RESUMO

Intensity of bed utilization in Croatia's hospitals in the peace-time period 1986-90 and the war year of 1991 was evaluated. The study covered 32 general-and-teaching hospitals disposing with 21,300 beds in 1986-90 and 27 of these hospitals with 19.000 beds in 1991. Basic information derives from hospital's statistical reports analyses by the Barber-Johnson's technique. While length of stay was reduced in the 1986-90 period, average number of discharges and deaths per available beds increased (1986/26.1; 1990/26.9). In the peace year of 1990 the performances of 11 of the 32 hospitals grouped in a zone limited to 10.0-12.9 days for the length of stays and 1.1-2.5 days for turnover interval. In the war year of 1991 the performances of 10 of the 27 hospitals were grouped in a zone defined by 8.5-10.9 days for length of stay and by 3.1 and 6.2 days for turnover interval. The Barber-Johnson's analysis provides key information in the decision making about hospital beds utilization, as well as in the evaluation of such decisions.

RESUMO

Mortality trends and distribution in a system of 32 general and teaching hospitals in the Republic of Croatia for the years 1966 through 1989, separately for 1986/1987, were evaluated as a possible informational support to the national policies regarding developing of health care system. By means of 13 elaborated and/or selected indicators, using hospital health statistics as a data base, we measured the changes in the dynamics, structure and level of death at institutions, individual medical departments and among patients. Hospitals mortality statistics was found to offer a good informational orientation for monitoring realization of the hospital's restructuring activities envisaged under the Aim 29 of the Croatian strategy Health for All by the Year 2000 as well as for planning technical surveillance and streamlining the special statistical studies on inpatient health care. It can also provide a solid base for the evaluation of the quality of inpatient medical care, if the diagnosis of causes of death is improved, methodology for sentinel-health-events adopted, commissions for the evaluation of inpatient health care organized, medical registration units established and the National Register of Autopsy begun.

In order to identify the presence of minor anatomical deformities responsible for the incongruity of the hip joint which causes the degenerative changes, the radiographies of the hips in 332 normal adults were reviewed. The deformities included the acetabular dysplasia and the tilt deformity of the femoral head as the residual condition of epiphyseolysis in adolescence. Using the Wiberg's CE angle acetabular dysplasia was found in 8.6% of cases: the tilt deformity, using the technique by Murray, was confirmed in 9.8%. It is concluded that these minor abnormalities can be recognized from more accurate examination of pelvic radiographies and their presence should indicate the joint incongruity and potential osteoarthritis changes. Earlier recognition of these deformities may permit earlier and simple operative measures and the arrest of the decline of the joint.

RESUMO

A comparative analysis of two angiography methods applied in two unrelated groups of patients with brain ischemia symptoms is described. The first group of 452 patients suffering from extracranial cerebral artery disease was analyzed by an intravenous digital subtraction angiography (DSA); the second group of 500 patients was subjected to a conventional aortic arch angiography. Research was based on comparative analysis of angiograms as per type and location of morbid changes. In the area of common carotid artery and internal carotid artery the stenosis was considerably more frequently detected by the conventional angiography than by the intravenous DSA, i.e. in 105 (20.0%) as compared to 40 (8.8%) patients, respectively. Similar difference is indicated in a total number of morbid changes in these arteries detected in 248 patients (49.6%) by aortic arch angiography and in 154 (34.1%) patients by intravenous DSA. The dissimilarity is noticed with minor atherosclerotic changes, while medium and major stenoses are detected with equal frequency by both methods. The intravenous DSA more often indicated flexion of carotid artery, and considerably more often the vertebral artery flexion as found in 53 (11.7%) patients analyzed by intravenous DSA compared to 33 (6.6%) patients analyzed by conventional angiography. Advantage of intravenous DSA is less complications and ambulatory performance. The conventional angiography better indicates morbid changes in common carotid artery bifurcation and vertebral artery origin.
