RESUMO

The children hypertension is a serious threat to population health because uncontrolled children hypertension can result in exponential increasing of cardiovascular and cerebrovascular diseases, progression of kidney failure and premature death in adulthood. The main risk factors for primary children and adolescent hypertension are considered as cardiovascular biomarkers (fibro-blast growth), obesity, especially central obesity, overweight, low birth weight, high sodium intake, sedentary lifestyle, lack of physical activity, socioeconomic and ethnic factors, as well as family anamnesis. The special place in studying children hypertension is occupied by problem of neonatal hypertension. The most common causes of neonatal hypertension are umbilical catheter-related thrombosis, renal parenchymal disease and chronic lung disease (bronchopulmonary dysplasia), as well as low birth weight, post-menstrual age and some other maternal factors, as well as medications (inotropic medicines, caffeine) and regulation of fluid intake.

The COVID-19 pandemic has led to changes in all medical and demographic indicators, including changes in the incidence of the population. The article provides an analysis of the age characteristics of the primary morbidity of the population in the conditions of the COVID-19 pandemic. A pattern has been determined: the highest rates of primary morbidity in the conditions of the COVID-19 pandemic (for 2020) were determined in the age group older than working age. It was in this group that the highest incidence rates of COVID-19 were established. On the contrary, a comparative analysis of the primary morbidity of children (0-14 years old) in Russia as a whole in the «pre-pandemic¼ year (2019) and in the conditions of the COVID-19 pandemic (2020) showed a decrease in the level of this indicator by 13.9% (from 172455.1 to 148537.7 per 100 thousand population, р < 0.05). Among the child population, 1208.3‱ cases of COVID-19 were registered. Thus, in the Central Federal District, the indicators of primary morbidity of children were lower than the general.

Presented herein is a review of scientific publications dedicated to studying the pharmacodynamics of L-arginine and possibilities of its clinical application. Interest to L-arginine is associated, first of all, with its role as a precursor in endogenous synthesis of nitric oxide (NO), playing an important role in regulation of the functional state of the vascular wall. According to numerous studies, oral and parenteral administration of L-arginine restores endothelial production of NO in such diseases as atherosclerosis, hypertension, type 2 diabetes mellitus, obliterating diseases of arteries of lower extremities. The NO-mediated effect of L-arginine manifests itself in increasing the capability of vessels to dilatation, decreasing blood platelet aggregation, and inhibiting proliferation of smooth muscle cells of vessels. The effect is most pronounced in patients presenting with hypercholesterolaemia and initially decreased reactivity of the blood channel. The mostly pronounced NO-mediated effect of L-arginine is observed in parenteral route of its administration. Prolonged administration of L-arginine slows down progression of atherosclerosis.

The influence of angioprotector and endothelium-protector drugs pentoxifylline and unifuzol as components of supportive therapy on the efficacy of combined cytostatic treatment has been experimentally studied. It is established that pentoxifylline and unifuzol do not affect the antitumor and antimetastatic activity of doxorubicin and cyclophosphan with respect to Pliss lymphosarcoma and Walker 256 carcinosarcoma, and in some cases even potentiate the effect of cytostatic therapy.

Specific pharmacologic activity of sodium-L-arginine succinate (unifusol) was studied on endothelium dysfunction model (EDM) in rats. EDM was induced by daily administration of N-nitro-L-arginine methyl ether (L-NAME). The effectiveness of experimental therapy with unifusol was assessed by changes in the arterial pressure level, duration of endothelium-dependent and -independent vasodilation, and the blood concentration of endothelial dysfunction markers including VEGF, NO, endothelin-I and the number of desquamated endotheliocytes. Administration of unifusol favors correction of blood vessel endothelium state manifested by normalization of its functional activity and reduction of the apoptosis of endotheliocytes. In addition, the obtained results unambiguously confirm considerable vasodilating and antihypertensive effects of unifusol.

RESUMO

The purpose: to ascertain possible cardioprotective effect of Sodium-L-arginine Succinate in case of myocardial lesions caused by Doxorubicin in rats. Methods. The effectiveness of the drug was assessed by morphometric and biochemical changes reflecting peroxidation intensity and structural and functional changes in myocardium. Results. Sodium-L-arginine Succinate used for the treatment and prophylaxis of antracyclic cardiomyopathy was demonstrated to decrease Doxorubicin effects lessening cardiomyocytal destruction and the left ventricular dysfunction with myocardial hypertrophy.

RESUMO

The published data on the modern concept of the biological role of succinate, an intermediate of the citric acid cycle are analysed in the review. Special interest to succinate is determined by investigations on the mitochondrial functions at different pathologies, discovery of the hypoxia-inducible factor HIF-1 and studies on the human genome, that resulted in detection of the G-protein coupled receptors, which selectively are bound with succinate. According to the published experimental data, besides participation in oxidative reactions, succinate is considered as a key contributor to physiological, metabolic and genetic processes.
