RESUMO

Possibilities and late results of nonselective surgical reinnervation, using formation of primary or deferred anastomosis of n. laryngeal recurrence (NLR) with one of cervical peripheral nerves (proximal fragment of NLR, the main branch of cervical loop - ansa cervicalis, nerve-donor) in a one-sided laryngeal paralysis, were studied up. Preoperatively and postoperatively the indirect laryngoscopy, videolaryngoscopy, the voice spectral analysis (main frequency, intensity and the rate harmonics-noise RHN, maximal period of phonation--MPP) were accomplished, subjective selfestimation by a patient of the voice quality (VHI-30) was done. Anastomosis with NLR was formatted in 95 patients, including 53--with cervical loop, in 34 - NLR - NLR, 8 - NLR - nerve donor). Postoperative follow-up have constituted (12 +/- 1.8) mo at average, the period up to occurrence of the first indirect signs of laryngeal reinnervation- (4.5 +/- 2.9) mo, were observed in all the patients: occurrence of the vocal plica tone, reduction of the vocal fissure dimensions while phonation (peculiarly in anastomosis of NLR with cervical loop)--from (2.25 +/- 0.86) to (0.35 +/- 0.17) mm. In accordance to data of acoustic analysis, reinnervation is mostly effective in anastomosing of NLR with cervical loop, RHN increased from (12 +/- 3.7) to (24 +/- 2.4) (see symbol) MPP--from (7 +/- 1.22) to (16 +/- 3.52) c (p < 0.01). Improvement in all subgroups of the main group was noted while performing analysis of subjective psychosocial selfestimation of the voice formation.

The activity of interferon-induced 2',5'-oligoadenylatesynthetase (2',5'-OA-synthetase) in thymocytes of rats simultaneously treated with ethanol and acetic zinc preparation was studied. The 2',5'-OA-synthetase activity was decreased by 75% after daily ethanol administration for 14 days. The changes were less pronounced at the late terms of the experiment. In vitro stimulation of enzyme activity by inducer cycloferon was more pronounced at the late terms of ethanol action. Combined administration of ethanol and acetic zinc increased the 2',5'-OA-synthetase activity (by 81% at 21th day and by 30% at 28th day) in comparison to the group of animals that consumed ethanol only. The zinc preparation augmented the stimulation of enzyme activity induced by cycloferon, the effect was most prominent at the late terms of the ethanol action (at 28th day). Probably, ethanol depresses the synthesis of interferon or disturbs the functioning of 2',5'-oligoadenylate cascade. The acetic zinc amplifies the interferon synthesis.

We investigated the changes in key parameters of carbohydrate and lipid metabolism, which correspond to the clinical picture that accompanies the development of prediabetic condition on the background of chronic alcohol intoxication. From the analysis of glycemic curves obtained during the insulin-glucose test, a speed of glucose uptake by peripheral tissues increased at the 1st day (1.5 fold) and the third day (1.3 fold) of administration of alcohol solution. At the later periods, at 7 and 11 days of ethanol administration, a decreased rate of glucose uptake in animals with chronic alcohol intoxication was detected. We also detected an increased content of serotonin in the blood serum and a decreased (1.2 fold) serotonin content in rat brain during the whole period of development of chronic alcohol intoxication.
