RESUMO

We report on a patient with a de-novo deletion of chromosome 6p. This male infant presented with multiple systemic congenital defects together with an unusual ocular phenotype. Slit-lamp examination revealed thin, opaque, rectilinear bands within the anterior segment partially connecting iris to corneal endothelium. These were associated with bilateral hyperopia and optic nerve hypoplasia. Ocular abnormalities in such patients have been documented although the number of individuals is small and identical cytogenetic defects are rarely encountered. We compare the clinical findings in this case with previously described phenotypes. Characterisation of such cases is important as it is becoming apparent that deletion of genetic information encoded on chromosome 6p has implications for ocular embryogenesis.

RESUMO

One discussion within the field of family-oriented care concerns the distinction between those who practise a family approach and those who think in family system terms. This paper presents a comprehensive model of systemic thinking that may be useful for both family systems teachers and researchers. The conceptual model includes: (i) clinicians' experiences with and attitudes towards family aspects of health problems, and the context of clinicians' judgements about these problems; (ii) the way clinicians organize their knowledge about family factors and health problems in order for that knowledge to be readily available for decision making; and (iii) the interview sequences and hypothesis-generating or testing strategies used by clinicians in encounters with patients. The extent to which clinicians exhibit systemic thinking can be determined through interviews, questionnaires and case materials such as encounter transcripts. A method that assesses systemic thinking allows rigorous evaluations of family-oriented teaching efforts and clinical interventions.

It has been suggested that loss of erythrocytes after abdominal aortic grafting is influenced by the type of synthetic graft used. A prospective randomized study was done to compare loss of erythrocytes in patients receiving Dacron (polyester fiber, Meadox woven double velour) and Gore-Tex (polytetrafluoroethylene [PTFE]) grafts during the perioperative period. A total of 25 patients (13 Dacron and 12 PTFE) was studied, including 21 with abdominal aortic aneurysms and four with aortoiliac occlusive disease. Erythrocyte volume (EV) was measured using 51Cr-labeled autologous erythrocytes on the day prior to the operation, one to two hours after the operation when the patients were hemodynamically stable and 24 hours postoperatively. In addition to measurements of 51Cr EV and the volume of intraoperatively salvaged washed erythrocytes, the length of storage of the units of homologous liquid preserved erythrocytes at 4 degrees C. prior to transfusion were recorded. The mean intraoperative erythrocyte loss (+/- S.D.) for the Dacron group was 892 +/- 543 milliliters and for the PTFE group, 842 +/- 403 milliliters (p = NS). Patients in the Dacron group received intraoperatively 2.2 +/- 1.6 (units +/- S.D.) milliliters with a range of zero to 4 units of homologous liquid preserved erythrocytes and patients in the PTFE group received 1.2 +/- 1.2 milliliters with a range of zero to 3 units of homologous liquid preserved erythrocytes (p = NS). The mean total loss of erythrocytes (+/- S.D.) was 1,055 +/- 649 milliliters for the Dacron group and 978 +/- 503 milliliters for the PTFE group (p = NS). Despite inherent differences in graft material, there were no significant differences in intraoperative or post-operative loss of erythrocytes or in the number of homologous units of liquid preserved erythrocytes transfused with a p value of less than 0.05 considered significant.

RESUMO

The effects of color on children's food choices were investigated. Subjects were 120 children who were equally distributed among each of the combinations of age (5 vs. 9 years old), sex (male vs. female), food type (3 types of candies), and color (red, green, yellow, and orange) in a counterbalanced, factorial, analysis-of-variance design with repeated measures on subjects. A significant main effect for color indicated that children preferred foods that were red, green, orange, and yellow, in that order. Interpretation of this main effect was not interfered with by main effects or interaction terms involving age, sex, or food type, which served as alternative rival hypotheses.

RESUMO

The relation between understanding of friendship and asociality was examined in 91 girls and 104 boys ranging in age from 9 to 17 years. Consistent with previous research, older children had higher understanding of friendship scores than younger children, and girls had higher understanding of friendship scores than boys. The relation between understanding of friendship and asociality was significant only for boys. Difficulty in understanding components of friendship appears to be one correlate of boys' delinquent tendencies.

Inorganic arsenicals have been used in agriculture as pesticides or defoliants for many years and, in localized areas, oxides of arsenic have contaminated soils as a result of fallout from ore-smelting operations and coal-fired power plants. Use of inorganic arsenicals is no longer permitted in most agricultural operations, and recent air pollution controls have markedly reduced contamination from smelters. Thus, this paper will concentrate on the effect of past applications on arsenic accumulation in soil, phytotoxicity to and uptake by plants as influenced by soil properties, and alleviation of the deleterious effects of arsenic. Once incorporated into the soil, inorganic arsenical pesticides and arsenic oxides revert to arsenates, except where the soil is under reducing conditions. The arsenate ion has properties similar to that of orthophosphate, and is readily sorbed by iron and aluminum components. This reaction greatly restricts the downward movement (leaching) of arsenic in soils and the availability of arsenic to plants. Several methods of estimating plant available arsenic in soils have been developed. They involve extraction of the soil with reagents used to estimate phosphorus availability. This extractable arsenic is reasonably well correlated with reduced plant growth by, and plant uptake of arsenic. For most plants, levels of arsenic in the edible portion of the plant are well below the critical concentration for animal or human consumption, even when severe phytotoxicity occurs. Alleviation of arsenic phytotoxicity has been attempted by increasing the soil pH, by use of iron or aluminum sulfate, by desorbing arsenate with phosphate and subsequent leaching, and by cultural practices such as deep plowing. Only limited benefits have accrued from these procedures the cost of which is often prohibitively high. Since attempts to reduce arsenic toxicity have not been very successful, its excessive accumulation in soils should be avoided.
