Introducción: Las técnicas mínimamente invasivas son las preferidas para tratar las roturas agudas del tendón de Aquiles. Representan una opción para evitar las complicaciones tegumentarias, y la lesión del nervio sural es uno de sus principales problemas. El objetivo de este estudio fue comprobar la utilidad de la ecografía para prevenir la lesión del nervio sural durante la reparación del tendón de Aquiles con técnicas percutáneas. materiales y métodos: Estudio en 12 piezas cadavéricas. Se recreó una lesión en el tendón de Aquiles 5 cm proximales de su inserción distal. En uno de los miembros del cadáver, se identificó el nervio sural o su vena satélite mediante ecografía. Se reparó el nervio sural por vía percutánea con dos agujas proximales y dos agujas distales a la lesión, y se representó el recorrido del nervio sural. En el miembro contralateral, no se identificó el nervio sural mediante ecografía. Se efectuó la reparación percutánea de las lesiones mediante la técnica de Ma y Griffith. Resultados: En el grupo ecográfico, no se identificaron lesiones del nervio sural. En el grupo de control, se observaron dos lesiones del nervio sural (p = 0,6). En todos los casos, la identificación del nervio sural mediante ecografía fue correcta. Conclusión: La asistencia ecográfica en el tratamiento percutáneo de las lesiones del tendón de Aquiles es un método eficaz y confiable para evitar las lesiones del nervio sural. Introduction: Minimally invasive techniques are preferred to treat acute Achilles tendon ruptures. They represent an option to avoid integumentary complications, and sural nerve injury is one of its main problems. This study aims to verify the usefulness of ultrasound in preventing sural nerve injury during Achilles repair with percutaneous techniques. materials and methods: Study in 12 cadaveric pieces. We recreated an injury at the level of the Achilles tendon, 5 cm proximally to its distal insertion. In one of the cadaver limbs, the sural nerve and/or its satellite vein were identified by ultrasonography. We repaired the sural nerve percutaneously with two needles at the proximal level and two needles at the distal level of the lesion and represented the path of the sural nerve. In the contralateral limb, the sural nerve was not identified by ultrasound. We performed the percutaneous repair of the injuries using the Ma & Griffith technique. Results: In the ultrasound group, no sural nerve injuries were identified. In the control group, two sural nerve injuries were observed (p=0.6). In all cases, the identification of the sural nerve by ultrasound was correct. Conclusion: Ultrasound assistance in the percutaneous treatment of Achilles tendon injuries is an effective and reliable method to prevent sural nerve injuries. Level of Evidence: III

RESUMEN Introducción: el síndrome de Burnout es una enfermedad mental causada por el estrés laboral, situación ampliamente asociada con los médicos y otros profesionales de salud, asimismo, el abuso de sustancias y la depresión también son problemas psiquiátricos que están relacionados con este ámbito. Objetivo: identificar la frecuencia de estudiantes de medicina del área clínica de la Universidad Nacional de Asunción con sospecha de Síndrome de Burnout y trastorno de abuso de alcohol. Metodología: estudio observacional, descriptivo, de corte transversal en estudiantes de medicina del área clínica de la Universidad Nacional de Asunción de Paraguay. Se utilizaron encuestas con la prueba de identificación de desórdenes de uso del alcohol (AUDIT-C), el inventario de Maslach y el screening de depresión PHQ-2. Resultados: de las 157 encuestas, el 43,9% cumplió con los criterios de Síndrome de Burnout, el 49% cumplió los criterios de abuso/dependencia de alcohol y el 38,9% tuvo criterios de alta probabilidad de trastorno depresivo mayor, de ellos la mayoría presentó ideación suicida en los últimos 12 meses. Conclusión: los resultados fueron parecidos a los hallados en la literatura, sin embargo, ciertos aspectos fueron mayores en este estudio, como el porcentaje que cumple los criterios de Síndrome de Burnout, abuso de sustancias e ideación suicida. El Síndrome de Burnout y la dependencia al alcohol son frecuentes en estudiantes de medicina y se asocian comúnmente a trastornos depresivos.


ABSTRACT Introduction: Burnout Syndrome is a mental disorder caused by occupational stress, a situation widely prevalent in doctors and other health professionals. Substance abuse and depression are also psychiatric problems that are related to this area. Objective: To identify the frequency of Burnout Syndrome and its association with Alcohol misuse in medical students in their clinical years at the National University of Asunción. Methodology: Observational, descriptive, cross-sectional study in medical students from the clinical area of the National University of Asunción, Paraguay. Surveys were used with the alcohol use disorder identification test (AUDIT-C), the Maslach inventory and the PHQ-2 depression screening. Results: Of the 157 surveys, 43.9% met the criteria for Burnout Syndrome, 49% met the criteria for alcohol misuse / dependence and 38.9% had high probability crite-ria for major depressive disorder. Most of them presented suicidal ideation in the last 12 months. Conclusion: The results were similar to those found in the literature, however, certain aspects were greater in this study, such as the percentage that meets the criteria for Burnout Syndrome, alcohol misuse and suicidal ideation. Burnout Syndrome and alcohol misuse are frequent in medical students and are commonly associated with depressive disorders.

La otitis media crónica es una inflamación crónica del mucoendostio del oído medio y anexos, con o sin perforación de la membrana del tímpano, con o sin exudado. Según los datos obtenidos por la Organización Mundial de la Salud posterior al análisis de trabajos de investigación publicados, la prevalencia para Sudamérica y América Central se encuentra alrededor del 1,5%. El Paraguay no cuenta con un estudio similar al presente que haya sido publicado. Por tal motivo nos propusimos a determinar la carga de la enfermedad y las características sociodemográficas y clínicas de los pacientes con otitis media crónica operados en el Hospital de Clínicas en el período 2015-2019. Se realizó un estudio observacional, descriptivo de corte transverso, retrospectivo. De 41885 pacientes operados (9,42%) fueron pacientes de la Cátedra y Servicio de Otorrinolaringología y de estos, 274 tenían otitis media crónica, lo que representa el 6,94% del total de pacientes operados por la Cátedra y Servicio de Otorrinolaringología y el 0,65% del total de pacientes operados en el Hospital en el mencionado periodo de tiempo. Pudimos concluir que la otitis media crónica es una patología de alta prevalencia para el servicio de otorrinolaringología del Hospital de Clínicas, no obstante, representa una baja carga para la administración hospitalaria total.


Chronic otitis media is a chronic inflammation of the mucoendostium of the middle ear and annexes, with or without perforation of the eardrum, with or without exudate. According to data obtained by the World Health Organization after the analysis of published research papers, the prevalence for South America and Central America is around 1.5%. Paraguay does not have a study similar to this one that has been published. For this reason, we set out to determine the burden of the disease and the sociodemographic and clinical characteristics of patients with chronic otitis media operated at the Hospital of Clinics in the period 2015-2019. An observational, descriptive, cross-sectional, retrospective study was conducted. Of 41885 patients operated 9.42% were patients of the Chair and Department of Otolaryngology and of these, 274 had chronic otitis media, which represents 6.94% of the total of patients operated by the Chair and Department of Otolaryngology and 0,65% of the total of patients operated in the Hospital in the mentioned period of time. We could conclude that chronic otitis media is a pathology of high prevalence for the otolaryngology service of the Hospital of Clinics, however, it represents a low burden for total hospital administration.

The first challenge in the 2014 competition launched by the Teach-Discover-Treat (TDT) initiative asked for the development of a tutorial for ligand-based virtual screening, based on data from a primary phenotypic high-throughput screen (HTS) against malaria. The resulting Workflows were applied to select compounds from a commercial database, and a subset of those were purchased and tested experimentally for anti-malaria activity. Here, we present the two most successful Workflows, both using machine-learning approaches, and report the results for the 114 compounds tested in the follow-up screen. Excluding the two known anti-malarials quinidine and amodiaquine and 31 compounds already present in the primary HTS, a high hit rate of 57% was found.

Identifying distinct anatomical structures within the brain and developing genetic tools to target them are fundamental steps for understanding brain function. We hypothesize that enhancer expression patterns can be used to automatically identify functional units such as neuropils and fiber tracts. We used two recent, genome-scale Drosophila GAL4 libraries and associated confocal image datasets to segment large brain regions into smaller subvolumes. Our results (available at https://strawlab.org/braincode) support this hypothesis because regions with well-known anatomy, namely the antennal lobes and central complex, were automatically segmented into familiar compartments. The basis for the structural assignment is clustering of voxels based on patterns of enhancer expression. These initial clusters are agglomerated to make hierarchical predictions of structure. We applied the algorithm to central brain regions receiving input from the optic lobes. Based on the automated segmentation and manual validation, we can identify and provide promising driver lines for 11 previously identified and 14 novel types of visual projection neurons and their associated optic glomeruli. The same strategy can be used in other brain regions and likely other species, including vertebrates.
