BACKGROUND: Local preparation of rectum governs the possibly survenue of complications during transrectal ultrasound guided biopsies of prostate. OBJECTIVE: To determine the efficacy and acceptability of rectal preparation (Eductyl(®) suppository) in patients undergoing a transrectal prostate biopsy. PATIENTS AND METHODS: From May to August 2005, eight urologists (Bel-Air Urological Center, Bordeaux) included 137 patients (mean age 66.4 years) with an indication of prostate biopsies. All patients were administrated prophylactic antibiotic therapy. They used Eductyl(®) effervescent suppositories for local preparation, the day before and/or the morning of the exam. RESULTS: Rectal vacuity was satisfactory or very satisfactory for 99% of patients. Introduction of probe and tracking of prostate were easy or very easy in 99% of cases. Eight days after the exam, only 35% of patients had suffered anal or rectal pain during a mean of 2 days. Most of patients recovered bowel function without requiring treatment and without any difficulty or pain. The bowel function recovery occurred the day of prostate biopsy and the day after for 28.4% and 62.2% of patients, respectively. CONCLUSION: Urologists considered that the use of Eductyl(®) suppositories before prostate biopsies ensured a satisfactory rectal preparation and optimal conditions for the exam. Moreover, this preparation was well accepted by patients.

Renal primary neuroectodermal tumours (PNET) are exceptional and belong to the vast group of peripheral malignant primary neuroectodermal tumours (MPNT), the best known forms of which are Ewing's sarcoma of bone and Askin's thoracopulmonary tumour. In the light of a case, the authors discuss the clinical features, course, treatment and pathological characteristics, particularly the value of immunohistochemistry and cytogenetics, which demonstrate a specific chromosomal abnormality, the t (11;22) (q24;q12) translocation.

Carcinoma of the papillary ducts of Bellini is a rare malignant tumour of the kidney, with a generally unfavourable prognosis because of late diagnosis, often at the state of metastases. The diagnosis is based on pathological examination of the nephrectomy specimen with immunohistochemical study. The role of adjuvant chemotherapy needs to be evaluated.

Hormone therapy for prostate cancer is associated with a number of adverse effects including hot flushes, which constitute a significant problem as they sometimes interfere with everyday life. Every effort must be made to relieve patients complaining of this symptom. Several different therapeutic modalities are proposed.

In the light of a case report, the authors recall the clinical signs of adenocarcinoma of the rete testis and the precise histological and immunohistochemical features allowing confirmation of this very rare diagnosis. They emphasize the persistent controversies concerning the treatment of this testicular tumour.

The authors report a series of 31 hydroceles (26 patients) treated by drainage followed by sclerosing injection of minocycline hydrochloride. The pain usually experienced with this injection required the use of general anaesthesia in 38.70% of cases and local-regional anaesthesia in 41.94% of cases. The patients were able to return to work on the day after the procedure, but after 6 months, only 51.61% of them were free of recurrence of the effusion. Patients must therefore be warned about the inconstant results of this technique.
