROUND: The majority of complete dentures are still conventionally manufactured using a flask-and-pack technique. However, the polymerization process may introduce a distortion of the denture body. The aim of this study was to evaluate the three-dimensional fit of the posterior palatal seal of maxillary complete dentures with the original impression, and to give recommendations for scraping. METHODS: Four autopolymerising resins were used to manufacture 40 palatal plates each for high, medium and flat palates (total n = 120). The misfit was captured by taking a reline impression with a highly fluid silicone, the dimensions of which were measured with a flat-bed scanner. RESULTS: The shape of the palate had a significant impact (median p = 0.0435), but not the resin type (median p = 0.2575). It was largest for the flat palate and smallest for the high palate. The largest misfit was observed in the palatal midline area (flat-palate average median: 685 µm; high and medium palates: 620 µm) decreasing towards the lateral and anterior regions. CONCLUSIONS: The results suggest compensating for the palatal misfit that occurs with autopolymerising resins by scraping a postdam of an approximately 0.7 mm depth to the master cast, decreasing towards the anterior and lateral areas. In high and medium palates, the scraping could be less pronounced.

This study aimed to evaluate the biomechanical behaviour of adhesive endo-crowns and the influence of their design on the restoration prognosis when four loading positions are applied from the restoration-tooth junction. Two three-dimensional finite element models for the lower first molar were developed: endo-crown as a monobloc and endo-crown of a primary abutment and a full crown. Four crown loading positions were considered: 5, 6, 7 and 8 mm. A force of 1400 N was applied buccally on the middle of the mesiodistal width. No differences were observed for the two endo-crowns concerning restoration displacement and the distribution of equivalent von Mises stress and total equivalent strain. Shifting the position of the applied load to 8 mm resulted in an increase in the displacement from 25 to 42 µm and an increase of equivalent von Mises stress concentration at the tooth. The height of load application on the restoration has a significant role in the prognosis of endo-crowns.

INTRODUCTION AND AIM: The term osseoperception describes the capability of developing a subtle tactile sensibility over dental implants. The present clinical study aims at clarifying the question of how far tactile sensibility is to be attributed to the periodontium of the natural opposing tooth of the implant. MATERIAL AND METHOD: Thirty-two subjects with single-tooth implants with natural opposing teeth were included in this clinical, single-blind, split-mouth study. The natural antagonistic tooth of the implant and the corresponding natural contralateral tooth were anesthetized with a locally infiltrated articaine anesthetic. In a computer-assisted and randomized way, copper foils of varying thickness (0-100 µm) were placed interocclusally between the single-tooth implant and the natural opposing tooth, and between the contralateral pair of natural opposing teeth in order to investigate the active tactile sensibility according to the psychophysical method of constant stimuli and evaluate it statistically by the Weibull distribution. RESULTS: The average tactile sensibility of the implants with anesthetized antagonists at the 50% value calculated by means of the Weibull distribution was 20 ± 11 µm with a support area (90%-10% value) of 77 ± 89 µm. For the pair of natural teeth, the tactile sensibility at the 50% value was 16 ± 9 µm with a support area of 48.4 ± 93 µm. This resulted in an average intraindividual difference of 3.5 ± 7 µm at the 50% value and 29 ± 93 µm in the support area. The statistical calculations demonstrated an equivalent tactile sensibility (50% value) of the single-tooth implant and the contralateral natural control tooth with the natural antagonists being anesthetized in each case (double t-test, equivalence limit ± 8 µm, P < 0.01, power >80%). CONCLUSION: Apparently, the active tactile sensibility of single-tooth implants with natural opposing teeth is not only to be attributed to the periodontium of the opposing tooth but also to a perception over the implant itself. This could support the hypothesis according to which the implant may have a tactile sensibility of its own.

PURPOSE: The phenomenon of developing a certain tactile sensibility through osseointegrated dental implants is called osseoperception. Active tactile sensibility can be tested by having the subject bite on test bodies. The aim of the study was to describe the active tactile sensibility of single-tooth implants based on the 50% value and the slope of the sensibility curve at the 50% value. MATERIALS AND METHODS: Sixty-two subjects with single-tooth implants with natural opposing teeth were included in the study. In a computer-assisted and randomized way, copper foils of varying thickness (0 to 200 Μm) were placed inter?occlusally between the single-tooth implant and the natural opposing tooth, and the active tactile perception was studied according to the psychophysical method of constant stimuli and statistically evaluated by logistic regression. RESULTS: Tactile perception of the implants at the 50% value estimated by logistic regression was 20.2 ± 10.9 Μm on average, and the slope was 29 ± 15. Regarding implant surface structure, significant differences were observed. The sandblasted and acid-etched surface was significantly more sensitive than the titanium plasma-sprayed surface, and the machined surface was similar to the titanium plasma-sprayed surface. CONCLUSIONS: Active tactile sensibility of implants with natural antagonistic teeth is very similar to that of teeth, but the slope of the tactile sensibility curve is flatter. Significant differences in tactile sensibility as a function of different implant surfaces may indicate that receptors near the implant form the basis of osseoperception.

The purpose of this clinical trial was to determine the active tactile sensibility of natural teeth and to obtain a statistical analysis method fitting a psychometric function through the observed data points. On 68 complete dentulous test persons (34 males, 34 females, mean age 45.9 ± 16.1 years), one pair of healthy natural teeth each was tested: n = 24 anterior teeth and n = 44 posterior teeth. The computer-assisted, randomized measurement was done by having the subjects bite on thin copper foils of different thickness (5-200 µm) inserted between the teeth. The threshold of active tactile sensibility was defined by the 50% value of correct answers. Additionally, the gradient of the sensibility curve and the support area (90-10% value) as a description of the shape of the sensibility curve were calculated. For modeling the sensibility curve, symmetric and asymmetric functions were used. The mean sensibility threshold was 14.2 ± 12.1 µm. The older the subject, the higher the tactile threshold (r = 0.42, p = 0.0006). The support area was 41.8 ± 43.3 µm. The higher the 50% threshold, the smaller the gradient of the curve and the larger the support area. The curves showing the active tactile sensibility of natural teeth demonstrate a tendency towards asymmetry, so that the active tactile sensibility of natural teeth can mathematically best be described by using the asymmetric Weibull function.

RESUMO

The aim of this research was to study the impact of loading on partial dentures within the supporting soft tissue with respect to different attachment techniques. A finite element model was developed to calculate the stress and strain distribution in this tissue. The model consisted of the left half of a mandible with three remaining teeth that had suffered an atrophy in the anterior region, and a partial denture over the toothless area that was connected at the left mandibular canine using an attachment system. Resulting stress/strain distributions are presented for different load cases using a commercially available prefabricated attachment system.

OBJECTIVE: To quantify wear processes by measuring the retention force changes and the fitting tolerance at different prefabricated attachment systems to estimate the wear constancy and applicability in clinical practice. METHOD AND MATERIALS: Seven prefabricated attachment systems (Dalbo-Classic, Dalbo-PLUS, Dalbo-Z, Mini-Gerber-PLUS, Stufenexzenter, SpharoLock, and Degussa-Kugelankersystem) with different construction and alloy composition were tested. Twenty samples of each system were subjected to 10,000 insertion-separation cycles in a wear simulator with a periodontium-simulating specimen holder. The simulator was designed to record the force needed to insert and to separate the attachment and the distance moved during the insertion and separation cycles. RESULTS: All types of anchors showed wear that led to a loss or to an increase in retention force at the beginning of the wear simulation. Anchors with a plastic retention insert showed the slightest changes in retention force. The wear does not have an effect on the fitting tolerance. CONCLUSION: Anchor systems that possess an adjustable matrix should be preferred. They allow decreasing or increasing the retention force of an anchor if this force is changed by wear. There is no risk of a loss of support if forces in occlusal direction are exerted because there is no clinically relevant change in the fitting tolerance.

AIM: The purpose of this randomized split-mouth clinical trial was to determine the active tactile sensibility between single-tooth implants and opposing natural teeth and to compare it with the tactile sensibility of pairs of natural teeth on the contralateral side in the same mouth (intraindividual comparison). MATERIAL AND METHODS: The hypothesis was that the active tactile sensibilities of the implant side and control side are equivalent. Sixty two subjects (n=36 from Bonn, n=26 from Bern) with single-tooth implants (22 anterior and 40 posterior dental implants) were asked to bite on narrow copper foil strips varying in thickness (5-200 microm) and to decide whether or not they were able to identify a foreign body between their teeth. Active tactile sensibility was defined as the 50% threshold of correct answers estimated by means of the Weibull distribution. RESULTS: The results obtained for the interocclusal perception sensibility differed between subjects far more than they differed between natural teeth and implants in the same individual [implant/natural tooth: 16.7+/-11.3 microm (0.6-53.1 microm); natural tooth/natural tooth: 14.3+/-10.6 microm (0.5-68.2 microm)]. The intraindividual differences only amounted to a mean value of 2.4+/-9.4 microm (-15.1 to 27.5 microm). The result of our statistical calculations showed that the active tactile sensibility of single-tooth implants, both in the anterior and posterior region of the mouth, in combination with a natural opposing tooth is similar to that of pairs of opposing natural teeth (double t-test, equivalence margin: +/-8 microm, P<0.001, power >80%). Hence, the implants could be integrated in the stomatognathic control circuit.
