RESUMO

Interaction of glycoprotein (GP) IIb/IIIa with fibrinogen is the final and key reaction in platelet aggregation. In order to evaluate GP IIb-IIIa functional activity in patients with acute coronary syndrome (ACS) we measured platelet aggregation induced by monoclonal antibody CRC54 which is directed against GP IIb/IIIa and is able to stimulate its binding with fibrinogen and subsequent aggregation. Patients with ACS were divided into 3 groups: (1) with Q-wave myocardial infarction (MI), (2) with non Q-wave MI and with unstable angina. Patients with stable angina (SA) and healthy donors formed 2 comparison groups. The level and rate of CRC54-induced aggregation measured both in the absence and in the presence of prostaglandin E(1) (PGE(1)), the inhibitor of platelet activation, in all groups of patients with ACS were > or =1.5 times higher than in SA patients and healthy donors. Observed differences in the parameters of CRC54-induced aggregation at least in the presence of PGE(1) could be caused only by increased GP IIb/IIIa fibrinogen binding ability in ACS patients, but not by differences in the level of platelet activation. In all groups of patients with ACS, but not in SA patients and healthy donors, strong correlation (r=-0.5-0.7) was observed between increased parameters of ADP-induced aggregation and aggregation stimulated by CRC54 in the presence of PGE(1). The data obtained indicated that increased of platelet aggregating capacity in ACS might be caused by changes of GP IIb/IIIa functional characteristics and not by enhancement of platelet sensitivity towards physiological agonists including ADP.

RESUMO

A study was made of the influence of some plant lectins on the aggregation of neutrophils and erythrocytes in healthy humans, and the state of carbohydrate determinants of glycoprotein receptors of these cells was characterized. These carbohydrate determinants, containing D-mannose, N-acetyl-D-glucosamin, and bD-galactose, provide neutrophils aggregation in healthy persons. The surface receptors of erythrocytes have remains of bD-galactose, several N-acetyl-D-glucosamin, N-acetyl-neyranimic acid, N-acetyl-D-galactosamin, L-fucose. Thus, in neutrophils and erythrocytes of healthy persons there is a definite composition of carbohydrate determinants of glycoproteins. Changes in these carbohydrate determinants are able to increase cells aggregation and, consequently, to disturb reological property of the blood, and to impair processes of microcirculation and thrombose stimulation.

Functional activity of platelets (aggregation and plasma levels of soluble P-selectin) and functional state of endothelium (antiaggregatory activity of vessel wall - AAVW, cuff occasion test, and plasma content of von Willebrand factor) were assessed in patients with unstable (n=41) and stable (n=19) angina and healthy donors (n=20). According to results of follow up for 1 year patients with unstable angina (UA) were divided into 2 groups - with (n=21) and without (n=20) events (coronary death, nonfatal myocardial infarction). In patients with UA and events parameters of spontaneous and ADP-induced platelet aggregation, contents of P-selectin and von Willebrand factor were higher while AAVW lower than in UA patients without events, patients with stable angina and healthy subjects. Correlations between AAVW and platelet aggregation, between AAVW and level of von Willebrand factor were negative in patients with UA, and positive in patients with stable angina and healthy subjects. These results of correlation analysis were interpreted as a reflection of impairment of antithrombotic endothelial function in patients with UA.

RESUMO

Lectin-induced aggregation of neutrophilic granulocytes was studied in patients with unstable angina pectoris. PHA-P, WGA, ConA, LCL, PHA-L, PHA-E, PSL, HPL, and LABA were used as aggregation inductors. The results indicate restructuring of carbohydrate determinants of glycoprotein receptors in neutrophilic granulocytes which stimulates their aggregation.

It is necessary to study the membrane receptors of platelets to understand the mechanism of their aggregation for effective treatment of patients with ischemic heart disease (IHD). The aim of this paper is to estimate the significance of carbohydrate residues of the glycoprotein receptors of platelet membranes and thrombogenesis in patients with unstable angina (UA). A total of 142 patients with UA and 20 healthy individuals were examined. The carbohydrate structure of platelet membranes was studied using lectins. Lectin-dependent platelet aggregation in patients with US was found to be associated with the activation of intracellular systems and to be fibrinogen/GP IIb/IIIa-dependent. The patients with US had elevated levels of fusoco-, N-acetyl-D-galactosamine-, and mannosamine-specific carbohydrate residues onto the platelet membrane surface. Higher platelet capacity for agglutination and aggregation by altering the carbohydrate specificity of GP IIb/IIIa receptors may be one of the causes of thrombogenesis in patients with US.

RESUMO

Lectin-induced platelet aggregation was studied in patients with unstable angina pectoris treated with tiklid and aspirin. ADP, PHA-P, WGA, Con A served as the aggregation inductors. In unstable angina pectoris lectin-induced platelet aggregation was enhanced. Changes in this aggregation in response to treatment with antiaggregants with different mechanisms of action are demonstrated.

RESUMO

The study of relationships between the level of Willebrand's factor (WF) and vascular wall antithrombogenic activity (VWAA) in patients with unstable angina (UA) has demonstrated that NA patients have high WF and VWAA activity; that in the control group there is a positive correlation between WF and VWAA which reflects a normal mechanism of thrombogenesis prevention, while UA patients have this correlation negative which may indicate impairment of mechanisms of vascular-thrombocytic hemostasis.

AIM: To evaluate effects of tiklid and aspirin on hemostasis, antithrombogenic activity of vascular wall in patients with unstable angina (UA). MATERIAL AND METHODS: An open randomized trial enrolled 30 UA patients given tiklid (n = 16) or aspirin (n = 14). Hemostasis, platelet aggregation, antithrombogenic activity of the vascular wall were examined on the treatment day 1 and 20. The response was assessed by the number of anginal attacks, dose of nitroglycerine, data of Holter monitoring. RESULTS: Tiklid was more effective than aspirin in UA patients as it by the treatment day 20 lowered platelet aggregation more, higher elevated anticoagulation potential of the blood and vessels, and stronger activated fibrinolytic hemostasis. CONCLUSION: Compared to aspirin, tiklid is more potent antiaggregant, has greater effect on antithrombogenic properties of the vascular wall.

RESUMO

An open randomized trial was performed of tiklid versus aspirin in patients with unstable angina pectoris (UAP). Tiklid was given to 28 and aspirin to 26 UAP patients. Hemostasis, platelet aggregation, antithrombogenic activity of the vascular wall were studied on the treatment day 1, 20, month 2 and 3. Clinical response was assessed also by the number of anginal attacks, dose of nitroglycerin, data of Holter ECG monitoring. Clinical effect of tiklid appeared higher than that of aspirin. 20-day treatment with tiklid produced a more pronounced decrease in platelet aggregation, and increase in blood and vascular wall anticoagulant potential as well as more potent fibrinolytic effect. These effects persisted for all the observation period. Thus, tiklid in UAP is more advantageous than aspirin because it has more potent antiaggregation action and corrects antithrombogenic properties of the vascular wall.

AIM: To examine antithrombogenic activity of the vascular wall, some hemostatic and rheological properties of blood in patients with unstable angina (UA) and various types of hyperlipoproteinemia (HLP). MATERIAL AND METHODS: 102 UA patients were divided into groups by HLP types according to D. S. Frideriksen. Antithrombogenic properties of blood vessel wall were examined with the cuff test, blood rheology--with AKP-2 analyser, hemostasis and lipid metabolism--with standard test. RESULTS: UA patients with HLP have low antithrombogenic activity of vascular wall. The most noticeable shifts to hypercoagulation were found in patients with HLP types 2a and 2b. Blood rheology was abnormal (high red cell aggregation in low deformability) in patients with HLP types 2a, 2b and 4. CONCLUSION: Patients with UA demonstrate relationships between antithrombogenic activity of vascular wall, intensity of lipid metabolism, hemostasis and blood rheology.

RESUMO

A study was made of the ability of some plant lectins, which bind specifically to different carbohydrate determinants of glycoproteins, to induce the platelet aggregation in healthy humans. It has been shown that phytohemagglutinin (PHA) and wheat germ agglutinin (MGA) induce a more marked platelet aggregation than concanvalin A (Con A). Lens culinaris agglutinin (LCA) had a slight aggregate activity. It was pointed at different roles played by carbohydrate determinants of platelet glycoproteins in fulfilling their aggregation function.
