The duration of gastric impairment before perforation as well as morphological features of sutured ulcer were shown to influence the protective functions of the gastric and duodenal mucosal membrane as shown by estimation of hexose, fucose and sialic acids in the blood serum and urine of 128 patients within later periods after suturing perforated gastroduodenal ulcers. Inhibition of the mucosal membrane protective functions in gastroduodenal tissues was increased with a longer disease period before perforation and with more distinct pathomorphological alterations of the perforated ulcer.

Estimation of protein-bound hexoses, fucose and sialic acids in the blood and urine of 64 patients with gastroduodenal ulcers within later periods after various types of gastric vagotomy showed that the denervation inhibited the protective barrier of gastric and duodenal mucosal membrane. Protective functions of gastroduodenal mucosal membrane were most distinctly inhibited after selective proximal vagotomy which may be responsible for higher amounts of recurrences of ulcerous disease after this type of vagotomy than other types of denervation.

[The effect of HBO therapy in the early postoperative period on the late outcomes of "compelled" suturing in perforated gastroduodenal ulcers]. / Vliianie GBO-terapii v rannem posleoperatsionnom periode na otdalennye iskhody "vynuzhdennogo" ushivaniia probodnykh gastroduodenal'nykh iazv.

Under study were long-term results of forced suturing of perforated ulcers of the stomach and duodenum in 96 patients. It was shown that in patients who were subjected to hyperbaric oxygenation in addition to medicamentous treatment at the early postoperative period excellent and good results were obtained two times more often and there was 1.5 time less amount of bad outcomes as compared with patients who were not given HBO therapy. A conclusion is made that HBO therapy has a positive influence at the early postoperative period on restoration of the disturbed balance in the system "aggression-protection" in ulcer disease which results in greater number of patients who recovered after suturing perforated gastroduodenal ulcers.

[The effect of smoking on the long-term outcome of closure of perforated gastric and duodenal ulcer]. / Vliianie kureniia na otdalennyi iskhod zashivaniia probodnoi iazvy zheludka i dvenadtsatiperstnoi kishki.

Examined were 164 patients at the long-term period after closure of a perforative ulcer of the stomach and duodenum. The negative effect of smoking on acid production and protective barrier of gastroduodenal mucosa leading to worsened healing of an ulcer closed and its recurrence in 50% of the patients operated on has been established. It was noted that in smokers, irrespective of a number of the cigarettes being smoked, an excellent and good long-term result was observed 2.5 times less often, and a poor one--3 times more often than in non-smokers.

The results of operative treatment of 20 patients with a perforated giant duodenal ulcer have been analysed. The difficulties associated with closure of a perforated hole and duodenal stump management in performance of gastric resection were noted. It was established that closure of a giant perforated duodenal ulcer was a palliative measure, which saved the patients from death but entailed the disease recurrence requiring reoperation.

Perforation of ulcer occurred after distinct impairments in protective mechanisms of gastric mucosal membrane; this suggestion was corroborated by studies of glycoprotein content in blood serum of patients with perforating gastroduodenal ulcer within the early postoperation period. Studies of the glycoprotein in blood serum and urine within the later periods after the operation showed that the mucosal protective barrier was completely restored in some patients, thus protecting against the ulcer exacerbation, while recurrence of the disease was observed in the patients with deteriorated protective mechanisms in gastric mucosal membrane.
