RESUMO

The modification of the reconstructive stage of gastropancreatoduodenal resection aims to increase the security of the pancreatojejunoanastomosis by minimizing the impact of such aggressive substances as bile and pancreatic juice. The modification represents the isolated pancreatojejunoanastomosis on the Roux-en-Y intestinal loop and gastro- and hepaticojejunoanastomoses on the second intestinal loop, separated with the use of the stub. Thus, the method allows the separate passage of pancreatic juice, bile and gastric contents, excluding their impact on other anastomoses. The described modification was performed in 6 patients. There were no cases of the anastomotic insufficiency. The mean hospital stay was 10,5 days. Thus. The method proved to be effective and safe, providing good initial results.

RESUMO

Complex hemodynamical investigations were conducted in 1985-2004 yrs in 60 patients with hepatic diseases before the operation, in 6-8 and 12-24 months after performance of portosystemic shunting operation (in 26 patients was formed H-like mesentericocaval anastomosis, in 10--central splenorenal anastomosis, in 14--splenorenal anastomosis side-to-side, in 10--H-like splenorenal anastomosis). Performance of shunting operation had promoted the lowering of volumetric speed of blood flow and pressure in portal vein, her diameter reduction, the general hepatic blood flow lowering. The lowering of blood flow in the portal vein system after the shunting have caused enhancement of arterial hepatic blood flow.

RESUMO

Experience of surgical treatment of 271 patients the extrahepatic biliary ducts tumor for the 1992-1999 yrs period is presented. Indirect signs of extrahepatic biliary ducts tumor were revealed in 84% of observations. Depending on the tumor localization the trustworthiness of the endoscopic retrograde pancreatocholangiography method had constituted from 79.8 to 96.4%. Correct diagnosis was established before the operation in 94.3% of patients. Radical operation was done in 93 (34.3%) of patients, including 22 with proximal localization of tumor, 13--with central one, 10--distal, 48--terminal. Palliative operation was performed in 178 patients, in 76 of them biliodigestive anastomosis was done. Total postoperative mortality was 14.8%.

RESUMO

Ten-year experience of operative treatment of cholelithic disease using mini-access cholecystectomy (MACH) in 920 patients was summarized. There were operated on 167 patients for an acute cholecystitis, chronic calculous cholecystitis--716, gall bladder polyposis--36, gall bladder cancer--1. In 48 patients the MACH was matched with choledochus drainage, in 9--choledocholithotomy was performed, in 1--choledochoduodenostomy. In 6 observations the MACH were done simultaneously inguinal herniotomy, in 10--umbilical herniotomy, in 7--uterine extirpation, in 13--operation for ovarial cyst. The bile leakage from gall bladder bed occurred in 3 observations, the operative wound suppuration--in 4. Duration of stationary treatment was 4.1 days at average.

In order to study gender differences in development of the latent inhibition (LI) and the influence of laterality (hemisphere) factor, two groups of subjects were examined. In the preexposure phase, subjects of the control group were presented with a pair of geometric figures, which appeared either in the right or in the left visual hemifield (at random) with the exposure time of 80 ms. A subject had to compare the figures in a pair and to press a button in case of their identity. In the preexposure (experimental) group, the subject solved the same task, but half of them was presented with the same set of stimuli which appeared in the right hemifield, another half of subjects received these stimuli in the left hemifield. Simultaneously with the target stimuli, the opposite hemifield was stimulated with a nonattended symbol "[symbol: see text]". In the test phase, all the subjects (both of the control and experimental groups) had to respond to the symbol "[symbol: see text]" (previously nonattended in the experimental group), which appeared in part of the trials, by pressing the button. Pairs of stimuli which had been target ones in the preexposure phase, became masking stimuli in the test phase. Correct responses were reinforced by presentation of a special sound. The number of trials required for acquisition of the instrumental contingency served as an index of the LI. Analysis of variance showed that significant strong effect of the LI was observed only in men (comparison between the control and preexposure groups), while there was no significant difference between women of the control and preexposure groups. There was no laterality effect. Gender differences and hemispheric asymmetry in the LI organization are discussed.
