ABSTRACT This article proposes two methodologies for the detection of lesions in the retina, which may indicate the presence of diabetic retinopathy (DR). Through the use of digital image processing techniques, it is possible to isolate the pixels that correspond to a lesion of RD, to achieve segmenting microaneurysms, the edges of the objects contained in the image are highlighted in order to detect the contours of the objects to select by size those that meet an area of 15 to 25 pixels in the case of 512x512 images and identify the objects as possible microaneurysms, while for the detection of exudates the green channel is selected to contrast the luminous objects in the retinography and from the conversion to gray scale, a histogram is graphed to identify the ideal threshold for the segmentation of the pixels that belong to the exudates at the end of the optical disk previously identified by a specialist. A confusion matrix supervised by an ophthalmologist was created to quantify the results obtained by the two methodologies, obtaining a specificity of 0.94 and a sensitivity of 0.97, values that are outstanding to proceed with the classification stage.


RESUMEN Este artículo propone dos metodologías para la detección de lesiones en la retina, que pueden significar la presencia de retinopatía diabética (RD). Mediante el uso de técnicas de procesamiento de imágenes digitales se logra aislar los pixeles que corresponden a una lesión propia de RD, para lograr segmentar microaneurismas se resaltan los bordes de los objetos contenido en la imagen con la finalidad de detectar los contornos de los objetos para seleccionar por tamaño los que cumplan con un área de 15 a 25 pixeles en el caso de imágenes de 512x512 y se identifiquen los objetos como posibles microaneurismas, mientras que para la detección de exudados se selecciona el canal verde para contrastar los objetos luminosos en la retinografía y a partir de la conversión a escala de grises se grafica un histograma para identificar el umbral idóneo para la segmentación de los pixeles que pertenecen a los exudados al final eliminar el disco óptico previamente identificado por un especialista. Se creó una matriz de confusión supervisada por un oftalmólogo para cuantificar los resultados obtenidos por las dos metodologías obteniendo una especificidad del 0.94 y una sensibilidad del 0.97, unos valores que son sobresalientes para proceder con la etapa de clasificación.

Fractures of the proximal femur are, more than ever, an important challenge in the field of traumatology. The Gamma nail, a combination of advantages of the sliding screw with the intramedullary nail, represents an efficient technique in the management of these fractures. A series of 224 fractures of the proximal femur in which this nail was used is reported. The average age of the patients was 79.2 years. The mean healing time was 68.2 days in 99.4% of the cases. The incidence of perioperative complications was 10.3% showing that, in most of the cases, the complications occurred because of poor technique. Postoperative complications occurred with an incidence of 14.1%. Seven cases of migration of a proximal screw, six shaft fractures, and one broken nail were the most important complications. The device allowed for early mobilization and full weightbearing of the affected hip regardless of the type of fracture. With adequate surgical technique and experience, the advantages of the Gamma nail increases as the complication rate diminishes.

BACKGROUND: Permanent central steep islands are an undesirable phenomenon that cause distorted images and a significant reduction in visual acuity. We describe treatment of central steep islands with repeat excimer laser photoablation in the central cornea. METHODS: Three patients with preoperative refractions of -7.50 -2.50 x 170 degrees D (right eye), -8.00 -2.25 x 10 degrees D (right eye) and -6.00 -1.50 x 90 degrees (right eye) developed central steep islands which persisted more than 12 months. All patients lost more than two lines of spectacle-corrected visual acuity and complained of visual disturbances. We retreated the central steep islands with a VISX 20/20 excimer laser PRK ablation that matched the size of the central island measured on videokeratography. RESULTS: Symptomatic glare and distortion were significantly reduced and the central steep islands were resolved. Several weeks after reablation, spectacle-corrected visual acuity improved to the preoperative level. CONCLUSION: Central steep island, an infrequent complication of excimer laser photorefractive keratectomy, can be safely removed with a repeat laser ablation that matches the central circular steep area.

RESUMO

Early posttraumatic arrest of the distal radial physis is an uncommon complication and usually appears after type II physeal injury. We report a 13-year-old boy who sustained an injury to his right wrist, incurring an avulsion fracture of ulnar styloid process. Twenty-one months later, a uniform closure of the distal radial physis was observed. An ulnar shortening osteotomy and distal ulnar epiphysiodesis were performed. We discuss different factors of this complication, but crush injury of germinal cells of the growthplate is considered the main etiology.

RESUMO

A retrospective study of retinoblastoma was done during 1978-1987, analysing the clinics aspects of the entity. We found nineteen children during this period, from them eight were girls (42%) and eleven were boys (58%). Ages of diagnostic ranged between three months and four years, with and average of 17.53 months. Nine children (47%) were bilaterally affected. Leocokoria was the most common initial sign, followed by strabismus. Ophthalmoscopy was the diagnostic procedure used, and surgery the recommended therapeutic, using radiotherapy as a palliative method in the cases which surgery has no indications.

The influences of the maternal diet and of the mothers' nutritional and socioeconomic status on the fatty acid composition of human milk have not yet been fully elucidated. Fatty acids from capric (8:0) to docosahexaenoic (22:6w3) acids were determined in 209 samples of human milk obtained from voluntary donors. Samples were classified according to the time of lactation and in relation to the socioeconomic status and to the weight of the lactating women. Capric and lauric acid increased in mature milk while palmitic and stearic acids decreased. Polyunsaturated fatty acids (PUFA) of the w6 and w3 series with more than 18 carbon atoms also decreased from colostrum to mature milk. However, the ratio w3 PUFA/w6 PUFA remained unaltered. Oleic acid decreased in milk from mothers of a medium and low socioeconomic status who consumed almost exclusively seed vegetable oils. The mother's weight did not influence the fatty acid composition of her milk.
