RESUMO

It is pointed out that distinctive features of malignant neoplasms of the female genital system determine peculiarity of organization of medical care for patients with cancer of uterus, ovary and external genital organs. Violation of the basic principle, i.e., examination and treatment of oncogynecologic patients should be carried out only in specialized departments, mostly is the cause of the main defects of the above kind of medical care (late diagnosis, inadequate treatment). The number of oncogynecologic departments and their bed fund should be planned on the basis of malignant neoplasm morbidity rates among the female residents of the given territory.

RESUMO

A comparative study of 84 cases of endometrial and ovarian carcinoma showed clinical course of endometroid tumor to be more favorable than that of non-endometroid one (five-year survival--72.5 and 8.2%, respectively).

RESUMO

An analysis of a more than 40-year experience of management of stage Ib, T1bN0M0 cervical cancer (611 cases) followed the development of combined treatment of the disease: Until 1970-ies, irradiation plus surgery was preferred. During the seventies no correlation between effectiveness and sequence of these component procedures was established. According to data on 33 surgical cases, extended extirpation of the uterus may be recommended in patients with stage Ib cervical cancer less than 5 mm deep and free from such adverse prognostic factors as concomitant pregnancy, poor cell differentiation and blood and lymph vascular microembolism by tumor.

RESUMO

The paper discussed the results of organ-saving treatment carried out in 388 cases of cancer of the cervix uteri (Ca in situ), early endometrial carcinoma and stage IIa tumors of the ovary. The choice of the procedure should be made on reliable evidence on tumor extension and morphology. Favorable end results and preservation of reproductive function justify the use of said treatment and opens up new vistas in research.

RESUMO

The results of a clinical study of 42 cases of synchronous endometrial and ovarian tumors are discussed. Clinical manifestations of synchronous cancer were found to be similar to those of endometrial carcinoma. Postmenopausal patients showed a favorable course of tumor process, thus suggesting its primary multiple origin, slightly more frequently than reproductive or climacteric females. As far as the findings on the distant end results went, multiple lesions should not be regarded as an indisputably unfavorable factor of prognosis.

RESUMO

Application of 17-oxyprogesterone-capronate as a component of complex treatment of 188 patients with stage I-III carcinoma of corpus uteri improved the long-term results at all stages of the process. Hormonotherapy should be recommended as a method of choice in the postoperative treatment of patients suffering from stage I-II endometrial carcinoma with superficial infiltration of the muscular layer or as an independent method of treatment in elderly patients in case of contraindications for surgery and radiation procedures. The method offers good advantages in young patients at the initial stages only.

The many-year experience with the treatment of cervical cancer in the gynecological clinic of the P. A. Herzen Research Institute of Oncology has indicated that a combined method in case of the appropriate indications provides for high and persistant late results. The five-year recovery of patients at all stages was noted in 78.7% (I stage--87.3%; II stage--71.6%; III stage--41.7%). Improvement of therapeutic and diagnostic methods, strict adherence to the principles of a differential approach to selecting the type of therapy enabled increasing the per cent of a five-year cure from 68.9%' (1945--46) to 91.8% in 1965--1969. The clinical features of microinvasive cervical cancer (stage Ia) made it possible to reduce the extent of the treatment applied on account of excluding a radiation component and lessening the extent of surgical intervention. A five-year cure by the surgical method in patients with cervical cancer of stage Ia made up 97%. The results of the combination therapy in patients with stage Ib within the same period yielded 94%.

The results of the complex clinico-morphological study of the effect of 17-a-hydroxyprogesterone-caproate are reported in 71 patients aged from 37 to 75. Stage I was noted in 17 patients, stage II--in 51, stage III--in 3. A total dosage was 5,0; 10,0; 20,0 and 30,0 for the course of treatment. Following the hormonal therapy all these patients were subjected to surgery. To study the hormonal pathomorphism specimens from the uterine cavity were assayed histologically and cytologically prior to and during the hormonal therapy, the removed specimen was examined after the operation. The clinico-morphological studies indicated that 17 GPC shows the antitumor effect in endometrial cancer, so the complete tumor regression seems to be possible. The antitumor effect is mostly pronounced at initial stages of the disease and with high-differentiated tumors. The data obtained may be used to solve the problem on the purposeful application of 17 GPC either in the complex of therapeutic measures or as a separate method of treatment.

RESUMO

Uterine aspiration specimens from 172 females were examined cytologically (cancer in 89, non-cancer process -- in 71, in 12 cases aspiration specimens were obtained in a reproductive period in proliferative and secretory phases of the menstrual cycle). It was shown that in a group of histologically verified observations in 45.8% a gynecologist failed to recognize the true endometrial affection. Some cytological criteria elaborated are suggested (dominant, adjuvant and temporary), which combination helped to precisely diagnose cancer cytologically in 93.2%, while various forms of hyperplasia -- in 89.9%.
