[Methods to prevent generalized peritonitis in peptic ulcer surgery]. / Metody profilaktiki generalizatsii peritonita v khirurgii iazvennoi bolezni.

Different variants of omentoduodeno- and omentogastroplasty were applied in 69 patients during performance of gastric resection, perforative ulcer excision for the peritonitis dissemination prophylaxis. In 18 patients was used the polychlorvinyl drain, newly constructed by the authors. In 124 patients gastric resection according to Haberer with an oblique gastroduodenoanastomosis formation, the Treitz ligament dissection and duodenojejunal flexure descending were performed for the peritonitis dissemination prophylaxis.

[Tubular gastrectomy with selective vagotomy in the treatment of peptic ulcer]. / Trubchataia rezektsiia zheludka s selektivnoi vagotomiei v lechenii gastroduodenal'nykh iazv.

The authors generalize the material on the surgical treatment of 151 patients who were operated on for complicated gastroduodenal ulcers by means of an improved method of tubular resection of the stomach with selective vagotomy, subtotal excision of the lesser curvature, and the formation of a ++post-colonic rectangular gastroenteroanastomosis on a short loop with oblique division of the jejunum. The late-term results were studied in follow-up periods of up to 4 years in 110 patients (72.8%) and appraised according to Visik's scale. The results were excellent in 78 (70.9%), good in 25 (22.7%), satisfactory in 6 (5.5%) patients, and poor (recurrence of the disease) in one (0.94) patient.

[Surgical treatment of complicated gastroduodenal ulcers]. / Khirurgicheskoe lechenie oslozhnennykh gastroduodena'lnykh iazv.

Results of surgical treatment of complicated forms of gastroduodenal ulcers in 225 patients were summed up. The patients were operated upon using different variants of gastric resection. Results of the surgical treatment were followed-up during the period from 2-3 months up to 3-5 years. Promising results of the operative treatment of patients with complicated forms of gastroduodenal ulcers using tubular resections of the stomach allow to widely recommend them for surgical practice.

[A method of tubular resection of the stomach with formation of a rectangular oblique gastro-jejunal anastomosis]. / Metodika trubchatoi rezektsii zheludka s formirovaniem priamougol'nogo kosogo gastroeiunal'nogo soust'ia.

The method of tubular resection of the stomach with deomentization, subtotal resection of the lesser curvature and formation of the retrocolic right-angled gastro-jejunal anastomosis on a short loop with an oblique section of the jejunum was developed experimentally and successfully used in 98 patients for the treatment of complicated forms of gastroduodenal ulcers and of oncological patients with the tumorous involvement of the outlet portion of the stomach.

[Ways to increase the competence of sutures of the duodenal stump in the surgical treatment of complicated gastroduodenal ulcers]. / Puti povysheniia nadezhnosti shvov duodenal'noi kul'ti pri operativnom lechenii oslozhnennykh gastroduodenal'nykh iazv.

The authors have made an analysis of their experience with the treatment of 119 patients who had tubular resections of the stomach with the anastomozed gastric stump to the side with the obliquely dissected short loop of the jejunum. Specific methods of treatment of a "difficult" duodenal stump and measures to prevent its incompetence are described. In order to increase the reliability of the duodenal stump sutures a complex of measures directed to the elimination of duodenal hypertension and adequate drainage of the periduodenal zone is to be used.

[Secondary stomach ulcers after organ-sparing operations in duodenal ulcer]. / Vtorichnye iazvy zheludka posle organosokhraniaiushchikh operatsii pri iazvennoi bolezni dvenadtsatiperstnoi kishki.

Eight cases of ulcer of the vagotomized stomach are described which appeared at different terms after organ-preserving operations. Resections of the stomach were fulfilled to all the patients. The main role in the appearance of secondary ulcers is attached to evacuatory disorders. The authors consider the operation of choice in ulcer stenosis of the duodenum to be tubular antrumectomy with subtotal dissection of the lesser curvature and reestablishment of the direct gastroduodenal continuity. The operations on 180 patients gave good immediate and long-term results.

[Method of preventing incompetent sutures of the duodenal stump after resections of the stomach]. / Sposob profilaktiki nedostatochnosti shvov duodenal'noi kul'ti posle rezektsii zheludka.

The authors have developed and successfully used in the clinic an original method of omentoduodenoplasty for prophylactics of incompetence of duodenal stump sutures. Its essence is to form an omental muff-channel between the external part of the sutured duodenal stump cupula and the peritoneum of the frontal abdominal wall with a temporary communication of the omentum channel cavity with the skin of the frontal abdominal wall by means of a drainage tube.

[Gastric esophagoplasty in cancer of the middle third of the esophagus]. / Zheludochnaia ezofagoplastika pri rake srednei treti pishchevoda.

The examination of 119 corpses, experiments in 67 dogs and clinical management of 127 patients with the II-III degrees of cancer of the middle third of the esophagus have supported new methods of esophagoplasty by a tube from the greater curvature of the stomach with highly germetic sutures, longer transplants and diaphragmo-percutaneous conduction of it onto the neck. The method is shown to be more safe, less traumatic, not followed by functional alterations of the heart, lungs, diaphragm.
