OBJECTIVES: To evaluate the effects of treatment of bilateral posterior crossbite (BPXB) on mandibular kinematics by the percentage of reverse chewing cycles (RCCs) during soft and hard bolus chewing before and after the correction of the malocclusion with function-generating bite (FGB). MATERIALS AND METHODS: This prospective study included 71 subjects: 19 patients with occlusally symmetric BPXB (M = 9; F = 10; mean age 9.3 ± 2.2[yr.mo]), 32 patients with occlusally asymmetric BPXB (19 with more teeth in crossbite on the right side (right prevalent side), M = 7; F = 12; mean age 8.2 ± 1.6 [yr.mo] and 13 on the left side, M = 7; F = 6; mean age 9.6 ± 1.9 [yr.mo]) and 20 controls without malocclusion (M = 8; F = 12; mean age 10.2 ± 1.7 [yr.mo]). Masticatory patterns were recorded before (T0) and after (T1) the correction of the malocclusion with FGB, with the K7-I® kinesiograph using standardized soft and hard boluses. RESULTS: BPXB was corrected in all included patients. At T0, the percentage of RCCs in BPXB was significantly increased compared to controls (P < .0001); symmetric BPXB showed no difference in RCCs between the sides, whereas asymmetric BPXB showed significantly more RCCs on the side with more teeth in crossbite (prevalent side). After treatment with FGB (T1), the percentage of RCCs was significantly reduced in both symmetric BPXB patients (soft bolus, P = .003; hard bolus, P < .001) and asymmetric BPXB patients (prevalent side: soft and hard bolus, P < .00001; non-prevalent side: soft bolus, P = .01 and hard bolus, P = .0002). CONCLUSION: Functional correction of BPXB with FGB significantly improved mandibular kinematics during chewing.

OBJECTIVES: To evaluate the effects of treatment of posterior crossbite (PXB) in the mixed dentition with the Function Generating Bite (FGB) appliance on the transverse dimension of the dental arches. MATERIALS AND METHODS: This study included 84 PXB patients (female = 46; male = 38; mean age, 8.2 ± 1.8 years) and 69 control (C) patients (female = 31; male = 38; mean age, 8.9 ± 1.4 years). Measurements were taken with digital calipers on maxillary and mandibular study casts before (T0) and after (T1) treatment for the following measures: intermolar (IMD), intermolar gingival (IMGD), intercanine (ICD), and intercanine gingival distances (ICGD). RESULTS: At T0, there was a significant difference in all maxillary measurements between the PXB and C groups (P < .001), reflecting maxillary hypoplasia in PXB patients. At T1, there was no difference between the groups. In PXB patients, the mean increase between T0 and T1 for IMD was 4.34 ± 2.42 mm; this difference measured 3.51 ± 2.19 mm for IMGD, 2.78 ± 2.37 mm for ICS, and 1.89 ± 1.7 mm for ICGD. There was no significant difference in mandibular measurements between groups at T0 and T1. CONCLUSIONS: Functional therapy with FGB is effective in significantly increasing the transverse dimension of the maxillary dental arch in PXB patients. Considering its efficacy in treating masticatory dysfunction, FGB may be considered a good treatment option for the correction of PXB in growing children.

OBJECTIVE: To evaluate reverse-sequencing chewing cycles (RSCC) and their kinematic parameters on both sides before and after correction with the Function Generating Bite (FGB) appliance. MATERIALS AND METHODS: Forty-seven patients, 8.3 ± 1.1 (mean ± SD) years of age, with unilateral posterior crossbite (35 on the right side, 12 on the left side) and 18 age-matched controls (9.1 ± 0.8 years) were selected for the study from the orthodontic division of the University of Turin, Italy. The crossbite was corrected in all patients using FGB, and mandibular motion was recorded with a kinesiograph K-7 (Myotronics, Tukwila, Wash), during chewing on both sides of a soft and a hard bolus before and after correction. RESULTS: After correction, the percentage of RSCC significantly decreased for soft and hard (P < .001) boluses and fell within the normal range for 75% of the patients. The indices of the chewing pattern (closure angle, axis, maximum lateral excursion) significantly improved (P < .001), becoming symmetric between sides. CONCLUSIONS: The results of this study showed that the functional appliance, FGB, was able to lower the percentage of RSCC significantly, bringing them back to the normal range in 75% of cases, and to improve the kinematic parameters that become symmetric between sides.

OBJECTIVE: To describe the effects of therapy on masseter activity and chewing kinematic in patients with unilateral posterior crossbite (UPC). DESIGN: Fifty children (age: mean ± SD: 9.1 ± 2.3 years) with UPC (34 on the right side, 16 on the left side) and twenty children (age: 9.5 ± 2.6 years) with normal occlusion were selected for the study. The mandibular motion and the muscular activity during chewing soft and hard boli were simultaneously recorded, before and after correction with function generating bite, after a mean treatment time of 7.3 ± 2.4 months plus the retention time of 5-6 months. The percentage of reverse cycles and the percent difference between ipsilateral and contralateral peaks of the masseter electromyography envelopes were computed. RESULTS: Before therapy, the percentage of reverse cycles during chewing on the crossbite side was greater in patients than in controls (P<0.001) and significantly reduced after therapy (P<0.001) towards the reference normal value (soft bolus; pre: 57 ± 30%, post:12 ± 17%; hard bolus; pre: 65 ± 34%, post: 12 ± 13%; reference value: soft bolus 4 ± 2%, hard bolus 5 ± 3%). Before therapy the percent difference between electromyography envelope peaks in patients was lower than in controls (P<0.01) and significantly increased after therapy (P<0.05) becoming similar to the reference normal value. CONCLUSIONS: The correction induced a normal-like coordination of masseter muscles activity together with a significant reduction of the reverse chewing patterns. The previous altered muscular activation corresponded to the altered kinematics of reverse chewing cycles that might be considered a useful indicator of the severity of the masticatory function involvement.

It is well established that patients with a unilateral posterior crossbite exhibit reverse-sequencing chewing patterns when chewing on the affected side. The aim of the study was to compare the prevalence of reverse-sequencing chewing cycles in patients with anterior versus posterior unilateral crossbite during chewing soft and hard boluses. Eighty-six children (39 boys, 47 girls) were included in the study: 26 (10.4 ± 2.7 years) with unilateral anterior crossbite, 43 (10.2 ± 4.2 years) with unilateral posterior crossbite, and 17 (10.6 ± 2 years) with normal occlusion were selected for the study. Mandibular movements were measured with a kinesiograph (K7, Myotronics Inc. Tukwila). The kinematic signals were analyzed using custom-made software. The results showed a low prevalence of reverse-sequencing chewing cycles in patients with anterior crossbite, without any significant difference between sides and with the control group, with both soft (P = 0.33) and hard (P = 0.29) bolus. The patients with posterior unilateral crossbite showed a significant higher prevalence of reverse-sequencing chewing cycles during chewing on the crossbite side with respect to the non-crossbite side (P < 0.001) and to the control group (P < 0.001). Comparing the patients with anterior versus posterior unilateral crossbite, a significant difference (P < 0.001) in the prevalence of reverse chewing cycles was demonstrated during chewing on the posterior crossbite side only with both soft and hard bolus. In conclusion, patients with anterior versus posterior unilateral crossbite show different functional characteristics depending on which dental region is involved.

OBJECTIVES: It is well established that patients with a unilateral posterior crossbite, when chewing on the affected side, show an increased frequency of reverse chewing cycles. It was hypothesized that the correction of reverse cycles may be due to the characteristics of the therapy. The aim was to investigate the prevalence of reverse chewing patterns in children with unilateral posterior crossbite before and after treatment with Function Generating Bite (FGB). MATERIALS AND METHODS: Twenty children, (9 boys, 11 girls; age, mean ± SD, 7.5 ± 1.1), 10 with a right and 10 with a left posterior unilateral crossbite were selected. Mandibular movements during chewing soft and hard boluses were measured with a kinesiograph (K7 -I, Myotronics Inc. Tukwila, Washington, USA). RESULTS: The results showed a significant difference when comparing the percentage of reverse chewing patterns, before and after therapy with FGB, during chewing on the crossbite side both with soft and hard bolus (p<0.0001). No significant differences were observed during chewing on the non-crossbite side. DISCUSSION: The results of this study confirmed that FGB corrects both the dental and functional asymmetries. Knowing that the rapid palatal expansion does not correct the masticatory function, it is of clinical relevance, for the orthodontists, the knowledge and the understanding of the functional outcomes with different therapies. CONCLUSIONS: The type of treatment and the biomechanics of the appliance used are of great importance for the correction of the reverse chewing cycles and for rebalancing the functional asymmetry of children with unilateral posterior crossbite.

The aim of the study was to evaluate the effect of bolus hardness on the kinematic of mastication and jaw-elevator muscle activity in subjects with normal dental occlusion and function. The mandibular motion and the surface EMG envelope of the masseter and temporalis anterior muscles were assessed in twelve subjects during mastication of a soft and hard bolus of the same size. When chewing the hard bolus, the chewing pattern in the frontal plane was significantly higher and wider, with smaller closure angle and higher peak velocity than when chewing the soft bolus. EMG peak amplitude of both the masseter and anterior temporalis muscles was higher for the side of the bolus but the contralateral side increased its activity significantly more than the ipsilateral side when the hardness of the bolus increased (for the masseter, mean+/-SD: 130.4+/-108.1% increase for the contralateral side and 29.6+/-26.9% for the ipsilateral side). Moreover, the peak EMG activity for both muscles occurred more distant from the closure point with hard bolus. The increased activity of the contralateral side may help maintaining the mandibular equilibrium, with indirect participation to the power stroke generated by the chewing-side masseter. The results provide kinematic and EMG adaptations to bolus hardness in healthy subjects and can be used as normative data in the development of methods for early diagnosis of impaired chewing function.
