RESUMO

Open noncomparative multicenter study of tolerability and efficacy of a retard form of indapamide (1.5 mg o.d. for 3 months) in patients older then 55 years was conducted in 14 centers in Russia. Numbers of patients included into was analysis of safety and tolerability were 1277 and 1121, respectively. After 3 months systolic, diastolic and pulse blood pressure (BP) decreased by 20.2, 13.2, 27.5% (supine) and by 19.4, 11.8, 26.9% (standing), respectively. There were no significant changes of heart rate. Effect of treatment was considered positive in 92.4% of patients. Normalization of blood pressure occurred in 51.8% of patients (in 46.2% and 53.4% among men and women, respectively, p=0.0252; in 55.7% and 48% among patients aged <65 and >65 years, respectively). In patients with type II diabetes rates of positive effect and achievement of target BP <130/85 mm Hg were 60.8% and 31.4%, respectively. Hypokaliemia (3.0-3.5 mmol/l) was registered in 43 patients (3.4%), age of 27 of these patients was 65 years. There were no pronounced changes of blood serum levels of creatinine, glucose and uric acid. Significant lowering of atherogeneity cholesterol index occurred in the whole group while both this index and total cholesterol significantly decreased in patients with baseline hypercholesterolemia. Thus in patients older that 55 years monotherapy with retard form of indapamide was demonstrated to be safe and effective antihypertensive intervention.

The trial enrolled 119 women with systemic undifferentiated dysplasia of the connective tissue (DCT) aged 14-39 years; 86.6% of them had asthenic constitution. Ultrasound most frequently found prolapse of mitral cusps and accessory chord as in the left ventricle. Use of chest x-ray, echocardiography, ECG 24-h monitoring and transesophageal pacing discovered combination of heart dislocation, changed intracardiac structures, disturbed rhythm and conduction in 87 examinees. This provided the diagnosis of dysplastic heart syndrome. Multiple significant correlations between echocardiometric parameters, hormonal indices, neuroendocrine disorders, reproductive disorders point to close relations between cardiovascular and neuroendocrine systems in women with DCT.

RESUMO

The study covered 119 women with systemic undifferentiated dysplasia of the connective tissue (DCT) aged 14-39 years. All the examinees had vegetative disorders, 26.1% had a neuroendocrine-metabolic form of hypothalamic syndrome (HS), 51.3%--premenopausal syndrome, 65.5%--dysmenorrhea. It was found that women with DCT and HS develop symptoms of neuroendocrine disorders at earlier age than those with HS but free of DCT. Arterial hypertension was registered in 96.8% of patients with DCT and HS, 80.3% of DCT patients with premenopausal syndrome. Central hemodynamics was different in DCT patients without neuroendocrine disorders and with neuroendocrine dysfunction evidencing the role of the latter in development of arterial hypertension in DCT.

AIM: It is the fragment of the trial aimed at the study of demographic indices and components of cardiovascular risk in patients over 55 with known and newly diagnosed arterial hypertension (AH) by referral to outpatient clinic depending on AH type. MATERIAL AND METHODS: For a week, 140 therapists from 14 regions of the Russian Federation measured AP in all persons aged 55 and older visiting outpatient clinics. A total of 5582 persons were examined. 3847 of them were already diagnosed to have hypertension. Primary diagnosis of hypertension was made in 5.7 examinees out of 2442. RESULTS: Patients with previous diagnosis of hypertension vs those with primary diagnosis had significantly greater index of body mass, had more often overweight, obesity and diabetes mellitus. Isolated systolic AH was encountered in 56% of all the primary cases. High and very high risk to develop cardiovascular complications was encountered in 59.9% of primary patients and 85.8% of those previously diagnosed. CONCLUSION: Patients aged 55 and older with newly diagnosed isolated systolic AH had the following most common factors of risk of cardiovascular complications: overweight, obesity, smoking, dyslipidemia, diabetes mellitus. In treated patients these risk factors are: overweight, obesity, dyslipidemia, diabetes mellitus, smoking.

RESUMO

Patients with chronic gingivitis and connective tissue dysplasia developed more marked dystrophic process in the gingival epithelium than those with chronic gingivitis without general somatic diseases. These dystrophic processes substantially increase the barrier properties of the gingival epithelium.

RESUMO

The aim of our investigation was assessment of enalapril effects in cor pulmonale. 20 patients with chronic obstructive pulmonary diseases complicate with cor pulmonale received combined treatment including enalapril. The course of such treatment led to a significant lowering of systolic and diastolic pressure in the pulmonary artery, systemic arterial pressure, specific peripheral resistance and minute volume in patients with hyperkinetic and mixed types of hemodynamics. Parameters of ECG and exercise tolerance improved. Enalapril is effective in correction of pulmonary and central hemocirculation in chronic cor pulmonale.

RESUMO

The prevalence and patterns of the hypertension syndrome (AH) in young subjects require further study. An examination of 879 young subjects revealed AH in 14.6 per cent; in 84 inpatients borderline AH was found in 20.2, essential hypertension in 17.9 and symptomatic AH in 61.9 per cent, in 22.6 per cent AH was attended by signs of connective-tissue dysplasia and minor developmental abnormalities (mitral valve prolapse, nephroptosis, pathology of cervical spine, structural disorders of the lid slit and floor of the auricle, etc.). Manifestations of connective-tissue dysplasia and minor developmental abnormalities have a bearing on the AH formation and course in this group of patients. Further detailed study of the AH course in young subjects with these manifestations is required.

RESUMO

The cholesterol content in blood serum and lipoprotein fractions was studied in 190 randomly selected and 386 specially selected newborns. There were no immature infants in either group. The content of cholesterol and triglycerides hardly differed in the two groups. Hypercholesterolemia (cholesterol above 118 mg/100 ml) was revealed in 9% of the infants. Analysis of cholesterol distribution in lipoprotein fractions showed no difference between boys and girls in these indices. In 11% of children hypercholesterolemia was accompanied by hypoalphalipoproteinemia. Hyperbetalipoproteinemia (cholesterol of low density lipoproteins above 64 mg/100 ml) was revealed in 7% of infants, in 10 of them in the presence of hypercholesterolemia. Newborns with a combination of hypercholesterolemia and hypoalphalipoproteinemia should evidently be kept under surveillance for many years during which preventive measures are applied.

The authors examined specially selected 165 practically healthy males and 373 patients with ischemic heart disease from 20 to 59 years of age. Normolipoproteinemia (total cholesterol (CS) less than or equal to 260 ml/dl, triglycerides (TG) less than or equal to mg/dl, CH of very low density lipoproteins less than or equal to 36 mg/dl, CS of low-density lipoproteins less than or equal to 190 mg/dl) was found in 172 males with ischemic heart disease (46%). The mean content in blood serum of TG and CS of very low density lipoproteins is higher and the concentration of CS of low density lipoproteins is significantly lower in patients than in healthy individuals. Antipyrin half-life in patients is significantly increased, while the amount of bile acids and concentration of CS in bile of individuals with normolipoproteinemia do not differ from the indices in healthy persons. A relationship was shown between the body mass/height index, antipyrin half-life and concentration of CS on low density lipoproteins.
