RESUMO

A numerical and analytical study was conducted to investigate the bifurcation mechanisms that cause desynchronization between the soliton repetition frequency and the frequency of external pulsed injection in a Kerr cavity described by the Lugiato-Lefever equation (LLE). The results suggest that desynchronization typically occurs through an Andronov-Hopf (AH) bifurcation. Additionally, a simple and intuitive criterion for this bifurcation to occur is proposed.

BACKGROUND: Overall, more than 50% of international travelers develop symptoms while traveling and 55% of them seek medical assistance during the trip. We conducted a study to evaluate the usefulness of a Smartphone app called TRIP Doctor® to provide telemedicine to international travelers. METHODS: Participants over 18 years old attending our travel clinic at Hospital Clinic in Barcelona were invited to participate during 2017-2019. After downloading the app, the health status of the traveler was monitored on a daily basis, providing specific medical advice and offering remote contact with specialized physicians through an integrated chat, if needed. RESULTS: From 449 users, 59 (13%) contacted for medical assistance through the app during the trip. Main reasons for telemedicine were diarrhea (25.7%), skin conditions (19.7%) and fever (12.1%). Among patients who contacted, 90% of the travelers did not require to be referred to a local doctor. Symptomatic treatment was the main treatment prescribed (38%). In a 14.7% of the cases a follow-up was not required, a 63.2% recovered and 22.1% were loss of follow-up. After a multivariate analysis, duration of trip >14 days was found to be the only factor associated with the use of telemedicine (OR 2.2, CI 95% 1.1-4.5, p = 0.03). CONCLUSION: In conclusion, travelers using telemedicine travelled for longer periods of time and mostly contacted for mild symptoms which could be solved successfully by remote assistance with our specialized doctors.

RESUMO

We numerically analyze a delay differential equation model of a short-cavity semiconductor laser with an intracavity frequency-swept filter and reveal a complex bifurcation structure responsible for the asymmetry of the output characteristics of this laser. We show that depending on the direction of the frequency sweep of a narrow-band filter, there exist two bursting cycles determined by different parts of a continuous-wave solutions branch.

In this Letter, we study theoretically a new setup allowing for the generation of temporal localized states (TLSs) and frequency combs. The setup is compact (a few centimeters) and can be implemented using established technologies, while offering tunable repetition rates and potentially high power operation. It consists of a vertically emitting micro-cavity, operated in the Gires-Tournois regime, containing a Kerr medium strong time-delayed optical feedback, and detuned optical injection. We disclose sets of multistable dark and bright TLSs coexisting on their respective bistable homogeneous backgrounds.

Using a delay differential equation model we study theoretically the dynamics of a unidirectional class-A ring laser with a nonlinear amplifying loop mirror. We perform linear stability analysis of the continuous-wave regimes in the large delay limit and demonstrate that these regimes can be destabilized via modulational and Turing-type instabilities, as well as by an instability leading to the appearance of square-waves. We investigate the formation of square waves and mode-locked pulses in the system. We show that mode-locked pulses are asymmetric with exponential decay of the trailing edge in positive time and faster-than-exponential (superexponential) decay of the leading edge in negative time. We discuss asymmetric interaction of these pulses leading to a formation of harmonic mode-locked regimes.

Among the various methods of treating coxalgia, the minimally invasive method of radiofrequency denervation of the sensitive nerves of the hip joint is becoming increasingly popular. Since the RFA technique is gradually beginning to be actively used in the practice of pain treatment, there is an urgent need for further rehabilitation of such patients in order to prolong the positive effect of the RFA procedure. The aim of the study was to develop a program of physiotherapeutic exercises for patients with coxarthrosis of the 3-4 stage, who underwent the RFA procedure of articular branches of the obturator and femoral nerves. The developed author's program was tested on 36 patients (37 joints) with coxarthrosis of 3-4 stages, who underwent outpatient treatment during 2017-2018. in the rehabilitation department of the State Institution "Institute of Traumatology and Orthopedics of the National Academy of Medical Sciences of Ukraine". The dynamics of the treatment results were evaluated after 2 weeks, 1, 3, 6 and 12 months after the procedure. Quantitative and qualitative assessment of pain was carried out on the basis of VAS pain. Joint functional limitations were measured using a Harris Hip Score (HHS). The use of physiotherapeutic exercises in patients who underwent RFA procedure n. femoralis and n. obturatorius positively affects the functional capabilities of the joints, which is confirmed by the results of studies on the Harris Hip SCOR and VAS questionnaire at all stages of observation. The use of physiotherapeutic exercises in combination with the RFA procedure of the femoral and obturator nerves in coxarthrosis can lead to a decrease in pain and stiffness, easier movements and increased flexibility, as well as maintaining the effectiveness of the RFA procedure for a longer period.

Recently, a mechanism of formation of light bullets (LBs) in wide-aperture passively mode-locked lasers was proposed. The conditions for existence and stability of these bullets, found in the long cavity limit, were studied theoretically under the mean field (MF) approximation using a Haus-type model equation. In this paper, we relax the MF approximation and study LB formation in a model of a wide-aperture three section laser with a long diffractive section and short absorber and gain sections. To this end, we derive a non-local delay-differential equation (NDDE) model and demonstrate by means of numerical simulations that this model supports stable LBs. We observe that the predictions about the regions of existence and stability of the LBs made previously using MF laser models agree well with the results obtained using the NDDE model. Moreover, we demonstrate that the general conclusions based upon the Haus model that regard the robustness of the LBs remain true in the NDDE model valid beyond the MF approximation, when the gain, losses and diffraction per cavity round trip are not small perturbations anymore.This article is part of the theme issue 'Dissipative structures in matter out of equilibrium: from chemistry, photonics and biology (part 1)'.

RESUMO

Trip Doctor®, a Smartphone-based app monitoring system, was developed to detect infections among travelers in real-time. For testing, 106 participants were recruited (62.2% male, mean age 36 years (SD = 11)). Majority of trips were for tourism and main destinations were in South East Asia. Mean travel duration was 14 days (SD = 10). Diarrhea was the most frequently reported symptom (15.5%). The system demonstrated adequate usability and is ready to be used on a larger scale.

We investigate the spatiotemporal dynamics of cavity solitons in a broad area vertical-cavity surface-emitting laser with saturable absorption subjected to time-delayed optical feedback. Using a combination of analytical, numerical, and path continuation methods, we analyze the bifurcation structure of stationary and moving cavity solitons and identify two different types of traveling localized solutions, corresponding to slow and fast motion. We show that the delay impacts both stationary and moving solutions either causing drifting and wiggling dynamics of initially stationary cavity solitons or leading to stabilization of intrinsically moving solutions. Finally, we demonstrate that the fast cavity solitons can be associated with a lateral mode-locking regime in a broad-area laser with a single longitudinal mode.

We study the dynamics of an array of nearest-neighbor coupled spatially distributed systems each generating a periodic sequence of short pulses. We demonstrate that, unlike a solitary system generating a train of equidistant pulses, an array of such systems can produce a sequence of clusters of closely packed pulses, with the distance between individual pulses depending on the coupling phase. This regime associated with the formation of locally coupled pulse trains bounded due to a balance of attraction and repulsion between them is different from the pulse bound states reported earlier in different laser, plasma, chemical, and biological systems. We propose a simplified analytical description of the observed phenomenon, which is in good agreement with the results of direct numerical simulations of a model system describing an array of coupled mode-locked lasers.

The objective of the present study was to elucidate the anatomical features of the structure of the middle ear and eustachian tube in the breast-fed infants of different gestational age that may be responsible for the formation of exudates (fluids). We have examined 150 temporal bones obtained from the children's cadavers that were allocated to three groups as follows: 50 temporal bones obtained at weeks 26-30 weeks of gestation (group 1), 44 bones 31-36 weeks of gestation (group 2), and 37-40 weeks of gestation (full-term babies, group 3),The analysis of the data obtained on an individual bases revealed either increase or decreases in the selected characteristics of the eustachian tube in comparison with the respective average values as well as the well apparent predominance of a single change or a combination of alteration of several parameters in one case in 26-30 weeks and 31-36 weeks groups. No significant changes were found in group 1. It is concluded that the presence of a single change or a combination of two or three abnormal changes in the parameters of the bone structures of the eustachian tube can affect the development of the secretory process in the middle ear especially in the children born after 36 weeks of pregnancy.

The porous titanium implants were introduced into the condyles of tibias and femurs of sheep. New bone tissue fills the pore, and the porous titanium-new bone tissue composite is formed. The duration of composite formation was 4, 8, 24 and 52 weeks. The formed composites were extracted from the bone and subjected to a compression test. The Young's modulus was calculated using the measured stress-strain curve. The time dependence of the Young's modulus of the composite was obtained. After 4 weeks the new bone tissue that filled the pores does not affect the elastic properties of implants. After 24 and 52 weeks the Young's modulus increases by 21-34% and 62-136%, respectively. The numerical calculations of the elasticity of porous titanium-new bone tissue composite were conducted using a simple polydisperse model that is based on the consideration of heterogeneous structure as a continuous medium with spherical inclusions of different sizes. The kinetics of the change in the elasticity of the new bone tissue is presented via the intermediate characteristics, namely the relative ultimate tensile strength or proportion of mature bone tissue in the bone tissue. The calculated and experimentally measured values of the Young's modulus of the composite are in good agreement after 8 weeks of composite formation. The properties of the porous titanium-new bone tissue composites can only be predicted when data on the properties of new bone tissue are available after 8 weeks of contact between the implant and the native bone.

Nonlinear polaritons in microcavity wires are demonstrated to exhibit multi-stable behavior and rich dynamics, including filamentation and soliton formation. We find that the multi-stability originates from co-existence of different transverse cavity modes. Modulational stability and conditions for multi-mode polariton solitons are studied. Soliton propagation in tilted, relative to the pump momentum, microcavity wires is demonstrated, and a critical tilt angle for the soliton propagation is found.

OBJECTIVE: Foetal CT has recently been added to the foetal imaging armamentarium, but this carries with it the risks of ionizing radiation, both to the mother and the foetus. Foetal "black bone" MRI is a new technique that allows assessment of the foetal skeleton without the risk of exposure to ionizing radiation and is a potential new sequence in foetal MRI examination. METHODS: Retrospective review of all foetal MRI studies over the past 4- to 5-year period identified 36 cases where susceptibility weighted imaging was used. Cases were selected from this group to demonstrate the potential utility of this sequence. RESULTS: This sequence is most frequently useful not only in the assessment of spinal abnormalities, most commonly the bony abnormalities in myelomeningocele, but also in cases of scoliosis, segmentation anomalies and sacrococcygeal teratoma. CONCLUSION: Although the utility of this sequence is still being evaluated, it provides excellent contrast between the mineralized skeleton and surrounding soft tissues compared with standard half Fourier acquisition single-shot turbo-spin echo sequences. Further assessment is required to determine whether black bone MRI can more accurately evaluate the level of bony defect in spina bifida aperta, an important prognostic factor. Potential further uses include the assessment of skeletal dysplasias, evaluation of the skull base and craniofacial skeleton in certain congenital anomalies and the post-mortem evaluation of the foetal skeleton potentially obviating the need for necropsy. ADVANCES IN KNOWLEDGE: Foetal black bone MRI can be performed using susceptibility weighted imaging and allows better demonstration of the mineralized skeleton compared with standard sequences.

We study the effect of noise on the dynamics of passively mode-locked semiconductor lasers both experimentally and theoretically. A method combining analytical and numerical approaches for estimation of pulse timing jitter is proposed. We investigate how the presence of dynamical features such as wavelength bistability in a quantum-dot laser affects timing jitter.

The effect of coherent single frequency injection on two-section semiconductor lasers is studied numerically using a model based on a set of delay differential equations. The existence of bistability between different continuous-wave and nonstationary regimes of operation is demonstrated in the case of sufficiently large linewidth enhancement factors.

We investigate a control of the motion of localized structures (LSs) of light by means of delay feedback in the transverse section of a broad area nonlinear optical system. The delayed feedback is found to induce a spontaneous motion of a solitary LS that is stationary and stable in the absence of feedback. We focus our analysis on an experimentally relevant system, namely the vertical-cavity surface-emitting laser (VCSEL). We first present an experimental demonstration of the appearance of LSs in a 80 µm aperture VCSEL. Then, we theoretically investigate the self-mobility properties of the LSs in the presence of a time-delayed optical feedback and analyse the effect of the feedback phase and the carrier lifetime on the delay-induced spontaneous drift instability of these structures. We show that these two parameters affect strongly the space-time dynamics of two-dimensional LSs. We derive an analytical formula for the threshold associated with drift instability of LSs and a normal form equation describing the slow time evolution of the speed of the moving structure.

Localized structures (LSs) in dissipative media appear in various fields of natural science such as biology, chemistry, plant ecology, optics and laser physics. The proposal for this Theme Issue was to gather specialists from various fields of nonlinear science towards a cross-fertilization among active areas of research. This is a cross-disciplinary area of research dominated by nonlinear optics due to potential applications for all-optical control of light, optical storage and information processing. This Theme Issue contains contributions from 18 active groups involved in the LS field and have all made significant contributions in recent years.

We investigate the behaviour of a short cavity swept source laser with an intra cavity swept filter both experimentally and theoretically. We characterise the behaviour of the device with real-time intensity measurements using a fast digital oscilloscope, showing several distinct regimes, most notably regions of mode-hopping, frequency sliding mode-locking and chaos. A delay differential equation model is proposed that shows close agreement with the experimental results. The model is also used to determine important quantities such as the minimum and maximum sweep speeds for the mode-locking regime. It is also shown that by varying the filter width the maximum sweep speed can be increased but at a cost of increasing the instantaneous linewidth. The consequent impacts on optical coherence tomography applications are analysed.
